    Clinical Pharmacy Manager - Ambulatory Transplant We are seeking an experienced and motivated Ambulatory Clinical Pharmacy Manager to lead outpatient solid organ transplant pharmacy services. In this vital role, you will oversee clinical program development, guide CDTM protocols, and provide advanced direct patient care to support exceptional outcomes for transplant recipients. Key Responsibilities Program Leadership Lead strategic direction for the Solid Organ Transplant Clinical Pharmacy Program in alignment with institutional and regulatory standards. Develop, implement, and evaluate clinical pharmacy services for kidney, liver, heart, and/or lung transplant programs. Maintain clinical guidelines, protocols, and CDTM agreements. Direct Patient Care Deliver comprehensive pharmacotherapeutic management for pre- and post-transplant patients. Optimize medication therapy through monitoring, pharmacokinetic consultation, and individualized treatment plans. Conduct medication reconciliation, education, and discharge counseling for patients and donors. Support safe transitions of care across inpatient and outpatient settings. Collaboration & Education Partner with transplant providers and care coordinators to integrate pharmacy services into care pathways. Precept pharmacy residents and APPE students during transplant ambulatory rotations. Regulatory Compliance Participate in quality initiatives and ensure compliance with CDTM and regulatory requirements. Research & Performance Improvement Contribute to clinical research, quality improvement, and performance metrics related to transplant care. Present outcomes and support scholarly activities with residents and students. Stay current with emerging evidence and best practices. Eligibility Requirements Education & Licensure Doctor of Pharmacy (PharmD) from an ACPE-accredited program. Active NYS Pharmacist License and Registration. PGY2 Solid Organ Transplant Residency highly preferred. Board Certification (BCTXP) preferred. Experience Leadership or supervisory experience preferred. Academic medical center or teaching hospital experience desirable. Research and quality improvement experience preferred. Skills Strong expertise in transplant pharmacotherapy. Reporting to the Director of Clinical and Educational Pharmacy Services and supervising two clinical pharmacists, the Clinical Pharmacy Manager is responsible for optimizing pharmacotherapy outcomes through direct patient care, interdisciplinary collaboration, and provider education. Additional responsibilities include evaluating cancer-related medications such as antineoplastic and supportive therapies, contributing to research and quality initiatives, and partnering with revenue cycle teams to expand billable services and strengthen compliance and reimbursement. Education & Licensure Requirements: Doctor of Pharmacy (PharmD) degree required Active license to practice pharmacy in the State of New York Board Certification in Pharmacotherapy or equivalent certification preferred Eligible for credentialing as an Allied Health Professional Experience: Completion of a PGY2 Residency in Oncology is strongly preferred. In lieu of a PGY2, completion of a PGY1 Pharmacy Residency and a minimum of 3 years of direct clinical experience in oncology pharmacy practice is required. Performs pharmacist tasks including compounding, drug therapy reviews using EMR when it is a mutual WYH clinic patient, verification, and medication management. Counsels patients and answers their questions regarding usage of medicine, side effects, interactions, contraindications, patient information privacy, generics, less expensive medicines, over-the-counter products, and refers to medical provider as needed to ensure medication is taken correctly, health needs addressed, and satisfaction with service. Reviews, interprets, and accurately dispenses prescribed medications. Follows-up with medical providers' offices to clarify prescribed medications, dosages, refills, interactions, and allergies to suggest alternative medications, and answer medical provider questions. Performs clinical and wellness services such as immunizations, diagnostic testing, disease state management and other healthcare services. Operational Responsibilities Promotes teamwork and motivates team members by establishing expectations, monitors and recognizes progress, and fosters a shared vision. Improves prescription numbers, pharmacy sales, profit, and image through the accurate filling of prescriptions, and by providing courteous and efficient service to patients, prescribers, pharmaceutical manufacturers, payers, and health systems. Reports prescription errors immediately and adheres to WYH policies and procedures relating to prescription incidents. Assures accurate and timely reporting of inventory for limited distribution drugs (LDD) when required (daily, weekly, and monthly). Improves and maintains pharmacy department housekeeping, maintenance, and appearance for the safety, health, and well-being of patients and team members, and ensures adherence to applicable government regulations and laws. Complies with all federal and state laws and regulations and Company policies governing all drugs and products; maintains required records, documentation, and reports. Assures proper operation and maintenance of pharmacy department systems and equipment. Maintains awareness of third-party pharmacy business opportunities and assures proper administration and implementation of third-party pharmacy systems and procedures to lessen payment rejections. Resolves third-party rejection problems promptly and resubmits rejected claims for payment promptly. Manages and controls Prescription Department inventory. Adheres to WYH buying procedures, recommends and orders merchandise from proper internal and external sources. Miscellaneous Ensures the use of all elements of the Good Faith Dispensing policy in conjunction with state and federal controlled substance laws when filling prescriptions. The Product Review/Retail Fill Process Pharmacist is responsible for ensuring that elements of Good Faith are present. Maintains current knowledge of information technology associated with pharmacy systems including workflow, prescription fulfillment, billing, clinical documentation, training, inventory management, and point of sale registers to provide support to patients as well as pharmacy staff. Seeks new and better ways to further promote productivity. Maintains current knowledge and required licensing/credentialing/certification as established by federal and state regulations to provide such clinical services. Maintains current knowledge and skills related to pharmacy and healthcare by reading pharmacy related journals, updated treatment guidelines, and communications; pursuing best practices that would enhance performance. Obtains necessary certifications, education credits and training, including learning modules, as required by WYH. Demonstrate excellence in both internal and external customer service. Understand and adhere to HIPAA compliance, corporate compliance and client confidentiality. Ensure and/or remain in compliance with local, State, and Federal regulation, i.e. DHHSHRSA and NYSDOH, and all accreditation standards (e.g. Joint Commission and NCQA-PCMH). Adhere to the National Patient Safety Goals as defined by the Joint Commission and WYH. Complete other duties as assigned. Requirements MINIMUM QUALIFICATIONS: Graduate of an approved Pharmacy Program (Bachelor of Pharmacy or PharmD program). At least five (5) years of pharmacy experience. At least two (2) years of supervisory experience. Current Basic Life Support (BLS) certification. Effective oral and written communication skills. Proficient knowledge of Microsoft Office.
