SocialWorkers serving in a variety of departments and clinics are responsible for responding to referrals from the interdisciplinary team. The socialworker will complete psychosocial assessments, develop and implement a plan of care that addresses patients' needs that promotes continuity of care, and facilitates transitions to the next level of care. The socialworker will provide education, consultation, and serve as a liaison to the healthcare team regarding the patient and families' emotional, psychological, social, financial and developmental aspects of care.
Engagement and Exploration:
Engage patients and/ or their families into a helping process. Assess the psychosocial and mental health needs of patients and families to:
* Identify barriers to treatment
* Enhance patient's health status.

* Develop rapport with patient and family for ongoing support as needed
* Adheres to the tasks and responsibilities specific to the department, service or clinic.
Goal Development and Implementation:
Develop a comprehensive plan of intervention in collaboration with the patient/ family and healthcare team. Implement a plan of care and monitor patient's response:
* Provide individual/family counseling, and social work intervention, as it relates to complex crisis management, mental illness, adjustment to illness, material assistance, healthcare decision-making, sexual assault, domestic violence, partner violence, elder abuse, competency, and any other psychosocial barriers in order to maximize the health status of patients.
* Advocate, mediate and negotiate to formulate a cohesive plan for maintaining or enhancing patient's health status, improving social supports, and moving patients safely across the continuum of care.
* Provide family education.
* Provide education about Advance Care Planning and assist with completing Advance Directives paperwork.
* Initiate referrals to community resources as indicated.
* Facilitate discharge process to home, nursing facility, rehabilitation facility, LTAC, group home, treatment or other facility.
Evaluation and Termination:
* Briefly summarize care plan with patient/family and respond to questions and concerns.
* Provide patient/ family with linkages to community resources and services.
* Participate in process improvements in evaluating patient outcomes.
* Identify barriers in service delivery systems.
* Assess for transportation needs and arrange as appropriate.
* Facilitate referrals to disease specific agencies, especially as it relates to employment, financial resources, support services, chemical dependency services.
Education and Consultation:
* Provide consultation and training to medical staff and other healthcare professionals on psychosocial issues regarding illness and barriers to maximizing improved health status.
* Collaborate with co-workers and management to ensure the on-going development of the Social Work Department.
Competency:
* Participate in activities that support enhanced customer service and results in increased Press Ganey scores.
* Attend staff meetings and mandatory departmental in-service training sessions & continuing education in-services; maintain positive working relationships with and knowledge about community agencies and services.
* Maintain timely, clear and concise documentation; and write reports as needed.
* Respond to E-mail requests and provide information as requested.
* Participate in quality and safety initiatives.
Education: Master of Social Work from an accredited School of Social Work, minimum required.
License and Certification: Licensed by the State of OhioSocial Work Board (LSW, LISW, or LISW-S)
Years of experience: One-two Years equivalent experience, minimum required. Three-five years, preferred.
Required Skills and Knowledge:
Casework principles, including diagnosis, assessment, crisis intervention, and treatment planning; techniques in individual, family and group therapy; awareness of community resources, public assistance systems, Medicare/Medicaid; accepted professional and ethical standards within the existing medical community and professional organizations.

The community-based Behavioral Health Therapist serves as a consultant for a multi-disciplinary team to ensure treatment interventions are implemented and that the patient is in compliance with treatment.
In a residential setting, the Behavioral Health Therapist provides treatment for patients requiring a higher level of care than outpatient therapy; however, do not meet the criteria for psychiatric inpatient treatment. Treatment occurs at the residential treatment facility and includes a broad range of duties: assessment and diagnosis, individual and group therapy, development, implementation, and evaluation of therapy programs, and consultation and coordination with a multi-disciplinary team.
Credentialing through the Allied Health Staff of the organization where you will be working will be required. Must be completed prior to start; typical time from offer acceptance 60-90 days
Qualifications
Master's degree in Counseling, Social Work, Psychology, or related Behavioral Health Sciences required; graduation from an accredited school preferred.

About Inpatient Psychiatry, located at the College Hill Campus
The Division of Child and Adolescent Psychiatry at Cincinnati Children's acute crisis units focus on stabilizing patients, most of whom stay an average of three to five days. In total, our hospital has 92 inpatient beds available to serve children and adolescents who are 17 years old and younger, who are experiencing a mental health crisis. The units are explained below:
* Adolescent Unit 4.200 (ages 12-17): Patients with a mental/behavioral disorder
* Youth Unit 5.400 (ages 8-17) Patients with both a mental/behavioral diagnosis and developmental disability
* Young Child Unit 5.200 (ages 2-8) Patients who have a mental/behavioral health diagnosis
* Youth with chronic medical conditions in addition to mental health diagnosis
Youth admitted for inpatient treatment can participate in various therapeutic and medical treatment programs depending on their personal needs. Their stay can include group therapy, music therapy, occupational therapy, speech pathology, and chemical dependency education.

