As an Early Intervention Social Worker (LSW), you will empower children and families in their natural environments (primarily home settings), focus on developmental growth, mental health, trauma-informed care, and social-emotional well being.

Location: Columbus
1st Shift: Monday to Friday 8am to 4:30pm
What You'll Do:
Deliver family-centered early intervention services in alignment with each child's Individualized Family Service Plan (IFSP).
Conduct functional assessments in home settings and help develop meaningful outcomes based on family concerns and priorities.
Provide counseling, trauma-informed care, and therapeutic support to children and their families.
Actively participate in IFSP development, reviews, and annual updates.
Collaborate with transdisciplinary team members using the principles of primary service provider (PSP) and coaching models.
Advocate for families and help them access needed supports and services.
Maintain accurate case documentation, data entry, and compliance with regulations.
Qualifications:
Bachelor's degree in Social Work
Ohio Licensed Social Worker (LSW)
Ohio Professional Registry (OPR) Developmental Specialist Credential, or able to obtain within first 24-months of employment.
10-30 hours of coursework/seminars in child development, family-centered practices, early intervention strategies, IDEA Part C regulation
Proficient in Microsoft Office
Ohio Driver's License
Preferred qualifications include: Early Intervention Certificate, two or more years' experience working with children birth-5 with developmental delays or disabilities, one (1) year experience working in I/DD or mental/healthcare setting.

JOB DUTIES (SUMMARY):
Responds in the field in accordance with the priority of the report and mandated time frames. Conducts face to face interviews with all children in the household of assessment, parent/caregiver(s) and alleged maltreater(s) to make a decision regarding child safety.
Assessment shall include pertinent information collection from all parties regarding alleged maltreatment, surrounding circumstances, adult and child functioning, parenting practices, and discipline to ensure an accurate safety determination. Visits are conducted in homes, schools, hospitals, and other community settings as required
Completes intake assessment of reports alleging child maltreatment by telephone and/or face-to-face contact. Collects sufficient information regarding the alleged maltreatment, surrounding circumstances, adult and child functioning, parenting practices and discipline. Utilizes interpersonal skills to professionally and respectfully engage each individual in sharing behaviorally-specific and detailed information. Responsible for information collection through hospitals, law enforcement, etc. to complete an assessment of child safety. Completes intake assessment on reports of elder abuse and/or neglect by telephone and face to face contact.
Researches previous agency history, criminal history, court involvement and other applicable information providing a comprehensive summary for all individuals associated with report. Maintain written documentation of assessment activities.
Clinically evaluates information obtained creating a screening recommendation by identifying the presence or absence of Present or Impending Danger threats or alleged maltreatment and/or a need for agency involvement. Review presence or absence of safety threat(s) in consultation with Intake Supervisor. Implement necessary safety intervention when applicable.
Prepares Request for Legal Action and provides court testimony.
Completes appropriate jurisdictional referrals to other Public Child Welfare agencies or law enforcement agencies as required. Participates in required Unit Meetings and Agency Trainings as required.
Performs other related duties as required. Attends required training and unit meetings.

PURPOSE OF JOB CLASS (NATURE OF WORK)
In a state agency or facility this class is accountable for the provision of intensive supportive social case work services to an assigned caseload of individuals and families.

NOTES:
* Closely related fields are: applied sociology, child development, child welfare, clinical psychology, counseling, human development and family studies, human service, marriage and family therapy, nursing, social and/or human services, education and criminal justice
* Qualifying experience at this level must include the use of professional interviewing techniques, provision of skilled counseling to an assigned client caseload and assessment of basic client needs (nutritional, environmental, financial, medical, protective service) through continuing personal observation during visits, intervention and evaluation.
MINIMUM QUALIFICATIONS - SUBSTITUTIONS ALLOWED
For state employees successful completion of the Social Worker Trainee program may be substituted for the General Experience.
SPECIAL REQUIREMENTS

