The Direct Care RN performs routine level nursing work providing for the assessment, care and treatment of individuals who reside in a State Supported Living Center (SSLC) setting. Evaluates, treats and documents healthcare needs and/or issues and reports needs and/or issues to the appropriate person(s). Communicates with others, such as direct care staff, physicians/PCP, administration, and other nursing staff to ensure optimal care as part of the interdisciplinary team. Assists with services provided by LVNs and Certified Medication Aides (CMA), as needed, and makes recommendations for improvements. Essential Job Functions (EJFs):

May serve in an on-call status on a rotating schedule as required to meet operational needs of the facility.
Performs routine nursing tasks and procedures in accordance with HHSC, SSLC and state regulations, such as tracheostomy care, gastrostomy/jejunostomy care, wound care, breathing treatments. Collects specimens for laboratory examination and conducts appropriate screening tests when requested by lab personnel.
Administers medications, immunizations, and treatments and observes patients for unusual symptoms and reactions. Makes rounds and/or home visits as needed and works in other locations at the SSLC as needed. Monitors individuals' weight, diet, feedings, and vital signs. Monitors for signs and symptoms of illness and uses appropriate nursing judgement to provide adequate care for the individual, including initiating acute care plans.
Assists in evaluating nursing intervention outcomes and provides input to other members of the healthcare team including recommendations for improvements. Instructs direct support professionals on how to monitor individuals after a treatment or procedure using instruction sheets such as that used for post-sedation monitoring of signs and symptoms.
Timely and accurately documents and reports pertinent treatments and observations in the individual's medical record (electronic health record or paper chart) in accordance with SSLC procedures and protocols. Prepares reports as required.
Assists with services provided by LVNs and/or Certified Medication Aides (CMA), as needed, and makes recommendations for improvements, including acclimating them to the individuals, their needs and preferences.
Assists LVNs with implementation of protocols, procedures and programs to improve overall health and safety of staff and individuals. Provides assistance with completing follow up assessments, guidelines, acute care plan (ACP) interventions, injury reports, restraints, and med variances.
Provides support to Nurse Manager's including reviews and follow up on controlled drug counts, medication counts, emergency checklist logs, refrigerator temp logs, and glucose controls.
Assists the Infection Control nurse by implementing measures that prevent the spread of disease, assists the Quality Assurance Nurse in the implementation of therapeutic nursing programs. Participates in facility Quality Assurance/Quality Improvement initiatives.
Responds to emergency situations to ensure safety of individuals. Follows safety and infection control procedures of the SSLC. Provides educational and safety training to SSLC staff upon request. Provides treatment to HHS employees injured in course and scope of employment.
Other duties as assigned include but are not limited to actively participating and/or serving in a supporting role to meet the agency's obligations for disaster response and/or recovery or Continuity of Operations (COOP) activation. Such participation may require an alternate shift pattern assignment and/or location.

Registrations, Licensure Requirements or Certifications:
Licensed to practice as a registerednurse in the State of Texas (or a state that recognizes reciprocity through the Nurse Licensure Compact).

Work experience as a practicing registerednurse is required.
At least six (6) months of experience as a practicing registerednurse in a health agency, hospital, or health-care facility is preferred.
Bachelor's Degree is preferred.

The Nurse Quality Monitor III reports to the Quality Monitoring Nurse Manager. This position makes a significant contribution to MCS's mission by conducting monitoring visits in Texas nursing facilities. The Nurse Quality Monitor III utilizes expertise in the area of nursing to assess conditions that could place facility residents' health, safety, and welfare at risk and assists providers in improving systems. This position must be able to apply the complex knowledge of nursing practice as well as the rules outlined in the Texas Nursing Practice Act. The Nurse III will be familiar with local, state, and federal laws and regulations relevant to long-term care.
The Quality Monitor Nurse (QMN) III works under limited supervision, with considerable latitude for the use of initiative and independent judgment. Performs highly complex (senior-level) nursing work. The Quality Monitor Nurse utilizes expertise in the area of nursing to assess conditions that could place facility residents' health, safety, and welfare at risk and assist providers in improving systems. Work may include serving as a resource on nursing practices and education programs by performing consultative and administrative work. May serve as a mentor providing direction to others.
Essential Job Functions

