Foundations For Living is an 84 bed secure residential treatment facility for youth addressing mental health and substance abuse issues. Located on the edge of Mansfield, Ohio, in a 14 rolling, wooded acres rural setting, Foundations For Living has easy access to I-71.
Qualifications
Education:
High school diploma or equivalent required; associate or bachelor's degree in a related field preferred
Experience:
At least one year of experience working with at-risk youth in a residential setting.
Ability to work collaboratively with a team.
Ability to maintain professionalism and composure in challenging situations.

The employee is occasionally exposed to outside weather conditions and may be subject to occasional local travel as required by facility needs. The noise level in the work environment is usually quiet to moderate.

Key Responsibilities
Ensure the completion of patient databases, treatment plans, progress notes, discharge summaries, and other essential documentation in accordance with applicable federal and state standards and agency policies and procedures.

Qualifications

If license‑eligible, must maintain the Board's requirements to be able to practice including receiving the necessary amount of clinical supervision by a Board approved supervisor.
Proof of licensure or certification by a State approved regulatory agency is required immediately once employed. Serves as liaison with system referral partners and provides supportive and informational communication with persons served. Ensures optimal intake and bed placement of patients. The BH Transfer Center Mental Health Counselor assists with the daily operations of the Bed Placement and the Transfer Center department to provide timely service to clients.Essential Functions/Key Responsibilities:1. Responsible for the procedural functioning of the shift and managing referrals in EMR, tracking and monitoring referral status.2. Answers phone calls, provides initial phone assessment and screening for bedded programs.3. Ensures that all referral information is complete.4. Consistently uses multiple computer systems to appropriately place clients, maintain awareness of, and monitor activity of bed capacity at programs.5. Conducts ASAM screenings for all members seeking service. Provides referral information to individuals who do not meet program admission requirements.6. Serves as primary liaison between the organization and other providers such as hospital emergency rooms and referring partners to ensure quality of care. Maintains relationships and communication with referral sources.7. Acts as a conduit for the flow of information regarding programmatic updates and clinical procedural alerts.8. Maintains professionalism through clear and appropriate communication within BILH Behavioral Services and other referral partners.9. Accesses administrator on call when needed and escalates referral information as indicated to treatment providers.10. Provides feedback to Transfer Center Manager regarding issues affecting shift performance.11. Ensures the transfer of necessary information to/from sending/receiving site for methadone guest dosing and any other medications that need to be filled prior to and to ensure successful admission.12. Maintains denial and acceptance in accordance with regulatory guidelines.13. Other related duties as assigned.QUALIFICATIONS & EDUCATION:Bachelor degree preferred. Associate's degree (A. A.) or equivalent from two-year college or technical school; or one or more years related experience and/or training; or equivalent combination of education and experience. 1. Full-Time, Non-Exempt, Helena - Crisis Facility Crisis Home Worker Position Overview: A paraprofessional person working on a therapeutic team involved in the operation of a transitional 24-hour crisis facility. Provides direct support in the crisis facility, assisting with monitoring, safety, de-escalation, basic care needs, and client engagement. Works as part of a multi-discipline team to ensure a therapeutic and trauma-responsive environment.
Job Duties:
Assists in matters relating to admission at the crisis facility including, but not limited to procurement of food, meal preparation, facility care and maintenance of the facility and grounds and supervision of the clients.
Assists clients in problems relating to daily living activities as they are relevant to their circumstances (food, showering, laundry).
Performs crisis intervention in connection with treatment team members.
Assists in behavior and medication monitoring for clients.
Must complete and pass medication training test within initial probation period.
Assists with distributing prescribed medications.
Maintains required paperwork for assigned clients in accordance with Many Rivers policy.
Performs operation or facility tasks as needed (i.e. - laundry, dishes, vacuuming) for upkeep of facility.
Coordinates on-site activities for clients as appropriate.
Must have a valid MT Driver's License, personal vehicle, and current insurance. Travel is required and may transport clients.
May work varied shifts including holidays.
Minimum Qualifications:
This knowledge is typically acquired through completion of high school or GED is required.
Hourly Rate: $18.15 + DOE
Shift Differential:
Day shift $1.00 an hour.
Evening and night shift $2.00 an hour.
Weekend hours are Friday from 11:00 pm until 9:00 am on Monday, $2.00 an hour.
