PHP Telehealth Administrator on Call
PHP Telehealth Administrators on Call have a passion for providing remote administrative, clinical triage, and care coordination support for the PHP Telehealth provider team, as well as seniors and other residents in a variety of healthcare settings that can include, but are not limited to, skilled nursing facilities, assisted living, and independent living communities, etc. They deliver support in a high touch, person-centered care model and collaborate with the interdisciplinary team to ensure that all residents receive the right care, at the right time.

Personalized Health Partners (PHP) is the medical practice arm for the CommuniCare Family of Companies. The Administrator on Call would directly support both the PHP Telehealth provider group as well answering calls for triage from our other telehealth use cases outside of the Skilled Nursing and Assisted living environments. The goal of the Administrator on Call is to support the Telehealth provider team as well as our patients to increase wellness, prevent illness, improve clinical outcomes and focus on customer experience and satisfaction.
PRIMARY RESPONSIBILITIES AND ESSENTIAL FUNCTIONS:
Duties of all PHP Telehealth Administrators on Call
Coordinate with telehealth providers to ensure appropriate patients receive telemedicine services.
Communicate with onsite nursing teams to ensure all clinical needs are coordinated
Oversee provider queue in Never Alone
Support technical issues that arise for telehealth provider team
Monitor EMR to triage routine clinical lab requests, routine imaging results, notifications, etc. that result after hours.
Maintain timely documentation of encounters with facility nurses in the EMR
Record information directly into CRM system
Connect patients that need to be seen acutely by an in-house provider the next day with Central Scheduling
Answer and triage calls from Never Alone use cases outside of the SNF use case
Provide health education
Provide an excellent customer experience to foster high customer satisfaction/retention
Practice ethically and in accordance with the Scope and Standards of Practice of their profession and Board Certification.
Follow all state and federal regulations, guidelines, and laws

Collaborate with telehealth provider group
Participation in monthly staff meetings
During downtime there may be some additional project related work related to the development of protocols, initial quality review of previous encounters, or projects to be defined as the role evolves.
EDUCATION and QUALIFICATION:
Required:
Must have valid RN license
Must be willing to be licensed in the 6 states served (Indiana, Ohio, Pennsylvania, Maryland, Virginia and West Virginia)
Strong clinical assessment and medical triage skills, with the ability to prioritize care.
Excellent customer service skills, with the ability to collaborate effectively with patients and healthcare teams.
Strong critical thinking and problem-solving abilities to manage a wide range of scenarios.
Ability to provide care and guidance to both direct-to-consumer clients and healthcare organizations.
Compassionate and empathetic approach to patient care.
Good technology aptitude. Accurate and efficient in typing.
Must be able to work independently and manage time efficiently in a remote environment.
Flexibility to work various shifts, including evenings, weekends, and holidays.
Preferred Qualifications:
Training or clinical experience in geriatrics, primary care/internal medicine and/or ED or urgent care
Minimum of 3-5 years of clinical experience in acute care, emergency, or primary care settings.
Be open to professional development through training, obtaining certifications if necessary, and attending team meetings
Experience working in a collaborative healthcare setting to drive positive outcomes and achieve goals
Training or experience in outpatient primary care setting

This person will follow the directives of the Behavioral Health Clinical Manager in all areas of behavioral health management, implementation, and productivity, providing therapy remotely using approved Electronic Health Record (EHR) telehealth software to care for patients.
This position is Remote and requires high-speed internet and office space where services can be conducted in a confidential and therapeutically appropriate setting.
Responsibilities include, but are not limited to: providing individual counseling sessions, reviewing productivity, assisting staff/psychiatrists in meeting productivity expectations, reviewing paperwork, promoting behavioral health services to the community, and working with human service providers. This position will have access to privileged information, and confidentiality must be maintained at all times.
ESSENTIAL DUTIES AND RESPONSIBILITIES
Provide remote individual counseling sessions using the approved EHR telehealth software platform.
Conduct screenings, assessments, create individualized treatment plans, monitor patient progress, maintain detailed records, collaborate with other professionals, and ensure strict client confidentiality.
Deliver services that meet HRSA and Joint Commission quality metrics and standards.
Work with assigned staff to ensure all clients are checked in as part of the billing process.
Review weekly productivity reports to determine performance gaps and make improvement plans, as needed.
Provide the supervisor with written and verbal reports on identified performance gaps on a regular basis.
Assist in marketing behavioral health services to the community.
Must be willing to work some nights, and one weekend a month, or when special events and/or presentations are scheduled.