Monitors and evaluates facility systems through observations, interviews, and record reviews identifying facility strengths and weakness making recommendations on opportunities for improvement. Instructs, counsels, and assists nursing facilities to achieve best practice. Reviews facilities policy/procedures and performance reports to determine if the facility's systems are consistent with evidence-based best practice. (50%)
* Documents accurate and complete reports of monitoring visits. Communicates accurate information, findings, recommendations, and technical assistance to the facility administrator and staff as well as quality monitoring leadership. Disseminates and teaches from appropriate education tools and materials. (20%)
* Communicates with various audiences (internally or externally) to provide clinical information, answer inquiries, address issues, and/or resolve problems. Maintains current knowledge in assigned areas by attending unit meetings, seminars, or other training and by continually reviewing program policies, procedures, and any changes. May conduct literature reviews and analyze complex information to update nursing-related evidence-based best practices technical assistance material. (10%)
* Develops and provides in-service education and other training to facility-licensed and non-licensed staff regarding evidence-based best practices. (10%)
* May train and mentor others in the context of a quality monitoring team visit, and orientation to peers as needed. May assists with the development and updating of program policy/procedures, evidence-based best practice standards, and educational programs. (10%)

As part of a collaborative team of public servants, a nurse III protects residents' health and safety by conducting on-site inspections and investigations in Texas long-term care settings (nursing homes, assisted living facilities, and day activities health services) to assess their compliance with state and federal laws and rules. The Direct Care RN performs routine level nursing work providing for the assessment, care and treatment of individuals who reside in a State Supported Living Center (SSLC) setting. Evaluates, treats and documents healthcare needs and/or issues and reports needs and/or issues to the appropriate person(s). Communicates with others, such as direct care staff, physicians/PCP, administration, and other nursing staff to ensure optimal care as part of the interdisciplinary team. Notifies the supervisor and PCP of trends and concerns to ensure that communication is in place to provide consistent, high quality care for all persons served. Completes data entry, required documentation and reports and maintains records.
Assists with services provided by LVNs and Certified Medication Aides (CMA), as needed, and makes recommendations for improvements. Participates in facility quality assurance/quality improvement (QA/QI) initiatives, as required. Provides treatment to HHS employees injured in course and scope of employment. Works under moderate supervision of the Nurse Manager or Nursing Operations Officer, with limited latitude for the use of initiative and independent judgment.
Essential Job Functions (EJFs):

May serve in an on-call status on a rotating schedule as required to meet operational needs of the facility.
Performs routine nursing tasks and procedures in accordance with HHSC, SSLC and state regulations, such as tracheostomy care, gastrostomy/jejunostomy care, wound care, breathing treatments. Collects specimens for laboratory examination and conducts appropriate screening tests when requested by lab personnel.
Administers medications, immunizations, and treatments and observes patients for unusual symptoms and reactions. Makes rounds and/or home visits as needed and works in other locations at the SSLC as needed. Monitors individuals' weight, diet, feedings, and vital signs. Monitors for signs and symptoms of illness and uses appropriate nursing judgement to provide adequate care for the individual, including initiating acute care plans.
Assists in evaluating nursing intervention outcomes and provides input to other members of the healthcare team including recommendations for improvements. Instructs direct support professionals on how to monitor individuals after a treatment or procedure using instruction sheets such as that used for post-sedation monitoring of signs and symptoms.
Timely and accurately documents and reports pertinent treatments and observations in the individual's medical record (electronic health record or paper chart) in accordance with SSLC procedures and protocols. Prepares reports as required.
Assists with services provided by LVNs and/or Certified Medication Aides (CMA), as needed, and makes recommendations for improvements, including acclimating them to the individuals, their needs and preferences.
Assists LVNs with implementation of protocols, procedures and programs to improve overall health and safety of staff and individuals. Provides assistance with completing follow up assessments, guidelines, acute care plan (ACP) interventions, injury reports, restraints, and med variances.
Provides support to Nurse Manager's including reviews and follow up on controlled drug counts, medication counts, emergency checklist logs, refrigerator temp logs, and glucose controls.
Assists the Infection Control nurse by implementing measures that prevent the spread of disease, assists the Quality Assurance Nurse in the implementation of therapeutic nursing programs. Participates in facility Quality Assurance/Quality Improvement initiatives.
Responds to emergency situations to ensure safety of individuals. Follows safety and infection control procedures of the SSLC. Provides educational and safety training to SSLC staff upon request. Provides treatment to HHS employees injured in course and scope of employment.
Other duties as assigned include but are not limited to actively participating and/or serving in a supporting role to meet the agency's obligations for disaster response and/or recovery or Continuity of Operations (COOP) activation. Registrations, Licensure Requirements or Certifications:
Licensed to practice as a registerednurse in the State of Texas (or a state that recognizes reciprocity through the Nurse Licensure Compact).

