**Job Summary**
Understands the necessity for timely completion of medical record documentation by analyzing medical records for missing documentation and signatures. Compares the documentation in the medical record against required standards and enters deficiencies for providers in the electronic medical record system.
May be assigned other HIM functions to support departmental workflows. Understands regulatory standards for accurate medical records. Performs record analysis by abstracting and recognizes the relation of a complete medical record. Applies knowledge of disease processes, anatomy, physiology, medical terminology, state laws and other regulatory standards in the analysis of the medical record.
Utilizes job specific software in analysis and monitoring functions. Maintains software system competence including the electronic medical record (EMR) and document imaging at a high level. Completes admission and discharge related functions including retrieval of previous medical records. Scans and captures documents in an extremely timely manner, and confirms capture clarity and quality. Indexes medical record documents using document imaging software. Completes document imaging accuracy checks and audits. Monitors timeliness and completion of various medical record components to assure Federal/State regulatory compliance. Enters and maintains medical professional information in the EMR.
Requires knowledge and application of coding guidelines and regulations in the assignment of diagnosis codes and sequencing specific to location and department guidelines. Assigns codes to appropriate medical diagnoses based on review of supporting documentation, and ensures these are captured for timely and accurate billing for all payers. Maintain application of facility policies and standards of practice to assure release of information requests (ROI) are appropriate and meet legal regulations. Processes ROI requests within requested timeframes. Prepares and generates reports as requested.
Will work extensively with protected healthinformation and is required to adhere to health insurance portability and accountability act (HIPAA) privacy and security regulations and policies related to the same. Handles requests for release of information according to policies, and maintain security of healthinformation and medical records. Supports and educates others on managing private information.
Prepares and participates in Health Department survey activity. Supports providers and other clinicians on EMR usage and workflows.
**Qualifications**
Associate's degree in HealthInformation or Medical Record Technology preferred.
Experience in health care or long-term care preferred.

All Paper documents/patient care records are scanned into the EMR within 24 hours of discharge, with the exception of Rehab records, while maintaining 98% accuracy.
Serves as resource person to assist with physician, patients, outside agencies, and Administrative Coordinators on record retrieval and appropriate assignment of document types in the EMR.
Audits the scanned documents in the Electronic Medical Records (EMR) to ensure integrity of the EMR is maintained.
Retrieves and stores paper records from and to Shared Services.
Supports scanning of the Pre Surgery, Diagnostic Scheduling, and Financial Clearance via uploading electronic documents into the EMR with productivity at least 8 charts an hour.
Consistently scans at least 500 pages per 8 hour shift.

Actively assists with the implementation of new procedures, schedules and training/cross training of coworkers by sharing critical information pertaining to newly discovered information or system updates

POSITION SUMMARY

Work Schedule: This is a casual position. Casual employees work on an ‘as needed' basis to meet the needs of the business. Work days/hours will often vary from week to week.
Responsible for greeting patients, employees, visitors.
Responsibilities:
Essential Functions
1. Greets patients, employees, and others in a professional manner to assure excellent response to request for information and service.
2., Provides direction and information assistance regarding the location of clinical services, administrative meetings and other general needs by communicating clearly to assure customers reach their destination.
3. Coordinates with Emergency Department staff for in-coming 3. surgical patients.
Directs and assists patients with appropriate forms.
Communicates with surgical staff to alert them of waiting patients.
4. Provides transportation assistance for patients and family members at the front entrance by taking them to the Admitting Department for registration.
5. Provides general communication duties. These duties include initiating, receiving, and directing telephone messages and maintaining written communication pieces like maps, patient handbooks etc. by clarifying the customer needs to
respond appropriately to requests for information.
6. Performs other duties as assigned, maintains teamwork, positive mentoring and willingly assists with training of employees by positively assisting with the implementation of new procedures and job functions to enhance admitting services.

Job Requirements
Education
High school diploma or General Educational Development (G.E.D) equivalent required. Individuals with significant progress toward high school diploma or significant experience in providing customer/patient service may be considered in lieu of high school diploma.
Must be 16 years of age.
Participates in mandatory in-services and continuing education programs as mandated by policies and procedures/external agencies and per administrative direction.
Experience
Previous customer service and medical terminology preferred.
License/Certifications
Requires the ability to provide transportation to other campus locations within a reasonable timeframe.
Skills/Knowledge/Abilities
Must have excellent written and verbal communication skills to effectively meet customer needs and appropriately read computer generated reports.
Ability to handle multiple priorities successfully required.
Ability to use a computer required.
Ability to transport/escort patients and guests/family members to appropriate locations via stairs, elevators or ramps.
Ability to communicate with others through a glass barrier.