Employee will work collaboratively with community mental health providers and community support services to develop, provide and support a continuum of care for patients.
Case Management and Treatment Coordination: The employee will work collaboratively with the primary care team to provide referral, case management and follow up services for patients with behavioral health needs. This will include engaging patients, initiating referrals, addressing barriers to care and follow-up. Employee should be familiar with community resources to address patient psychosocial and behavioral health needs.
Documentation: The employee will provide accurate and timely documentation in Epic, Agility or other authorized electronic health records to support the identification and treatment of patient's psychosocial and/or behavioral health needs. Will provide assistance in capturing program evaluation and fidelity measures as directed.
Quality Improvement: The employee will work collaboratively to assist in monitoring the behavioral health program and identifying areas for improvement. Employee will participate in meetings and quality improvement activities, continuing education, community education and serve as a member of committees, or related activites, as directed by supervisor.

Mobile Crisis Team (MCT) Clinician serve the entire Hamilton County residents and receive referrals from interdisciplinary team members, including psychiatrist, case managers, physicians, police, and individuals. The MCT Clinician will complete a psychosocial assessment develop and implement a plan of care that addresses the patient's needs that promotes stabilization in the community and or facilities transitions to the next level of care. The MCT Clinician will provide education, consultation, and serve as a liaison to the health care team and mental health team regarding the patient's familiars, emotional, psychological, social financial and development aspects of care.
Minimum Required: Master's Degree - Social Work, Psychology, Counseling, or related field. | Required: Licensed SocialWorker (LSW) or Licensed Professional Counselor (LPC). Complete required training and health officer certification per state of Ohio Revised Code per Mental Health, Addiction and Recovery Services Board of Hamilton County within one year. Preferred: Licensed Independent Social Work, Licensed Clinical Professional Counselor (LCPC). | Minimum Required: 1 - 2 Years equivalent experience. Preferred: 3 - 5 Years equivalent experience.

Patient Population: Engages in population appropriate communication. Has knowledge of growth and development milestones and tasks. Gives clear instructions to patients/family regarding treatment. Involves family/guardian in the assessment, initial treatment and continuing care of the patient. Identifies any physical limitations of the patient and deploys intervention when necessary. Engagement and Exploration: Engage patients and the families referred to Mobile Crisis in the helping process. Assess the psychosocial and make decisions regarding Mobile Crisis response and priority of the response. Identify barriers to treatment, Enhance patient's health status, Increase patient/family satisfaction and quality of life. Improve utilization of resources to ensure appropriate level of care in either community outpatient care, hospitalization, and criminal justice. Develop report with patient and family for ongoing support and in response to crisis in community. De-escalation of patients in crisis utilizing best practices including trauma informed care, etc.
Goal Development and Implementation: Develop a comprehensive assessment to determine the appropriate level of care in collaboration with care team. Identifying from assessment which referrals are needed for patient with goal of ensuring patient remains in the community without hospitalization or intersect with criminal justice system. Implement a plan of care and monitor patient's responses: Determine if an Application for Emergency Admission is necessary, Collaborated with medical team and psychiatry services when patients are admitted, Educate and provide counseling to patient, next of kin, and community regarding level of care decision using evidence-based practices. Utilize other resources in the community to avoid hospitalization and allow for stabilization in the community. Facilitate follow up in the community upon discharge from PES or inpatient hospital stay. Initiate referrals to community as indicated by plan. Provide education about treatment options and stabilization in community for mental illness. Advocate, mediate and negotiate to formulate a cohesive plan for maintain enhances patients' health status, improving social supports, and move patient safety across the continuum of care. Collaborate with law enforcement and criminal justice system at appropriate times. Provide appropriate intervention to patient and family. Counseling. Crisis management. Education and Consultation: Provide consultation and training to medical staff, health care professionals and law enforcement in response to a mental health crisis and follow up. Collaborate with team including peer specialist, behavioral health specialist and other clinical staff. Building relationships with community partners for ongoing referrals including Law Enforcement.
Evaluation and Termination: Briefly summarize care plan for patients in need of care transitions support post hospitalization/Emergency Psych visit. Provide patient and family with linages to community resources and services. Participate in process improvement to evaluating patient outcomes. Identify barriers in service delivery systems. Assess for transportation needs and arrange as appropriate. Facilitate referrals to disease specific agencies especially as it related to employment financial resources, support services chemical dependency services.
Competency: Participate in activities that support enhanced customer service and results in increased Press Ganey scores. Attend staff meetings and mandatory departmental in-service training sessions & continuing education in-services; maintain positive working relationships with and knowledge about community agencies and services. Maintain timely, clear, and concise documentation; and write reports as needed. Respond to E-mail requests and provide information as requested. Participate in quality and safety initiatives.

UC Health is hiring a part-time Physical Therapist (PT) for the Outpatient Therapy service's department for the first shift at UC Gardner Neuroscience Institute.
Evaluate, plan, and implement Physical Therapy treatment programs for referred patients in order to restore function, prevent disability, reduce pain, and return to his/her maximum function. Follow hospital department approved policies related to documentation, attendance, education, etc. Show respect and courtesy to all staff, patients, and visitors.
Minimum Required:
Degree in Physical Therapy from approved PT School.
License to practice Physical Therapy or licensure eligible in the applicable state.
Recently certified, licensed or graduates within past 12 months.