Work experience as a practicing registerednurse is required.
At least six (6) months of experience as a practicing registerednurse in a health agency, hospital, or health-care facility is preferred.
Bachelor's Degree is preferred.

The Direct Care RN performs routine level nursing work providing for the assessment, care and treatment of individuals who reside in a State Supported Living Center (SSLC) setting. Evaluates, treats and documents healthcare needs and/or issues and reports needs and/or issues to the appropriate person(s). Communicates with others, such as direct care staff, physicians/PCP, administration, and other nursing staff to ensure optimal care as part of the interdisciplinary team. Notifies the supervisor and PCP of trends and concerns to ensure that communication is in place to provide consistent, high quality care for all persons served. Completes data entry, required documentation and reports and maintains records.
Assists with services provided by LVNs and Certified Medication Aides (CMA), as needed, and makes recommendations for improvements. Participates in facility quality assurance/quality improvement (QA/QI) initiatives, as required. Provides treatment to HHS employees injured in course and scope of employment. Responds to emergency situations to ensure safety and care of individuals and staff. Works under moderate supervision of the Nurse Manager or Nursing Operations Officer, with limited latitude for the use of initiative and independent judgment.
Essential Job Functions (EJFs):
Attends work on a regular basis and may be required to work a specific shift schedule or, at times, even a rotating schedule, extended shift and/or overtime in accordance with agency leave policy and performs other duties as assigned.
May serve in an on-call status on a rotating schedule as required to meet operational needs of the facility.
Performs routine nursing tasks and procedures in accordance with HHSC, SSLC and state regulations, such as tracheostomy care, gastrostomy/jejunostomy care, wound care, breathing treatments. Collects specimens for laboratory examination and conducts appropriate screening tests when requested by lab personnel.
Administers medications, immunizations, and treatments and observes patients for unusual symptoms and reactions. Makes rounds and/or home visits as needed and works in other locations at the SSLC as needed. Monitors individuals' weight, diet, feedings, and vital signs. Monitors for signs and symptoms of illness and uses appropriate nursing judgement to provide adequate care for the individual, including initiating acute care plans.
Assists in evaluating nursing intervention outcomes and provides input to other members of the healthcare team including recommendations for improvements. Instructs direct support professionals on how to monitor individuals after a treatment or procedure using instruction sheets such as that used for post-sedation monitoring of signs and symptoms.
Timely and accurately documents and reports pertinent treatments and observations in the individual's medical record (electronic health record or paper chart) in accordance with SSLC procedures and protocols. Prepares reports as required.
Assists with services provided by LVNs and/or Certified Medication Aides (CMA), as needed, and makes recommendations for improvements, including acclimating them to the individuals, their needs and preferences.
Assists LVNs with implementation of protocols, procedures and programs to improve overall health and safety of staff and individuals. Provides assistance with completing follow up assessments, guidelines, acute care plan (ACP) interventions, injury reports, restraints, and med variances.
Provides support to Nurse Manager's including reviews and follow up on controlled drug counts, medication counts, emergency checklist logs, refrigerator temp logs, and glucose controls.
Assists the Infection Control nurse by implementing measures that prevent the spread of disease, assists the Quality Assurance Nurse in the implementation of therapeutic nursing programs. Participates in facility Quality Assurance/Quality Improvement initiatives.
Responds to emergency situations to ensure safety of individuals. Follows safety and infection control procedures of the SSLC. Provides educational and safety training to SSLC staff upon request. Provides treatment to HHS employees injured in course and scope of employment.
Other duties as assigned include but are not limited to actively participating and/or serving in a supporting role to meet the agency's obligations for disaster response and/or recovery or Continuity of Operations (COOP) activation. Such participation may require an alternate shift pattern assignment and/or location.

