What You'll Do
* Provide comprehensive OT evaluations and treatments for adult and geriatric patients in their homes
* Develop individualized care plans focused on functional improvement and safety
* Collaborate with interdisciplinary teams including nurses, PTs, and physicians
* Educate patients and caregivers on adaptive techniques and equipment
* Document care accurately and timely using EMR systems

* Current Tennessee OT license (or eligibility)

KEY RESPONSIBILITIES
* Analyzes comprehensive information from formal and informal assessment that is pertinent to the patient's care and management to determine a rehabilitation treatment diagnosis.
* Collaborates with stakeholders in establishing the plan of care, coordinates and implements care delivery while anticipating patient care needs across the rehabilitation continuum.
* Applies appropriate, evidence-based interventions to ensure effective outcomes and re-evaluates performance to assess patient's progress towards goals.
* Employs strategies to promote successful return to home, community, and school/vocational environments.

TECHNICAL CAPABILITIES
* Rehab Therapist Patient Documentation (Novice): Demonstrates proficiency in documentation practices. Documents in the appropriate templates and utilizes only approved abbreviations within patient documentation. Meets institutional standards for quality and timeliness of documentation.
* Physical/OccupationalTherapist Patient Education (Novice): Demonstrates proficiency in communicating functional impact of assessment findings, treatment interventions, treatment planning diagnosis, and disease process. Considers the communication needs/level of understanding of audience and may share appropriate information/literature. May supervise, educate and train students from professional programs of physical therapy, occupational therapy and athletic training.
* Treatment Planning & Goal Setting (Novice): Demonstrates sufficient proficiency in developing a plan of care that incorporates treatment interventions focused on meeting objective goals. Develops and documents patient focused short and long term goals that are functional and measurable. Identifies the need for and referral to other services, as needed.
* Treatment Implementation (Fundamental Awareness): Demonstrates proficiency in providing treatment consistent with physician prescription and established plan of care. Able to modify treatment plan as needed to demonstrate provision of skilled therapy services including progression of treatment throughout course of care, and revision of treatment interventions of goals not met. May supervise and direct certified athletic trainers (PT only), physical therapist / occupationaltherapist assistants, technicians and volunteers and students. Selects and follows correct protocol for care and utilize evidence based treatments.
* Rehabilitation Therapist Patient Evaluation (Novice): Demonstrates proficiency in evaluating patients with physical deficits including impairments, developmental disabilities, functional limitations, disabilities, or changes in physical function by obtaining information pertinent to the assessment and administering and scoring standardized tests and objective measures of deficit areas. Selects appropriate measures and performs a functional assessment to document patient's status requiring skilled therapy. Interprets the results of the assessment and their significance to the patient, family, referring physician and other members of the health care team.

Performs and implements patient treatments based on assessment and written plan. Performs patient treatment activities to reduce physical, emotional and social limitations including teaching self care skills and the use of adaptive equipment, including mobility, manual dexterity and daily activities; assisting patients to maintain maximum function by teaching proper body mechanics and adaptations to daily activities and assessing patient's potential to return to work and determining appropriate referrals.
Provides ongoing patient assessment through observation of patient and records. Documents and changes in patient's condition and any resulting changes in patient treatment (both physical and human response).
Evaluates treatment plan and patient's response to treatment. Re-assesses short-term goals as needed.

Plans and implements specific treatment programs for individual patients according to principles and practices of occupational therapy. Performs appropriate initial occupational therapy evaluations and re-evaluations and designs an occupational therapy plan of care
Demonstrates advanced knowledge of diagnoses/conditions seen in occupational therapy
Selects, performs, and interprets the results of OT examinations of neurological, musculoskeletal, cardiovascular, and pulmonary systems
Performs and interprets standardized evaluations appropriate for the patient
Identifies actual or potential problems that would impact the OT treatment plan as well as the overall patient plan of care
Establishes specific treatment goals based on the results of the initial assessment
Designs the treatment plan appropriate for the patient's age, educational level, and cultural background
Designs a plan of intervention that is coordinated and integrated with other services.
Revises the treatment plan as indicated
Implements and manages a specific and comprehensive treatment plan, and participates in patient discharge planning
Demonstrates working knowledge of various Occupational Therapy techniques with flexibility and creativity in the implementation of these therapy techniques
Interacts regularly with others involved in the patient's care to incorporate an interdisciplinary treatment approach for coordinated quality care

**About the Role**
We are seeking an experienced Pediatric OccupationalTherapist-ideally Certified Hand Therapist (CHT)-to deliver high-quality therapy for children with upper extremity conditions.
This role involves splint fabrication after surgery, developing home programs, and creating individualized treatment plans. You'll work in a team-based environment with physicians, therapists, and other specialists.
**Key Responsibilities**
+ Perform comprehensive evaluations and create individualized treatment plans
+ Provide splinting and post-operative care for pediatric patients
+ Educate families on home exercise programs and adaptive techniques
+ Deliver evidence-based interventions to improve functional outcomes
+ Collaborate with multi-disciplinary teams across outpatient clinics (and occasionally hospital settings)

- Treatment Planning & Goal Setting (Intermediate): Demonstrates mastery in developing a plan of care that incorporates treatment interventions focused on meeting objective goals. Develops and documents patient focused short and long term goals that are functional and measurable. Identifies the need for and referral to other services, as needed. May serve as a mentor for other clinical staff to develop their skills in treatment planning. May consult with team members or implement new procedures.
- Rehabilitation Therapist Patient Evaluation (Intermediate): Demonstrates mastery in evaluating patients with physical deficits including impairments, developmental disabilities, functional limitations, disabilities, or changes in physical function by obtaining information pertinent to the assessment and administering and scoring standardized tests and objective measures of deficit areas. Selects appropriate measures and performs a functional assessment to document patient's status requiring skilled therapy. Interprets the results of the assessment and their significance to the patient, family, referring physician and other members of the health care team. Demonstrates experience in clinical evaluation skills and may serve as a mentor for other clinical staff to develop their skills in evaluation techniques. Contributes to the development of best practices for the organization. May consult with team members to implement new procedures.
- Treatment Implementation (Intermediate): Demonstrates mastery in providing treatment consistent with physician prescription and established plan of care. Able to modify treatment plan as needed to demonstrate provision of skilled therapy services including progression of treatment throughout course of care, and revision of treatment interventions of goals not met. Supervises and directs certified athletic trainers (PT only), physical therapist / occupationaltherapist assistants, technicians and volunteers and students. Selects and follows correct protocol for care, as established, and actively seek out, utilize and mentor/teach others on current evidence based treatment strategies.
