ESSENTIAL FUNCTIONS:
* Collaborate with interdisciplinary treatment teams, other departments and administration to ensure that all residents' physical, biopsychosocial, age, developmental and cultural needs are met and when they are not met, acknowledge and work to resolve customer complaints.
* Anticipate and effectively manage changes in census and acuity and allocates nursing resources based on measurement of patient acuity/care needed.

* Accountable for the standard of care at the facility, providing consultations and expertise in the delivery of care to patient.
* Serve as a clinical and service resource to nursing staff and other staff as assigned.
* Manage the activities of staff, coordinating safe and appropriate care between departments and disciplines.
* Manage and evaluate work activities of nursing, technical, clerical, service or maintenance staff for the unit, work group and other facility staff as directed.
* Analyze information/situation to choose the best solution(s) to solve problems.
* Identify the educational needs of others and develop educational or training programs.
* Assist with the development and implementation of organizational policies and procedures for the facility.
* Prepare reports to inform management of the status and implementation of programs, services and quality initiatives of the unit.
* Perform administrative or managerial functions such as planning budgets and authorizing expenditures.
* Provide staff management to include hiring, development, training, performance management and communication to ensure effective and efficient department operation.
* Provide direct care as needed.

EDUCATION/EXPERIENCE/SKILL REQUIREMENTS:
* BSN or higher strongly preferred.
* Current registered nursing license in State of facility required.
* Three years as a registered nurse strongly preferred.
* Two years of registered nursing experience required.
* Two years of prior experience as a registered nurse with patient population of the facility is preferred.
* One year of prior experience with patient population is required.
* Prior experience as a house supervisor strongly preferred.
* One year of charge RN or other administrative experience required.
LICENSES/DESIGNATIONS/CERTIFICATIONS:
* Current RN license as required by state.

* Rounds are completed at the beginning of shift and perform random audits.
* Assist DON to make sure all staff are oriented and competent to perform required duties. May include coaching staff.
* Provide mentoring and training as needed.
* Assist DON by ensuring facility compliance with professional, regulatory and governmental standards.
* Ensure the effective delivery of competent, compassionate and safe care by monitoring and evaluating patient care processes and outcomes on the patient care units.
* Coordinate all care rendered to patients, including physician orders and diagnostic information, and ensure consultations are ordered.
* Document medical, behavioral health information and nursing care.
* Administer medications and maintain records in accordance with company policies and procedures.
* Assist in developing a plan of care with specific and measurable goals, objectives and interventions defining actions unique to each patient's needs. Assist in updating and revising the plan of care as goals/objectives are met or when the patient's condition changes.
* Prioritize tasks and delegate staff to ensure quality patient care.
* Leader on shift for all admissions and discharges, as well as informing physician of changes in the patient's conditions. Adjusts staffing accordingly.
* Utilize administrator-on-call for situations that require additional attention.
* Provides direct care as needed.

Job Summary
The Care Manager - RN is responsible for coordinating and overseeing discharge planning, transitions of care, and case management activities to ensure optimal patient outcomes. This role involves collaborating with interdisciplinary teams, reviewing medical records for appropriateness and medical necessity, and maintaining compliance with federal, state, and accreditation standards.
Essential Functions
Conducts daily reviews of medical records to assess the appropriateness of admission, continued hospital stay, and utilization of diagnostic services.
Collaborates with interdisciplinary teams (IDT) to ensure effective communication and coordination of patient care, including identifying avoidable days and resolving care transition issues.
Develops and implements discharge plans, coordinating post-hospital placement and social services to meet patient needs.
Refers cases to physicians or managers when patients do not meet established criteria, ensuring timely and appropriate interventions.
Serves as a liaison with community agencies, maintaining relationships and facilitating seamless transitions for discharged patients.
Facilitates interdisciplinary meetings to address patient care needs, resolve challenges, and support collaborative care planning.
Maintains accurate and timely documentation of case management activities, including records of referrals, patient interactions, and compliance with reporting requirements.
Identifies and appropriately refers cases to Child/Adult Protective Services, ensuring compliance with legal and ethical standards.
Provides professional assistance to patients, families, and physicians regarding discharge planning and post-hospital care options.
Performs other duties as assigned.

Qualifications
Bachelor's Degree in Nursing preferred
2-4 years of clinical nursing experience in a hospital, home health, or nursing home setting required
2-4 years of care management experience preferred
Knowledge, Skills and Abilities
Strong understanding of case management principles, discharge planning, and transitions of care.
Knowledge of federal, state, and Joint Commission standards related to case management.
Excellent communication and interpersonal skills to collaborate effectively with patients, families, and interdisciplinary teams.
Ability to assess complex situations, identify solutions, and implement care plans efficiently.
Proficiency in electronic medical records (EMR) and documentation systems.

Assists in examination and treatment of patients under a Provider's direction by performing the following duties.
A Day in the Life

Customer care:
· Conducts outreach to patients for Annual Wellness Visits (AWVs), chronic care management, and preventive health screenings.
· Educates patients and families on self-management strategies, chronic disease care, and lifestyle modifications to support overall wellness.
· Triages patient calls and walk-ins, addresses routine concerns, and escalates clinical issues as appropriate.
· Documents all patient interactions and communications in the electronic medical record (EMR) with accuracy and timeliness.
· Encourages patients to stay current with preventive services, immunizations, and chronic condition management.
Clinical:
· Conducts Medicare Annual Wellness Visits and other preventive health assessments, documenting findings and collaborating with Providers to establish or update care plans.
· Administers vaccines and injections, including psychotropic medications, per Provider direction and organizational protocols.
· Assists Providers with clinical procedures, medication administration, and vital sign collection.
· Closes care gaps by ensuring patients receive recommended screenings, labs, immunizations, and follow-up visits.

As the Registered Nurse Case Manager, you will assume full nursing responsibility for the delivery of the Plan of Care for each hospice patient. Continuously evaluating personal and professional performance and making necessary changes to increase productivity and quality of care delivered. You'll also develop the patient's Plan of care in collaboration with other professionals and implement treatment strategies based on scientific nursing theory that promotes physical, psychosocial, emotional, and spiritual well-being. Familiar with the concepts and needs of patients/families who are facing death and dying.
**Primary Responsibilities:**
+ Makes the initial nursing evaluation in determining eligibility for hospice services during visit within forty-eight (48) hours of referral
+ Identifies the patient's/family's physical, psychosocial, and environmental needs and re-assesses as needed, no less than every fifteen (15) days
+ Initiates and coordinates the plan of care
+ Documents problems, appropriate goals, interventions, and patient/family response to hospice care
+ Collaborates with the patient/family, attending physician and other members of the IDG in providing patient and family care daily
+ Instructs and supervises the patient/family in self-care techniques when appropriate

**Required Qualifications:**
+ Current and unrestricted RN licensure in the state of practice
+ 1+ years of clinical experience
+ Current CPR Certification
+ Current driver's license, valid vehicle insurance, and access to a dependable vehicle, or public transportation
+ Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client
**Preferred Qualifications:**
+ Able to work independently
+ Good communication, writing, and organizational skills
