We're seeking compassionate, dedicated individuals to join our team and support the most vulnerable in our community. Position Title: OEC Case Manager Program(s) and Location: Outreach and Engagement Center (Sacramento, CA) Schedule: Full-time, Non-Exempt, 40 hours per week Monday through Friday 10:00 AM to 6:30 PM Pay rate: $23.99 hourly About the Position: The Outreach and Engagement Center Case Manager will assist individuals experiencing homelessness by offering brief respite and information, advocacy, and referrals for continued assistance and support after they leave the Outreach and Engagement Center. The Outreach and Engagement Center is a program designed to provide respite for adults experiencing homelessness who are referred to us by the City of Sacramento Department of Community Response, Sacramento County Park Rangers, or the City of Sacramento Impact Team who are camping in the immediate vicinity of the Center. The Center is a place to receive support, services, and linkages and to gain hope for a resolution. This program is designed to provide a safe environment for individuals who need relief from the stresses of homelessness, and who need assistance in accessing services and resources. The Outreach and Engagement Center is open and welcomes individuals 24 hours a day / 7 days a week by referral invitation only. Services will be offered from a Trauma-informed perspective and will utilize Motivational Interviewing and other Evidence-Based Practices as appropriate. Essential Duties & Responsibilities: Support individuals with active listening, demonstration of empathy, and carry a message of hope and recovery. Observe, assess, intervene, and refer (within one's scope of practice) in situations with clients regarding mental health symptoms, physical health symptoms and substance use issues, and homelessness. Develop service/goal plans with clients and meet with the clients as needed to encourage, support and advocate for progress and assist with skill development. Provide positive public relations, community referrals, information, and support by linking clients to needed resources; advocate with and on behalf of clients and significant support persons the clients have identified. Accurately complete all necessary forms, reports, and paperwork required by the program on a timely basis (e.g., progress notes, special incident reports, discharge summaries, HMIS, ViSpdat, and other reports as required). Ensure that the Center and surrounding environment are clean and fit for occupancy, where applicable. Provide and assist with cleaning as needed. May be asked to attend to pets and pet kennels as needed. Act as a designated Mandated Reporter for the observed or suspected abuse and/or neglect of children, disabled individuals, dependent adults, and elder/senior citizens. Mandated Reporter also includes “Duty to Warn” which includes the responsibility to notify authorities and the intended victim(s) of physical harm. Ensure the protection and confidentiality of all Protected Health Information (PHI) by following all policies and procedures in the Agency HIPAA Manual. Regular and predictable attendance is required. Qualifications: Ability to work with individuals who have are experiencing homelessness and may have co-occurring disorders. Experience is highly desirable. Lived experience with homelessness, mental health challenges or substance use disorder is highly desirable. Knowledge of Sacramento community organizations and community resources is highly desirable. Mental health crisis intervention experience is highly desirable. Experience with persons who have substance abuse problems, preferably individuals who are dually diagnosed (mental health & substance abuse) is highly desirable. Knowledge of 12-Step Programs and/or personal experience with substance abuse recovery is highly desirable. Serve as back-up during absences for other Mental Health Nurse Case Managers. Support Joint Commission Readiness to support quality and safety for patients. Under direction and with standing orders, use independent judgment within the licensed capacity to meet as needed with individual clients, on site, in other locations, or by telephone. Assist mental health staff in coordinating mental health referrals to outside community resources for patients in the mental health department. Monitor and track referrals to inside programs, including other mental health services. Educate patients about community based mental health recovery programs. Perform telephone callbacks, prescription refills, ordering labs, etc. as assigned by SRCH mental health providers. Follow all established clinic protocols for patient care and clinical duties and assist with quality assurance functions. Participate in triage of patient needs. KEY RESPONSIBILITIES Case Management: Prepares extended treatment episode authorization requests and routes to medical leadership for review. Forwards finalized document to Department of Public Health for authorization and, if received, ensures the authorization information is relayed to internal stakeholders. Facilitates the confidential exchange of client's protected health information (PHI) between Admissions and WM to adequately evaluate unit appropriateness for client's needs. Monitors, coordinates, and resolves obstacles between prescribing providers, payors, and retail pharmacies to make certain that clients receive critical Medication Assisted Therapies (MAT) with few to no lapses in treatment days. Outreach and Relationship Management: Collaboratively work and communicate with other agencies and local pharmacies to provide information regarding resources and service opportunities. Leadership Responsibilities: Works collaboratively with all invested staff. Provides administrative leadership for client medication support, and within the quality improvement infrastructure of the healthcare and residential program. Participates in matters related to performance and quality improvement, planning, protocols, and goal setting. Comfortable in changing systems, and champions change. Organizational Responsibilities: Accurately and consistently documents required information on records and reports. Keeps up to date with operational and procedural requirements. In conjunction with WM Nurse Manager, Behavioral Health Nurse Director, and/or Director of Addiction Medicine, assures organizational readiness for accreditation surveys and ongoing monitoring and reporting of conformance to quality within the program. Provides direct care coordination support for clients to access all aspects of HR360 healthcare services. And perform other duties as assigned. QUALIFICATIONS Education, Certification, and Experience Required: Possess a minimum of an AA degree in a related field; or 3-5 years related experience. BLS and First Aid Certification must be obtained within 30 days of hire. Experience and interest in working with safety-net populations and in treating substance use disorders. Experience working successfully with issues of mental health, criminal background, and other potential barriers to economic self-sufficiency.

