Be the Best Hospice Admissions RN You Can Be
If you meet these qualifications, we would love to meet you:
Licensed as a Registered Nurse in the state(s) of practice, with an associate degree preferred and CPR certification recommended (required for direct patient care in Georgia). Must meet state-specific regulatory requirements.
Previous nursing experience in hospice, home health, general medical-surgical, or population-specific care (e.g., pediatrics, geriatrics) is required, along with willingness to serve on-call.
Must have a reliable vehicle, valid driver's license, and the minimum required liability auto insurance.
Strong communication and organizational skills, with working knowledge of Microsoft Office Suite and EMR systems. Must be able to work effectively as part of an interdisciplinary team.
Physically able to lift patients over 200 pounds (with assistance), stand, walk, sit, stoop, and perform other physical movements frequently. Regular exposure to indoor environments, vibration, and moderate noise expected.
Responsibilities:
Assess patient and family for physical, psychological, social and spiritual needs and collaborates with patients and
families, physician, and Interdisciplinary Group (IDG) to establish the plan of care, implement interventions and/or
evaluate care
Initiate and maintain communication with attending physician, Medical Director, IDG, Team Director (TD), and IDG
and other agencies to coordinate optimal care and use of resources for the patient and family
Especially, in the event of unusual or potentially problematic patient and family issues
Document according to protocol and maintain up-to-date patient records. Ensure plan of care problems, interventions, actions and goals reflect baseline, current, and ongoing status,
and changes as they occur
Participate in On-Call or Admissions assignment/rotation, as directed
Provide education to patient and caregiver(s) as needed, based on knowledge needs and learning assessment,
patient and family condition, in keeping with plan of care

The Office Coordinator plays a vital role in supporting daily branch operations and ensuring compliance with non-clinical policies and practices. This position is key to maintaining accurate medical records, coordinating patient care, and providing essential administrative support to the hospice team.
Essential Functions of the Position
Maintain and close medical records per policy.
Review records to ensure compliance with documentation standards.
Identify and obtain missing chart information, including physician signatures and other required documentation.
Manage appropriate release of information, including obtaining authorizations and distributing copies.
Collaborate with clinical staff to support timely admissions.
Conduct medical record audits as requested.
Purge closed cases, prepare records for archival, and maintain the master patient index.
Participate in hospice quality assessment and performance improvement programs.
Oversee tactical HR processes, contracts, and personnel files for staff and contractors.
Support branch leader with new hire orientation and onboarding.
Serve as a resource connecting staff to IT, HR, and Compliance as needed.
Prepare weekly schedules, review for productivity and geographic efficiency, and coordinate with the Executive Director for approval.
Coordinate interdisciplinary team meetings and manage care coordination communication.
Provide administrative support including answering phones, ordering supplies, and data entry.
Process accounts payable and route invoices as needed.
Complete annual compliance training and attend in-services as required.
Perform other duties as assigned.

Education
High school diploma required.
Experience
Minimum of three years of experience in the healthcare industry.
Strong preference for medical records experience.

About the Role
The Afterhours Care Manager RN is responsible for assessing and coordinating patient and family needs during after-hours shifts and maintaining primary clinical oversight for assigned patients. This includes routine visits, admissions, death visits, urgent after-hours calls, and any visit assigned per the patient's plan of care. This role serves as an active member of the Interdisciplinary Group (IDG) to ensure coordination between physicians, families, and the hospice team. Support the IDG process through chart reviews, plan of care updates, and interdisciplinary communication.

Communicate significant changes in patient condition promptly to hospice and attending physicians.

Participate in calendar and scheduling modifications, including service codes and clinician assignments.
Support administrative and compliance activities including workflow audits, survey readiness tasks, and documentation collection.
Assist with TPE reviews, QAPI efforts, and regulatory compliance.
Ensure infection control standards and universal precautions are consistently followed.
Provide leadership and guidance to field staff; conduct supervisory visits when appropriate.

Associates degree, preferred
Registered Nurse license required in state(s) practicing
Current and valid CPR certification, preferred - required in Georgia

Responsibilities:
Conducts comprehensive nursing assessments and develops individualized care plans in coordination with the interdisciplinary team (IDG), addressing physical, emotional, social, and spiritual needs
Maintains timely and accurate documentation of care, interventions, and patient status updates, ensuring compliance with internal protocols and regulatory requirements
Coordinates care activities including visit scheduling, medication management, physician collaboration, and interdisciplinary communication to ensure continuity and quality of care
Provides education and support to patients and caregivers, and responds to urgent needs during on-call or admissions assignments
Supervises LPNs, LVNs, and hospice aides as needed, and participates in IDG meetings, patient conferences, and ongoing clinical education
Supports pre-hospice services by evaluating patient status via phone, telehealth, or in-person visits and identifying needs for additional care or services

Essential Functions of the Certified Nursing Assistant (CNA):
* Provides for patient's personal hygiene, grooming, and comfort needs, i.e., toileting and elimination, baths, backrubs, shampoos, shaves, bed baths, tub baths or showers, skin and nail care, and oral hygiene.
* Promotes a safe environment during care.
* Utilizes infection control measures such as hand washing and use of personal protective equipment.
