About the Program
The Home's Child and Family Counseling Center (CFCC) is a licensed outpatient mental health clinic, providing treatment, therapy and prevention services to adults, children and their families who are facing emotional, behavioral, developmental or academic challenges.
Many of those who receive services have experienced toxic stresses. In addition to meeting with families at our clinic, The Home's CFCC clinicians are also embedded in over 50 schools.
How You Will Be Making a Difference
* Provide case management services; act as a liaison with collateral agencies and programs; establish and maintain relationship services that support client and family growth. Act as an advocate for families, support parents where appropriate to advocate for themselves and their children. Help families develop the skills needed to navigate the social service system.
* Utilize treatment modalities that are consistent with program goals and that best fit with clients' age and treatment needs, including play/expressive therapy, group therapy and family therapy.
* Perform initial assessment and evaluation of new clients and families. Identify treatment needs and barriers to growth and development. Design treatment plans that address and specify outcomes. Collaborate with clients and family members to develop treatment objectives. Demonstrate cultural awareness, humility, curiosity and sensitivity.
* Work as part of an interdisciplinary team to provide quality care. Attend staff meetings and case conferences, present client material. Consult with supervisor and team members on challenging or difficult client issues. Facilitate communication between various providers.
* Submit reports, documentation and billing in a timely manner as determined by agency expectations. Ensure that documentation meets utilization review and quality assurance standards.
* Attend in-service training and outside seminars to enhance and develop clinical skills
* May provide training, workshops and consultations to teachers, parents and collateral staff

We know that the best place for children to thrive and grow is in a stable home with their families. Often, youth and their families need additional supports to make home a safe place for a child that is struggling with their mental and behavioral health. Safe at Home is a team of therapists who work intensively with a child and his or her family in their home for a period of 3 weeks or 3 months.
The Home for Little Wanderers' Safe at Home program in Somerville/Cambridge provides In-Home Therapy and Therapeutic Mentoring services to the Greater Boston area. Comprised of master's level clinicians and bachelor's level therapeutic training and support staff, In-Home Therapy teams provide intensive family therapy services, working to keep children and their families together in the community. Working from a variety of therapeutic modalities, Safe at Home emphasizes a family systems approach that works with every member of the family to help meet therapeutic goals.

POSITION OVERVIEW:
CFTSS Clinical services are provided to children/youth and are intended to help identify and prevent the progression of behavioral health needs through early identification and intervention. Services may be provided to children/youth in need of assessment for whom behavioral conditions have not yet been diagnosed or to youth with a diagnosis for whom flexible community-based treatment is needed to correct or ameliorate a developmental delay or problem in functioning or capacity for healthy relationships. The clinician conducts a comprehensive assessment to determine if children/youth are eligible for CFTSS, makes recommendations regarding which CFTSS services are to be provided, and completes a treatment plan, which serves as a rehabilitative service package to support a child and family whose needs may be complex and/or require flexible nontraditional approaches. We are seeking a compassionate, dedicated, and proactive therapist to join our team, providing high-quality behavioral health services to individuals in the Bronx. The ideal candidate will have a strong clinical background and a passion for community-based work. This is a hybrid role and involves both direct clinical care and community engagement.
KEY ESSENTIAL FUNCTIONS:
Provide individual and/or group therapy to clients in community-based settings and virtually as needed.
Facilitate psychoeducational and skills-building groups within Jewish Board community residences and for external clients (in-person or via telehealth).
Conduct outreach to potential partner agencies for collaborative opportunities and community engagement.
Travel throughout the Bronx as required to meet with clients at residences, community centers, or other appropriate locations.

The purpose of a Trauma Systems Therapy (TST) Therapist in Preventive Service Department is to provide the TST model for families with child(ren) (ages 5 and over) who are impacted by trauma and there may be risk for foster care placement. POSITION OVERVIEW:
The Therapist works with families to assess their needs, develop intervention plans, and deliver therapy sessions that are tailored to the family's unique circumstances. The therapist also plays a crucial role in monitoring the family's progress, adjusting treatment plans as necessary, and working towards successful case closure.

Participate in hospital discharge planning meetings to identify the best community resources for returning participants.
Provide short-term therapeutic counseling and support to participants both pre- and post-housing.
Collect and report data as required, and collaborate with the team leader, data analyst, and other SOS teams to use data to inform future care delivery.
Once participants are housed, work with them and their housing providers to resolve clinical issues impacting the participant's ability to manage and retain supportive housing.
Foster relationships with community providers to ensure that participants are connected to appropriate services as they transition back into the community.
Assist with appointment navigation, including accompanying participants to appointments, providing travel training, reengaging in community care, and addressing barriers to care.
Facilitate crisis interventions, referrals, and hospitalizations as appropriate.
Review documentation and conduct comprehensive psychosocial assessments to determine the medical, psychiatric, housing, and other social needs in the community.
Obtain historical and collateral information from multiple sources to support participants' behavioral and physical health needs.
Monitor, evaluate, and record participant progress with respect to care plan goals.
Perform other related duties as assigned.
Requirements
Licensed master's degree or higher in social work or mental health counseling.
Experience working with homeless and/or precariously housed populations is preferred, but not required.
Knowledge of homeless resources, NYC shelter systems, and MTA transit systems is a plus.
Familiarity with counseling principles and methods for mental illness and substance use disorders.