Registrations, Licensure Requirements or Certifications:

At least six (6) months of experience as a practicing registerednurse in a health agency, hospital, or health-care facility is preferred.
Bachelor's Degree is preferred.

ICF/IID Nurse Surveyor - Starting Market Rate Salary $89,690.71
As part of a collaborative team of public servants, a Nurse surveyor protects residents' health and
safety by conducting on-site inspections and investigations, including investigations of abuse,
neglect, and exploitation, in Texas long-term care settings (Intermediate Care Facilities for
Individuals with an Intellectual Disability or Related Condition (ICF/IID)) to assess their
compliance with state and federal laws and rules. This position offers a competitive starting

Implements special precautions as ordered, explain and document rights restrictions and implements restraint/seclusion procedures according to departmental policy and established nursing standards to maintain patient safety. Protect the patient from causing harm to self and/or others and uphold patient rights.
Participates in the client abuse and neglect process, assures patient safety, and meets patient needs, monitors, and implements Treatment Intervention Protection Strategies (TIPS). Follows all established protocols, policies, and procedures. Participates as the nursing representative in Performance improvement teams. Brief Job Description:
The Nurse Consultant performs highly advanced professional and administrative functions. The work involves: 1) determining health care service requirements for persons infected with (or at risk for) sexually transmitted diseases, including HIV, statewide; 2) planning, developing, coordinating and assisting in the implementation of STD service delivery systems statewide; 3) monitoring the performance of local agencies that deliver these services; and 4-assisting in the evaluation of these services. Under the general direction of the Public Health Follow-Up Team Manager, the Nurse Consultant works with extensive latitude and is expected to use initiative and independent judgment.
Essential Job Functions (EJFs):
Plans, develops, implements, coordinates, and evaluates the service delivery programs of state funded grantees and their major subcontractors statewide that provide clinical services to persons infected with (or at risk for) sexually transmitted diseases, including HIV, and their families/significant others.
Activities include:
* Conduct annual and follow-up quality assurance reviews of funded Infertility Prevention Projects (IPP) and Syphilis Elimination Projects (SEP) as directed by supervisor.
* Analyze statistical studies and reports and reports on STD infection in order to evaluate the STD burden of disease and effectiveness of preventive efforts within local communities.
* Locate and evaluate research finding on STD health service delivery models and applies these to initiate new and/or improve existing STD services statewide.
* Conduct extensive medical record reviews of clients served by grantees and subcontractors to evaluate compliance with professional standards of care, HIV/STD policies and procedures, and to evaluate the quality of contracted clinical STD services. Use the computer to prepare correspondence and written reports. (40%)
Provides extensive consultation, training and technical assistance on issues related to STD/IPP/SEP to grantees, medical and nursing providers, the general medical community, and other health care organizations/individuals that deliver services to persons infected with (or at risk for) sexually transmitted diseases, including HIV, and their families/significant others. Advises public health regions, local health departments, and other STD service providers on clinical problems related to nursing for client populations at risk for, infected with, or affected by STDs. Serves as a resource for clinical information related to STDs, co-morbidity of HIV/AIDS and STDs, and provides consultation on STD clinical services to the general public. Consults and provides technical assistance to the staff from the HIV/STD Section and others within DSHS.
Serves as the Laboratory Liaison to ensure concerns and issues are addressed appropriately. Works with the Laboratory staff to provide training and technical assistance to contractors including, but not limited to, submission of samples and accessing results. (35%)
Coordinates the development, dissemination, and implementation of STD clinical care standards for use in evaluating the quality of care delivered by contract providers to persons infected with (or at risk for) sexually transmitted diseases, including HIV.
Development of standards may include facilitating clinic expert panels, locating and evaluating applicable research findings, and publishing documents for stakeholders. Maintains electronic files related to nationally recognized clinical practice guidelines, protocols for STD services, and current research on STD disease management and elimination and distributes this information to clinical providers and others, as appropriate. Coordinates the development of model physician standing delegation orders (SDO) for STD services. Assists in the development of policies and procedures related to the SDO and disseminates the model SDO to clinical providers, as appropriate. Directs and oversees clinical field investigations conducted by the HIV/STD Outbreak Response Team, as directed by supervisor. (10%)
Develops and delivers in-services and educational programs related to STDs. Presentations are given at the annual IPP workshop, the STD/HIV Integrated Programs meeting, and the Texas HIV/STD biennial conference. Serves as a member of the Conference Planning Committee to ensure STD clinical presentations are appropriate for the Texas HIV/STD biennial conference. (5%)
Coordinates the development, dissemination, and implementation of quality assurance records and tools used to evaluate STD clinical services delivered by grantees. Research existing materials, pilots new products, procures management approval to implement changes, and orients staff and external customers to use of the products. (5%)
As requested, provides input into the policy development of the HIV Program Operating Procedures and Standards; represents the Public Health Follow-Up Team on workgroups and advisory committees related to HIV/STD. Performs routine nursing duties. Work involves providing for the assessment, care and treatment of individuals. May train others. The Nurse I (Staff Nurse) performs entry-level to routine (journey-level) nursing work at a competent level in the state hospital under the supervision of the Charge Nurse or Nurse Manager (Nurse II/III). Conducts comprehensive assessments of patients nursing care needs using specialized skills in an inpatient psychiatric setting. Performs complex (journey-level) nursing work, under the supervision of the Congenital Syphilis Operations Director I. This position will support the Congenital Syphilis Prevention Hotline, providing consultation and guidance to healthcare providers regarding congenital syphilis questions, diagnosis, and care.
Responsibilities include:
* Responding to inquiries from physicians, nurses, hospital staff, and other healthcare professionals through the hotline.
* Offering evidence-based recommendations and clarifications on syphilis and congenital syphilis management, treatment protocols, and reporting requirements.
* Coordinating with regional congenital syphilis nurses to ensure appropriate follow-up on specific syphilis and congenital syphilis cases.
* Assisting with the development and modification of policies, procedures, and protocols related to congenital syphilis prevention and care.
* Participating in the creation and delivery of written and verbal program communications and educational materials for healthcare providers and stakeholders.
* Supporting program enhancements aimed at improving efficiency and service evaluation.
* Attending consultant meetings, educational workshops, and symposiums relevant to congenital syphilis prevention and treatment.

