We are excited to partner with Federally Qualified, Non-Profit Health Centers in California to enable their participation in CalAIM's new Enhanced Care Management Medi-Cal benefit program, which provides long-term, whole-person care coordination, inclusive of behavioral health and social needs supports.
We seek a part-time Family Physician to play a critical role in our whole-person, interdisciplinary care model. The Family Physician will be responsible for providing supervision for our Nurse Practitioners who provide multidisciplinary care to individuals living with Serious Mental Illness/ Substance Use Disorder, experiencing homelessness, and/or those who have high medical needs. We believe in the power of trust and relationships to successfully engage those who may have never received the kind of whole-health care that Pair Team can provide. The Family Physician will also have the opportunity to provide input on clinical programs that our team is developing. The Family Physician will report to our physician leadership.

Essential Functions:

Test water quality in the pool and maintain daily records; conduct proper maintenance and sanitary upkeep of the pool, pool area, and locker rooms; participate in the preparation of daily records; provide written reports on all accidents and incidents; maintain updated safety requirements.
Attend in-service training and teach said skills to staff; conduct patron swimming tests to assess their ability to swim independently; provide customer service to Community Center users, including handling customer conflict and complaints.
Perform work safely in accordance with departmental safety procedures; operate equipment safely and report any unsafe work conditions or practice to supervisor.
Participate in the daily preparation of aquatic programs; assist the Community Center Foreperson/Aquatics Manager in daily aquatic managerial duties; may be required to maintain chemical balances in mechanical/pump room or chemical problems as necessary.
Capable of working as a leader and team member of the staff and maintain positive work relations with fellow staff members and other departments.
Notify supervisor and find appropriate coverage due to illness or emergency.
Responsible for being on time and in uniform (swimsuit, ID, etc.).
Perform other related duties as required by the department head.
Education and Experience:
A High School Diploma or GED is required.
A minimum of two (2) years of related work experience preferred. Experience in water rescue methods, lifeguarding, and first aid practices, techniques and swimming pool operations (sanitation, maintenance, and safety).
Must possess a valid American Red Cross Life Guarding Certificate and American Red Cross First Aid Certificate or equivalent.
Must be certified in American Red Cross Cardiopulmonary Resuscitation (CPR) for the Professional Rescuer or equivalent. Skills:
Must be able to courteously and firmly interact with pool patrons
Ability to clearly and effectively communicate with Community Center Foreperson/Aquatics Manager, colleagues within the parks programming department, other BPCPC departments, site hosts, and colleagues at Stuyvesant High School.
Physical Requirements:
Ability to complete a continuous 500-yard swim and submerge to a minimum depth of 7” to retrieve a 10lb object and return to the surface.

Under the supervision of the Director of Housing and Shelter Programs, the Program Manager is responsible for the operation and security of the shelter sites and the supervision of shelter guests. The Program Manager supervises all staff, ensures they are working as a team and following strict program procedures. Essential Duties and Responsibilities:
Ensure the safety and security of shelter sites and respond to emergency situations, as needed.
Hire, train, supervise, and evaluate full- and part-time shelter monitors.
Coordinate, plan, and facilitate staff training and meetings.
Schedule staff on a weekly/bi-weekly basis
Consistently enforce all shelter policies for staff and program participants
Assist Director of Housing and Shelter Programs with program planning, implementation, and evaluation
Make regular reports to management using daily logs, forms, e-mail, and phone calls; provides pertinent shelter data to Director of Housing and Shelter Programs for contract and grant reports.
Provide after-hour on-call support as needed for urgent shelter matters; cover shifts in emergency situations, as needed.

Case managers receive the SFUSD referrals and coordinate with the Program Manager to schedule new intake.
The Program Manager is responsible for keeping a daily log of all referrals for the Stay Over Program. The daily log includes such information as referring entity, family size, contact information, household information and status update on the referral. This daily log of referrals is the most updated information source of families in the program.
Mediate and de-escalate conflicts.
Remain awake and alert during assigned shifts and ensure that other night staff are also doing so.
Attend organizational and departmental meetings, trainings, and activities.
Coordinate other shelter operations and perform other duties as assigned.
Maintain professional attitude and work ethic.
Comply with Mission Action policies and procedures, and laws pertaining to the operation of Mission Action.
Participate in ongoing professional development throughout the year.
Cover shifts as needed for last minute call outs and other emergencies
Qualifications:
At least 2 years of experience providing services to homeless or extremely low-income adults, the LGBTQ community, day laborers and/or immigrants.
At least 2 years of college or university education in a related field strongly preferred.
At least 2 years of experience training and supervising staff required; program oversight/management experience strongly preferred.
Bilingual fluency in spoken English and Spanish, and the ability to write fluently in at least English, required.
Must be a self-starter, able to work independently, and be reliable.
Ability to stay awake during any assigned shift, lift approximately 30 pounds, and navigate stairs and narrow workspaces.