Position Overview:
Under the direction of the Behavioral Health Clinical Manager, the Remote Behavioral Health Therapist provides individual counseling sessions via telehealth using the approved Electronic Health Record (EHR) platform. This role supports quality behavioral health care through therapy, training, chart reviews, utilization reviews, and productivity monitoring, while ensuring confidentiality and compliance with HRSA and Joint Commission standards.

Key Responsibilities:
Clinical Duties:
Provide individual therapy sessions using approved EHR telehealth software.

This position supports primary care and behavioral health providers by delivering comprehensive psychiatric care for adults, children, and adolescents. Key Responsibilities

Psychiatrist - Tele-Medicine (REMOTE)
Compensation: $250,000 - $280,000 (employed) OR $170/hour (independent contractor) Schedule: Full-time; flexible scheduling with productivity goals

Position Summary
We are seeking a Board-Certified (or Board-Eligible) Psychiatrist to provide high-quality tele-psychiatric services. This position supports primary care and behavioral health providers by delivering comprehensive psychiatric care for adults, children, and adolescents. The Psychiatrist plays a key role in assessment, diagnosis, treatment, and coordination of care to improve outcomes for patients with behavioral health conditions.
Key Responsibilities
Provide continuous, comprehensive psychiatric treatment for adults, children, and adolescents via tele-health in accordance with evidence-based guidelines.
Review and document patient histories; assess behavioral health conditions for individuals and families.
Administer and interpret validated screening tools (PHQ-2, PHQ-9, GAD-7, Vanderbilt, etc.) to establish care baselines and monitor progress.
Collaborate with care teams (care managers, medical providers, therapists) to adjust treatment plans for patients not progressing.
Order and interpret diagnostic tests as clinically appropriate.
Diagnose, prescribe medications, and refer patients to other treatment options including therapy or hospitalization when needed.
Educate patients and families on behavioral health conditions, treatments, and available care team supports.
Prepare accurate medical records, clinical reports, and correspondence.
Support quality improvement initiatives and participate in staff meetings and trainings.
Meet or exceed productivity goals (2.7+ encounters per day/week).
Complete all required Core Curriculum training and continuing education.
Perform other duties as assigned.
Qualifications
Doctorate of Medicine (MD/DO) from an accredited institution.
Completion of an accredited Psychiatry residency program.
Board Certification (or eligibility) in Psychiatry with ongoing maintenance of certification.
Active, unrestricted Illinois medical license and DEA registration.
Current CPR certification.
Demonstrated experience in assessment, diagnosis, and treatment of diverse mental health conditions.
Fluency in Spanish preferred.
Other Requirements
Current driver's license and auto insurance.
Ability to maintain compliance with CME and professional development requirements.

Advocate for your clients by monitoring for potential issues and communicating concerns
Share your knowledge and experience through direct supervision, support, training, and orientation of our growing team of behavioral technicians
Physically keep up with clients as they move about the session area(s). This will require many physical activities including, but not limited to, standing, sitting, bending, lifting 25+ pounds, running, squatting, kneeling, and generally keeping pace with children from 2-15 years old
Qualifications

Master's degree in behavioral analysis, education, psychology, or a related field
Current BCBA license (depending on state) or sitting for your exam in the next 3 months- students are welcome!
Demonstrated skills implementing applied behavioral analysis (ABA) with children with Autism Spectrum Disorder or related disorders
Ability to administer and interpret a variety of clinical assessments
Excellent clinical competence and judgment
Commitment to handling confidential information responsibly
Exceptional communication, problem-solving, organizational and project management skills while working in a fast-paced environment
Compensation Range: $90,000-$105,000 full-time or $90-$100 per hour for part-time