Essential Job Functions:

(40%) Provides professional consultation to healthcare providers regarding congenital syphilis diagnosis, treatment, and management. Responds to inquiries via the congenital syphilis hotline and offers evidence-based guidance. Coordinates with regional nurses to ensure appropriate follow-up on specific cases, including re-testing and evaluation as needed. Maintains close liaison with other state and agency programs to ensure coordination.
(25%) Provides professional follow-up of women of childbearing age and infants with syphilis diagnoses, as required by the Congenital Syphilis Provider Consultation Hotline Director and the Community Health Improvement Division. Enters clinical care coordination notes into the central case management system following policies and procedures. Maintains electronic reports and documentation of the consultation as required by the Congenital Syphilis Provider Consultation program.
(15%) Answers telephone inquiries testing and treatment history for medically complex syphilis cases from emergency departments, OB/Gyns, delivery hospitals, primary care providers, and pediatric specialists. Provide up to date guidance and support for disease management, treatment protocols, and access to additional resources. Assigns tasks to Regional Congenital Syphilis Response Nurses for field follow-up, care coordination, and provider support.
(15%) Develops and provides resources, educational materials, and literature to providers about congenital syphilis prevention. Participates in planning, developing, and executing meetings for consultant workgroups. Performs other congenital syphilis program support activities as assigned by the Director of Operations.
