    Job DescriptionDescription: At Home Healthcare, Hospice and Community Services (HCS), we are committed to providing compassionate, patient-centered end-of-life care that supports the physical, emotional, and spiritual well-being of our patients and their families. We are seeking a dedicated facility-focused Clinical Care Transition Specialist - RN to provide timely hospice informationals, evaluations for hospice clinical eligibility, and admissions. Referrals are from the HCS VNA, hospitals, facilities, and patients/families. In addition, when facilities have patients on Hospice General Inpatient Level of care this position will perform oversight and visits. The Clinical Care Transitions Specialist - RN works with the intake department to complete timely informationals, hospice clinically eligible assessments, and admissions with a primary focus on facilities and hospital settings. This role would provide formal and informal hospice training to facility staff from basic Hospice 101 to Pain and Symptom Management. Partnership with the Hospice Medical Director and Clinical Leadership will be critical for eligibility determination. In collaboration with the hospice team, the Specialist ensures the timely, coordinated care of patients transitioning from home to a facility or hospital to home and other care settings. The position is focused on ensuring seamless coordination of care with referral sources and the hospice team. Responsibilities: Support increasing access to hospice by collaborating internally with HCS team members and with HCS referral sources. Responds to requests for informationals timely which lead to assessments for clinical eligibility and admission, working closely with the hospice provider and clinical leadership. Completes hospice admissions Educates facility staff to identify hospice appropriate patients through formal presentations as well as one on one interactions Partners with facility administrators, directors of social services, directors of nursing as well as floor staff to build relationships which lead to seamless customer and clinical service. Completes GIP admissions Completes same-day hospice admissions in community or facility settings as needed. Able to cope with family emotional stress and tolerance of individual lifestyles. Skill in using psychological principles in both one-on-one and group situations. Knowledge of measures to comfort the dying. Emotional stability, flexibility, and respect for various religious and spiritual beliefs.
