The Department of Pharmaceutical Systems and Policy in the School of Pharmacy at West Virginia University is currently accepting applications for a Pharmacist at the Rational Drug Therapy Program. About the Opportunity As a Pharmacist at Rational Drug Therapy Program, you will be responsible for conducting clinical reviews of prior authorization requests, performing concurrent drug utilization review with program instructions, and providing general pharmacy information using the point-of-sale computer system established by the West Virginia Bureau of Medical Services (WVBMS) and the current point-of-sale computer system utilized by the PEIA program. Through this role, you will assist in carrying out Rational Drug Therapy Program's mission to ensure WV Medicaid, WV PEIA, and CHIP members receive safe, rational, cost effective, and patient outcome-oriented drug therapy. In this role, you will additionally be performing reviews of prior authorization requests for the Peak Healthcare program in the computer system utilized by that program.

What You'll Do
Provide professional services to the WVBMS which include but are not limited to:
* Prior authorization clinical review.
* Evaluations of appropriateness of therapeutic duplication.
* Recommendations of appropriate formulary alternative.
* Conduct reviews of home infusion therapy request.
* Literature reviews applicable to prior authorization request.
* Clinically evaluate the appropriateness of various infectious disease drug therapy regimens.
* Evaluate the clinical appropriateness of drug therapy approval with newly marketed medications as allowed by the program.
* Evaluate claims for accuracy of billing.
* Review individual prior authorization, drug utilization review, and billing inquiries and use clinical decision-making skills and therapeutic judgment to determine if claim payment can be approved.
* Acquire and maintain clinical knowledge and information on various disease states in which the program is involved via current, evidenced-based recommendations.
* Review inquiries from technical employees of the program and ascertain the clinical appropriateness of the outcome in comparison to the drug utilization evaluation against which the inquiry is being presented.
* Respond to inquiries in a friendly, professional manner.
* Assist in the orientation and training of all new professional and non-professional personnel.
Weekend staffing of the program on a rotating basis (approximately every 7-10 weeks depending on staffing needs).
* Bachelor's degree in Pharmacy or Doctor of Pharmacy Degree
* A minimum of one (1) year of experience in the following:
* Retail or hospital pharmacy experience
* All qualifications must be met by the time of employment.

* Participates in quality improvement and quality assurance initiatives.
* Perform other duties as required.
Qualifications

Responsibilities:
GENERAL PURPOSE
Centralized order verification pharmacists are responsible for optimizing safe and effective drug therapy by assessing new medication orders for adult patients in the acute care setting. This is accomplished by performing medication order verification and providing evidence-based drug information to other healthcare professionals to optimize medication therapy and patient outcomes. The centralized order verification pharmacist will also have a working knowledge of the operational aspects of the department and will assist with training other pharmacy staff or learners. The initial training period will include on-site learning at Strong Memorial Hospital (SMH) and then may be expanded to include de-centralized training at an office. Upon completion of on-boarding and demonstration of order verification competency and efficiency, centralized order verification pharmacists will work remotely (either from a decentralized office or from home). Centralized order verification pharmacists must provide services from within NYS and may be called to report to work on site (SMH) in the event of an emergency incident that results in an inability to process orders electronically (e.g., extended electronic medical record [EMR] downtime) or a significant reduction in order volume (e.g., event causing prolonged significant decrease in patient volumes). Although these events are rare, it will require living within an approximately two-hour commute to SMH. Centralized order verification pharmacists will have a rotating schedule including day shifts, evening shifts, weekends, and holidays.
ESSENTIAL FUNCTIONS
Medication Authorization and Distribution:
Understand and follow departmental and hospital policy and procedures related to accurate medication order verification
Processes/validates medication orders utilizing current information system technology to create and maintain accurate patient profiles
Maintains current understanding of dispensing practices across all hospitals covered in order to facilitate safe and timely medication dispensing at each location
Maintains assigned patient order queues and all processes related to ensuring all patient care orders are evaluated and verified in a timely manner
Verbal communication to all hospital staff is conducted in an effective and professional manner
Clinical Responsibilities:
Use clinical knowledge and professional judgement to assess the appropriateness of medications prescribed by identifying potential medication interactions and contraindications as well verifying appropriate drug administration routes, doses, and instructions
Collaborate with providers, nurses, and other healthcare professionals to guide and recommend appropriate evidence-based medication use based on patient-specific information to optimize medication therapy
Ensure that medication orders align with patient-specific factors such as medical history, allergies, and lab results
Documents clinical activities and interventions on a regular basis as related to order verification responsibilities
Problem Solving:
Thoroughly resolves problems with appropriate and direct communication to all pertinent staff in a clear and concise manner.
Proactively identify and resolve patient care issues and problem medication orders, document clinical interventions, and communicate appropriate handoff at change of shift
Appropriately identify orders that require on-site, direct patient care follow up and ensures all information is communicated in a timely manner
Assists in coordinating and implementing staffing plans based on available resources, conflict resolution, and appropriate communication from shift to shift or across order verification team.
Contacts supervisor for assistance when solution is beyond his/her expertise or authority.
Collaborate with providers, nurses, and other healthcare professionals to guide and recommend appropriate evidence-based medication use based on patient-specific information to optimize medication therapy
Training and Education:
Participate in the education and mentoring of other pharmacy staff or learners, as needed (newly hired pharmacists, residents, technicians, students)

MINIMUM EDUCATION & EXPERIENCE
