This 12-week hands-on enrichment program will offer instruction to high school juniors to develop their understanding of careers in Respiratory Therapy, Surgical Technology and Sterile Processing. Students can expect to be enlightened on, but not limited to exploration of Respiratory Therapy, exploration of Sterile Processing, exploration of Surgical Technology, CPR certification/Heart Saver and mock code, shadow and hands-on experience, college and on-the-job training, collaboration with experts in healthcare and advanced medical training equipment. **This is a PRN position. A caregiver in this role will need to be available for up to 15 hours per week covering mostly evenings and weekends.** A caregiver who excels in this role will: + Perform comprehensive Caregiver risk assessments to determine risk stratification level and follow-up cadence for individualized coaching for up to one year. + Manage a caregiver caseload of 100+ caregivers at various risk stratification levels. + Engage caregivers in the development of an individualized plan to address barriers by reducing and/or eliminating social, psychological, financial, behavioral and regulatory barriers to facilitate successful first-year employment. + Maintain a network of resources for internal and external consultation and assist caregivers with linkage to resources to facilitate optimal support. + Document professional recommendations, coordination interventions and individualized coaching plans in accordance with HIPPA. + Utilize a tracking system to document and track caregiver demographic information, risk stratification level, and interventions, tools and referrals used to support caregiver success. + Act as a liaison between caregivers, unit leadership and Human Resources to support caregiver success and address urgent or emerging concerns. + Collaborate with Human Resources and unit leadership on investigations and development of performance improvement plans. + Educate caregivers on career pathway options in support of career progression and support academic transition. + Track current and emerging entry-level workforce trends and concerns. + Design, revise, and facilitate core curriculum and professional development workshops to support workforce readiness. Minimum qualifications for the ideal future caregiver include: + Bachelor's degree in a human services field (mental health counseling, psychology, social work, etc.) or nursing + Three or more years of related work experience + Ability to facilitate large group training sessions and provide one-on-one coaching to individuals + Ability to build relationships at all levels of the organization by participating on committees, supporting management and advocating for caregivers + Set the tone for caring and compassion through communication and facilitation + Excellent interpersonal skills, as evidenced by supportive and constructive communication + Demonstrated cultural competency across diverse cultures and socioeconomic statuses + Clear and concise written and verbal communication skills Preferred qualifications for the ideal future caregiver include: + Degree-related certification + Master's degree in human services related field (i.e. education, mental health counseling, psychology, sociology, social work) or nursing preferred + One year of experience in group/individual facilitation in a counseling or teaching environment **Physical Requirements:** + May require extended periods of standing, walking or sitting. + Must have normal or corrected vision and be able to clearly communicate verbally by phone or in person. + Must have own transportation to travel to multiple sites. Functions in collaboration with a physician according to a standard care arrangement and specialty certification. Provides advanced nursing care to patients with complex acute, critical, and chronic health conditions, including the delivery of acute care services. PA: Nationally certified licensed healthcare provider who provides primary and or specialty medical care (the practice of medicine) in a variety of healthcare settings. Functions the collaboration agreement of a Physician, (as per O.R.C. 4730) performs complete histories and physicals on patients of all ages to establish and record patient's current health status and to develop a working diagnosis and treatment plan to provide continuing medical care, including daily examinations, writing orders, chart notes, discharging patients and completing discharge summaries. This medical care may be in a variety of clinical settings. PA's are privileged by the Board of Governors. PA's in this role provide advanced care to patients with in any setting in which patients require complex, acute, critical and chronic health conditions. Practice in a setting that requires complex monitoring and therapies and high intensity medical/surgical interventions and/or continuous vigilance within the range of high-acuity care. Cleveland Clinic's Department of General Surgery is currently seeking an experienced Physician Assistant or Nurse Practitioner **(Adult or Acute certified)** to join the Inpatient Emergency General Surgery Team at Main Campus. The hours for this position will be days 7:00am-4:00pm Monday-Friday with rotating weekends, no nights or call. One year of APP experience preferred. New graduates with General Surgery may be considered. **Responsibilities:** + Conducts thorough medical histories, performs complete physical examinations (where indicated), initiates appropriate lab, radiology tests or other special tests required for evaluation of illness, and scrutinizes lab data on new and former clinic patients and hospital admissions to establish a record of the patient's current health status and to develop a working diagnosis and treatment plan. + Must be able to work with collaborating physician. + Performs and interprets, at least to the point of recognizing deviations from normal, common laboratory, radiologic, cardiographic and other routine diagnostic procedures used to identify pathophysiologic processes as privileged by CCF Credentialing Board. + Performs routine procedures such as injections, immunotherapy, suturing and wound care, managing simple conditions produced by infection or trauma, assisting in the management of more complex illness and injury which may include assisting surgeons in conducting operations and taking initiative in performing evaluation and therapeutic procedures in response to life-threatening situations, as privileged by CCF Credentialing Board. + May prescribe and monitor medications as allowed by prescriptive authority through applicable state Board of Nursing/Medicine and CCF policy. + Performs specialized procedures, including bone marrow aspirations, biopsies and harvest, blood draws, designated IV starts, spinal taps, thoracentesis and paracentesis as well as other specific procedures as directed by the physician, following approval by CCF Credentialing Board. + Provides information and answers routine questions of patients regarding their disease, including preventive health maintenance topics such as diet, weight, smoking and topics related to medical specialty, with basic recommendations. + Educates patients regarding their disease, treatments, related drug and treatment side effects and hazards. + Participates in medical team rounds; collaborates with nursing, medical and other healthcare team members regularly to ensure quality patient care Refers patients to specialists as appropriate for consultation or for specialized health resources and treatment. + Ensures continuity of care by serving as a liaison between patient and other members of the multi-disciplinary care team or with other specialty areas as necessary.     Cleveland Clinic's Digestive Disease and Surgery Institute is currently seeking a Physician Assistant or Nurse Practitioner (Acute Care Certified) for a full time position with Liver Transplant. This is not a surgical position. This position would cover a medically complex patient population which would include: adults post-liver transplant in the ICU, inpatient regular nursing floor, as well as cross-coverage of the inpatient pediatric liver transplant patients. This provider will be responsible for the overall medical management of the patient post liver transplant as well as inpatient liver transplant surgical consults. There will also be coverage of the outpatient liver transplant clinics as needed. Daily virtual or onsite visits to local LTACs and subacute facility will also be required on a rotational basis. The hours for this position will be **four 10 hour shifts** per week to include occasional weekends. Coverage and hours may change depending on practice needs. The ideal candidate will be willing to work well with a team, but also be comfortable functioning autonomously. This provider will manage immune suppression, complications of transplant related medications, metabolic syndrome and recurrent liver disease in the new liver allograft. General Hepatology clinic will make up approximately 2.5 days of the week and includes seeing new and established patients for conditions such as elevated liver biochemistries, fatty liver, viral hepatitis, genetic liver conditions and compensated/decompensated cirrhotics. The primary location for this position will be Main Campus, regional sites can be considered based on patient and department needs. Some paracentesis clinic coverage is possible based on candidate interest and department needs.
