Program Description:
The Second Chance Heroes: Veteran Employment and Support Services Program is dedicated to assisting returning veterans reintegrate into society. This initiative provides comprehensive support to veterans who are temporarily disadvantaged, ensuring they receive the respect and aid they deserve to achieve self-sufficiency through mentoring, vital training, work opportunities, and assistance in stabilizing home life.
Objective:
To facilitate comprehensive casemanagement and job development services for recently released veterans, ensuring their successful reintegration into the workforce and society, while enhancing their long-term economic self-sufficiency and residential stability.
Position Purpose:
To deliver high-quality casemanagement services to program participants, ensuring successful reintegration through effective employment and support services coordination. The role focuses on maintaining high standards in service delivery, reducing recidivism, and supporting veterans' stable and independent living.
Principal Responsibilities:
General CaseManagement:
Manage a caseload of at least 50 participants, overseeing intake, assessment, work readiness training, mentoring, job placement, and follow-up activities.
Ensure individualized employment plans are developed and coordinated with client and staff involvement.
Assist clients in securing necessary supportive services.
Documentation and Compliance:
Maintain complete and accurate records of participant progress towards goals and objectives, including development and renewal of Individual Service Plans.
Document all services provided in the Management Information System (MIS) database and ensure timely entry of case notes within 72 hours of contact.
Provide all required documentation in a timely manner, including client follow-ups, outcome evaluations, and other relevant reports.
Employment Development:
Provide career counseling, job coaching, and facilitate access to job skills training as outlined in employment plans.
Arrange interviews with employers and assist clients with job retention strategies.
Develop linkages with key resources such as HCPS, Career Source, Department of Rehabilitation, vocational schools, the VA, and other relevant social services and employment resources.
Community Engagement and Networking:
Actively represent the program in networking and public relations opportunities.
Develop and maintain relationships with community organizations, including small businesses, employers, civic clubs, and employment agencies.
Conduct workshops, mentoring sessions, and group training to deliver life skills and work readiness training.
Team Collaboration and Reporting:
Work collaboratively with other re-entry and community-based programs to ensure client compliance and successful participation.
Report directly to the lead casemanager and program manager, providing weekly status reports on the caseload.
Participate in regular staff meetings, training programs, and supervisory sessions.
Required Goals:
Enrollment Rate: Maintain an enrollment rate of 110% of the goal (28 clients).
Retention Rate: Ensure an 80% retention rate of clients in the program.
Participation Rate: Maintain a minimum monthly participation rate of 70%.
Employment Rate: 60% of enrollees must enter employment no later than 12 months after enrollment.
Employment Retention Rate: Maintain a post-employment retention rate of no less than 70%, measured three months after program exit.
Mentoring Rate: Maintain a mentoring rate of 30% each quarter.
Recidivism Rate: Keep recidivism rate no higher than 20% throughout the program duration.
Resource Development: Add at least two new participant resources each month.

The Pennsylvania State University, Center for Counseling and Psychological Services (CAPS), University Park, has an opening for a Mental Health CaseManager - Commonwealth Campuses-Senior Professional available beginning March 1, 2026. The purpose of this position is to improve continuity and coordination of mental health care for students at the Counseling Services of the Penn State Commonwealth Campuses.
The Commonwealth Campus CaseManager- Mental Health position provides consultation regarding psychological concerns. This position works closely with counselors, care teams, physicians, and students to ensure students have access to meet their needs including mental health, insurance, and other needs. This is a hybrid remote position, and the successful candidate must be located within Pennsylvania. This position reports to the Associate Director, Commonwealth Campuses, CAPS, University Park, and is a member of a multi-disciplinary team assisting the Commonwealth Campus Counseling service staff. Responsibilities include:
Work in collaboration with Commonwealth Campus counselors to facilitate an effective mental health care plan for students.
Assist with psychiatric hospitalization of students and liaison with local community hospitals and agencies for continuity of care.
Provide remote casemanagement appointments for Commonwealth Campus students to facilitate client connection to ongoing mental health services.
Responsible for casemanagement coordination; call insurance companies, perform online searches for services, communicate electronically with students regarding coverage and provider options.
Advocate for client access to mental health care and other needed services; explain services available to clients when .

POSITION OVERVIEW
The Social Worker is a year-round, certified position that is responsible for working closely with families, school counselors, teachers, students and school leader(s) to enhance the school's ability to meet its academic mission, especially where home, school, and community collaboration is key to achieving that mission. The work of the licensed Social Worker is to identify and support students and their families in eliminating barriers that impact learning.
Success in this role is measured by improving student social, emotional, behavioral, and academic outcomes and acting as a liaison with community agencies to ensure services.
This role reports directly to the Assistant Principal of Operations and will work regionally with the Manager of Student Support Services. The hours mirror the staff hours of the school the role is based.
SPECIFIC DUTIES
Conduct assessments of individuals and families with the goal of improving student social, emotional, behavioral and academic outcomes.
Conduct reliable and valid assessments of students and families to inform the design of interventions to remove barriers to learning.
Participate in the development of school-wide prevention, initiatives and policies related to attendance to both be proactive to truancy and determine barriers and reasons for excessive absences.
Participate in the development of school-wide prevention, initiatives and policies related to safety, violence prevention and positive culture, social/emotional health, and the welfare of students
Conduct home visits, when necessary, in partnership with the school counselor's plan for attendance interventions and supports.
Complete assessments of campus needs to prioritize service delivery
Provide consultation to school personnel, parents/guardians, and community partners for the purpose of enhancing their understanding of student needs, developing effective interventions, and building school capacity.
Act as a liaison with community agencies to ensure continuity of services in partnership with the school counselors.
Provide follow up crisis counseling and referrals to appropriate providers
Demonstrate the use of evidence informed practices in their interventions
Utilize data in assessing needs of, planning interventions for individual students, groups, and/or whole school population.
Maintain accurate, complete and punctual records, as well as safeguard the privacy and confidentiality of information as required by law, district / state policy, and licensing requirements.
Meet the minimum professional development requirements as set forth by the licensing board
Adhere to state and district mandating reporting requirements as they pertain to safety concerns.
Ensure that students and their families are provided services within the context of multicultural understanding and competence.
Utilize evidence-informed practices, skills, and techniques that reflect the understanding of the role of culture in the helping process
Empower students and families to gain access to and effectively utilize school and community resources.

Our current starting salary for a first-year school social worker is $50,000 and this would be adjusted for years of experience and the required Master's degree that is required for this position.
