The Department of Ophthalmology at the University of Pittsburgh is seeking an optometrist to join our department to provide top quality patient care with a clinical emphasis in specialty contact lenses including specialty soft, rigid gas permeable, hybrid, scleral, Pentacam CSP, EyePrint Prothetics and cosmetic contact lenses. Residency training strongly preferred but not required if the optometrist has 3+ years of experience in specialty contact lens fitting.

Albany Medical College seeks an individual to join our team as a full-time faculty member in the Department of Obstetrics and Gynecology, Division of Maternal Fetal Medicine (MFM). The Division supports the MFM needs of the entire Capital District with the only Level IV NICU in the region. The selected physician will provide maternal fetal medicine services to patients (high risk ultrasounds, consultations, and antepartum services) within Albany Med Faculty Physician Group and its inpatient facilities, and will participate in the education and training of medical students, residents and fellows.
Successful candidates should be board eligible or board certified by the American Board of OBGYN and have the ability to obtain medical licensure in the State of New York. The selected candidate should possess outstanding clinical skills and must work independently and collaboratively with other health care providers and support staff in both inpatient and outpatient settings. Faculty rank appointment will be commensurate with experience and qualifications. It is expected that the selected candidate will pursue scholarly activities that will lead to presentations at regional and national meetings, and/or publications in peer-reviewed journals.
The Department of Obstetrics and Gynecology has five clinical divisions in the department including General Obstetrics and Gynecology, Maternal Fetal Medicine, Gynecologic Oncology, Family Planning, and Urogynecology. The department supports 24 residents and 3 MFM fellows (1 per year). The division of MFM performs fetal echocardiography, amniocentesis, chorionic villus sampling, and more advanced procedures.

With 1 in 7 U.S. adults estimated to have chronic kidney disease (with many progressing to dialysis), SCM partners with health plans to manage these high-risk, high-cost claims through a platform of predictive data analytics, proactive clinical programming, and financial cost-containment and claims underwriting. Role Summary
As a Clinical Health Coach, your role is central to SCM's clinical efforts, collectively designed to deploy best in class clinical methodologies and coaching methods to improve member health. You'll be interacting with members at all stages of kidney disease: pre-disease (addressing common co-morbidities); CKD Stages 1-5; ESRD. A Clinical Health Coach has strong assessment skills, a firm command of the nursing process and a passion for empowering members to improve their self-care as the means to identify and reduce their risk for ESRD.
Clinical Health Coaches make proactive outreach and regular follow-up calls to members who carry complex (and often multi-morbid pathophysiology) with compassion and effectiveness. Our RNs are expected to work to the top of their licensure for the benefit of members, families, and employer groups served by our kidney disease management programs. In addition, you'll act as the clinical expert to support Sales and Operations teams from time-to-time.
How You'll Contribute
Possess a firm command of nursing processes and work full-circle with each member:
Obtain member health assessment and history via telephone or secure video chat using motivational interviewing and a coaching framework.
Gather and utilize pertinent medical records including laboratory data and communicate discrepancies back to MD/providers.
Utilize industry guidelines to provide and ensure high-quality and appropriate care.
Utilize tools/formulas to calculate key metrics and facilitate risk identification (GFR, KFRS, UPCR-UACR, etc.)
Develop care plans and evaluations to ensure your members are on the right track.