Position Title: Peer Family Advocate Program(s) and Location: CORE Howe (Sacramento, CA) Schedule: Full-time, Non-Exempt, 40 hours per week, Monday - Friday 8:00am-4:30pm (may require working some evenings and weekend schedules) Pay rate: $21.87 hourly About the Position: The Peer/Family Advocate will serve as a member of a multi-disciplinary team to provide peer and/or family support in collaboration with the Service Team. Their focus will be to assist with accessing program services and other community resources to support wellbeing based off their prior personal or familiar experiences. The Peer/Family Advocate will function as a role model exhibiting competency in personal recovery and use of coping skills as well as assisting in helping clients learn coping skills and develop wellness plans congruent with cultural norms and expectations. The Peer/Family Advocate will serve as an advocate for client choice and voice. Essential Duties & Responsibilities: Support clients and/or family members with active listening, demonstration of empathy and carry a message of hope and recovery and empower clients in meeting their identified goals. Collaborate with Service Team members in coordinating and providing services for clients. Observe, assess, intervene and refer (within one's scope of practice) in situations with clients regarding mental health symptoms, physical health symptoms and substance use issues. Assist clients in developing and following a wellness plan unique to their needs and desires. Assist member with various independent living skills: house cleaning, laundry, food preparation, hygiene, yard work, budgeting money, shopping, medication management and safety. Using personal experiences and interactions with mental health systems and other community partners, to guide and support client in advocacy; as well as provide hope. Provide/Assist the clients with transportation in personal vehicle as necessary. Meet with clients and/or family members in the community and their homes, and attend meetings to ensure continuity of care for clients. Provide positive public relations, community referrals, information and support by linking clients to needed resources; advocate with and on behalf of clients and significant support persons the clients have identified. Facilitate planning of social, cultural, educational, supportive and recreational group activities. Accurately complete all necessary forms, reports and paperwork required by the program on a timely basis (e.g., progress notes, special incident reports, discharge summaries, Medi-Cal billing, EHR forms). Obtain Peer Certification within 6 months of employment Act as a designated Mandated Reporter for the observed or suspected abuse and/or neglect of children, disabled individuals, dependent adults and elder/senior citizens. Mandated Reporter also includes “Duty to Warn” which includes the responsibility to notify authorities and the intended victim(s) of physical harm. Regular and predictable attendance required. Use motivational interviewing, trauma-informed care, and harm-reduction practices. Qualifications: Lived experience as a mental health consumer or a family member of a mental health consumer or as someone who has experienced homelessness. Ability and willingness to work with people experiencing homelessness where they are in encampments, on the street, or in shelters. Work experience with persons who have substance abuse problems, preferably individuals who are dually diagnosed (mental health & substance abuse) is highly desirable. Knowledge of 12-Step Programs and/or personal experience with substance abuse recovery is highly desirable. Demonstrated knowledge of General Assistance, Food Stamps, Temporary Aid to Needy Families, Unemployment Insurance, and SSI/SSDI regulations High School Diploma or GED required Certificates, Licenses, Registrations: Possess a valid California Driver's License and current vehicle insurance. Driving records must meet, and continue to meet, the underwriting standards established by the generalized driving guidelines used by our insurance broker. Must provide proof of insurance. Must have at least 3 or more years of driving experience.
