A DAY IN THE LIFE OF A REGISTERED NURSE (RN) As an RN, you play a key role in assessing the needs of our patients to create and implement their plan of care. In accordance with evidence-based practices, state and federal regulations, and our company's policies, you serve as a case manager for assigned patients. Responsibilities include but are not limited to the following: Actively manage the patient's treatment and needs. Maintain up-to-date and accurate documentation of nursing care provided to ensure the effective integration of information. Take action to ensure that infection control standards are maintained and stay educated on a variety of high-tech skills, including IV use and maintenance, wound VAC, and complex wound care. Carry out visits with patients in a timely manner and ensure that patients and caregivers are educated on health matters Coordinate with a wide variety of physicians, providers, team members, and vendors in order to fulfill the recommended plan of care. Assist with any patient discharges or transfers and complete all the correct paperwork and reports. QUALIFICATIONS FOR A REGISTERED NURSE (RN) Registered Nurse (RN) license 2+ years of nursing experience is preferred Have self-direction and initiative, as well as be able to work collaboratively with colleagues within the agency. Experienced with electronic medical records required. Willingness to work a flexible schedule, including rotational weekends, holidays and on call for both hospice and home care patients. Exceptional bedside manner and communication skills. Compassionate and friendly demeanor. Valid Vermont Driver's License and car insurance. Do you enjoy helping people?
