Physical Therapist Job Type: Per Diem (PRN) Setting: Home Health Arbor Home Health Therapy provides home health agencies with staffing for Physical Therapy, Occupational Therapy and Speech Therapy. Our partnerships with each home health agency will determine the documentation system in which you will document your visits (Therapy Boss, Kinnser or Home Care Home Base). We are currently seeking a Physical Therapist to provide physical therapy services in the community and AL settings. The Home Health Therapist must be: Self-motivated, organized, and responsible for patient care including scheduling of appointments. Timely with documentation of visits and communication with the Clinical Program Manager and therapy team. Comfortable entering patient homes in the community, as well as Independent & Assisted Living facilities. Willing to commit to following a patient throughout the POC for initial, interim and discharge visits. Routine follow up visits will typically be assigned to therapist assistants (PT/OT), and the evaluating therapist is responsible for cosigning treatment visit notes on a weekly basis. Rate of pay is based on the type of visit assigned and paid at a flat, per visit rate (Range from $55-$90/visit). We are currently seeking a Physical Therapist to provide physical therapy services in the community and AL settings. The Home Health Therapist must be: Self-motivated, organized, and responsible for patient care including scheduling of appointments.