Community Services and Resettlement Internship Overview
The community services team provides case management and other services to newly arrived refugees, asylees and other clients for up to 90 days after arrival, with some case management services provided after the first 90 days. Job Description
Community of Veterans (CoV) is the first and largest online social network exclusively for paperwork-confirmed Iraq and Afghanistan veterans. CoV provides veterans with a safe space to share what they're going through with others who “get it” because they've been there too. The Licensed In-Home Mental Health Counselor will provide counseling and mental health services to youth and families. Mental health assessment and counseling may be provided in the home or in the community for a youth and their family.
Major Responsibilities/Activities:
Provide mental health assessment, diagnosis, treatment planning and psychotherapy services
Provide individual, family, and group counseling to a diverse population with many challenges
Provide individual and family interventions with the goal of developing and implementing social, interpersonal, self-care and independent living skills to restore and maintain stability, to support functional gains and to adapt to, and or maintain community living.
Guide families during and following a crisis episode experienced by a child/family with the goal of stabilizing the child/youth in the home and natural environment.
Maintain the crisis phone number, on call, for a week at a time (rotated amongst staff)
Collaborate with other treatment providers and collaterals, as appropriate
Complete and submit required documentation in a timely manner (e.g. progress notes, treatment plans, et cetera). Maintain positive working relationships with other professionals, community agencies and team members
Demonstrates sensitivity to cultural and ethnic norms
Follows agency's Worker Safety protocols
Participates in team meetings
Engage and guide the youth and family through active listening skills and therapeutic engagement methods
Maintain positive working relationships with program/agency staff, collaborating agencies, and stakeholders
Continue professional growth and training in best practices, as necessary/appropriate (e.g. cultural competencies)
Attend coaching/supervision meetings as required

Minimum Requirements:
NYS Education Department license in Social Work, Mental Health Counseling, Psychology or Marriage and Family Therapy
At least one (1) year of experience providing direct services for children with one or more of the following primary diagnoses: mental illness, alcoholism, chemical dependency and substance abuse
Demonstrated cultural competence in understanding and working with disadvantaged/minority populations in community-based programs in an urban and suburban setting

The Behavioral Health Counselor is working with a PROS program to conduct individual and group services.
Personalized Recovery Oriented Services (PROS) is a comprehensive model that integrates rehabilitation, treatment, and support services for people with serious mental illness and possible other co-occurring disorders.
The Behavioral Health counselor works with PROS participants towards goals in different areas:
Living independently
Building natural supports
Finding and keeping a job
Reaching higher levels of education
Securing preferred housing
Improving medication management
This will be carried out as indicated by NYS OMH Part 512 Regulations and Medicaid requirements.
Essential Functions:
Manage a caseload of up to 25 consumers, including case management, collateral contacts, referrals, vocational assistance, and other needs, to coordinate services.
Conduct required assessments including Wellness Recovery Action Plans, Psycho-socials, Psychiatric Rehabilitation, and Substance Use screenings and assessment.
Develop Individualized Recovery Plans (IRP) in collaboration with participants, on a bi-annual basis, or more often as necessary.
Maintain and update demographic, treatment and employment data in electronic health records (Foothold AWARDS, OMH CAIRS database, and NYESS).
Facilitate member access to community support services including medical, mental health, substance abuse, housing and entitlements.
Develop and facilitate approximately 6 -8 groups, according to protocol and curriculum, which will cover CRS, ORS, IR and Clinical Services as dictated.
Conduct individual counseling sessions to assist program participants with attainment of the life role goals specified in their IRPs.
Document consumers' progress towards goals in monthly progress notes and determine the need for addition or subtraction of services offered to each participant on an ongoing basis.
Coordinate treatment including the sharing of Individual Recovery Plans and Progress Notes for participants who are co-enrolled with other agencies as necessary.
Provide services according to Evidence Based Practices including Wellness Self Management, Family Psychoeducation (CCFC), Medication Management, Co-occurring Disorders (IDDT), and Supported Employment (IPS).
Coordinate follow-up services once a participant is ready for discharge from the program.
Act as a member of an interdisciplinary treatment team, including participation in weekly administrative, clinical and utilization review meetings.
Assume overall responsibility for maintenance of client records, including timely documentation of services in Electronic Health Record (AWARDS), as required by Medicaid and the Office of Mental Health.
Develop and maintain collateral relationships with consumers' other providers and family as clinically indicated
Carry a 24-hour support cell phone in rotation with the clinical team.

Qualifications/Basic Job Requirements:

A minimum of a Bachelor's Degree in Psychology, Social Work, Human Development and related field, 3+ years of experience working with adults with mental illness and/or families dealing with mental illness.
Masters Degree preferred.
Team-based skills are essential.