3. First Stage of Interview process:
* Selected Candidates will also meet with DCF Academy for Workforce Development to learn more about the role and responsibilities of a Social Worker/Trainee and will receive a writing assessment.
* Selected candidates will have a panel interview with DCF Regional staff.
4. Second Stage of Interview process:
* Recommended candidates are contacted by the Department Children and Families Human Resources team members for additional documents. Candidates will be asked to supply:
* Official transcripts for degrees listed on application
* 3 Written Supervisory References including current supervisor (s) and former supervisor (s).
5. Background Investigation:
Recommended candidates will go through a comprehensive review of employment, education, criminal, motor vehicle history and a physical examination (including drug screening).
6. Appointment:
Selected candidates are scheduled for their first day of work!
PURPOSE OF JOB CLASS (NATURE OF WORK)
In a state agency or institution this class is accountable for receiving intensive training in the provision of social case work services to individuals and families.
EXAMPLES OF DUTIES
* Receives training in agency social work procedures, personnel policies, program guidelines and legal reporting requirements;
* Receives training in and is given progressively complex assignments involving utilization of interviewing techniques required to assess client needs, elicit information and establish community referral sources;
* Provides basic counseling services to a limited number of clients enrolled in departmental programs, inmates or patients in residence or families of clients receiving services;
* Prepares initial treatment plans, psychosocial evaluation, case histories and other case records as components of a small assigned caseload;
* Refers clients, patients and family members for community or social services, medical or psychological services or housing, financial and employment assistance;
* Motivates clients to increase ability to understand and cope with problems, frustration and anxiety;
* Performs a greater number of specific and complex social work duties and maintains a larger caseload as skills are acquired during training program;
* Performs related duties as required.

SPECIAL REQUIREMENTS

Assesses and ensures child safety at every contact, which frequently includes direct consultation with supervisor regarding safety decision making; Assessments shall include pertinent information collection from all parties regarding alleged maltreatment, surrounding circumstances, adult and child functioning, parenting practice, and discipline to ensure an accurate safety determination. Understands and determines parent/caregiver protective capacities. Conducts interviews in homes, schools, hospitals, and other community settings as required. When appropriate, seeks medical evaluation of children and completes referrals to law enforcement.
Controls for child safety immediately when a child is determined unsafe by confirmation of Present or Impending Danger Safety Threats; Completes Safety Plan Analysis questions to determine least restrictive safety plan that is able to control for specific family circumstance and conditions; Confirms Safe Environment for all out of home safety plans and placements; Works with specialized staff to access appropriate placements and prepare children and families for placement; discusses and partners with family to develop case plan specific to increasing their protective capacities and decreasing active safety threats in home; completes necessary referrals; Maintains regular contact with supportive service providers to ensure coordination and effectiveness of services; ensures frequent and ongoing contact for parent/child parenting time; visits regularly with parent/caregivers and child(ren), including assessment of safety at every visit, per agency policies and required state standards; continues ongoing monitoring of case progression until the case is transferred or closed; completes necessary paperwork and SACWIS functions to transfer or close case as appropriate and deemed necessary for case progression.
Completes necessary paperwork and documentation on a timely basis for case activities, which shall include but is not limited to: case reviews, safety assessments, case plans, placements, court reports, court complaints, SACWIS activity notes and placement changes. Responsible for demonstrating proficiency in mobile technology to aid in maintaining written documentation of assessment activities within required time frames including but not limited to: Safety and Family Assessments, Activity Logs, and Present Danger and Impending Danger Safety Plans. Responsible for other mandated paperwork as outlined by state regulations and agency policy. The goal of the MHICM program is to provide coordinated, integrated, rehabilitative, and restorative clinical care to eligible Veterans, helping them maintain the highest level of functioning and independence possible in the community. Social Worker - Integrated Care (GS-11) (Springfield, OH): The incumbent is assigned as Social Worker (SW) in Integrated Care (IC) who works at the interface between primary care (PC) and/or specialty medicine (SM) in an integrated care model. The Patient Aligned Care Team is a comprehensive team which delivers primary care to Veteran patients in a longitudinal rather than episodic fashion and which has as its focus: prevention, health promotion, coordination and chronic disease management. The primary duty assignment will be Primary Care, Patient Aligned Care Team; however, this assignment is mission dependent and may include assignments to inpatient medical/surgical floors, Emergency Department, or other social work coverages.
The Cincinnati VA Medical Center (CVAMC) is a two-division campus located in Cincinnati, OH and Fort Thomas, KY serving 15 counties in Southwest OH, Northern Kentucky, and Southeast Indiana with 6 Community Based Outpatient Clinics located in Bellevue, KY; Florence, KY; Lawrenceburg, IN; Hamilton, OH; Clermont County, OH, and Georgetown, OH. The CVAMC also supports two off main campus locations in the Cincinnati area via an Eye Center and Mental Health Outreach Community Division.
Duties:
(1) Establishes and maintains effective therapeutic relationships with Veterans in the Primary Care Clinic and/or their families.
(2) Works independently with Veterans and their families who are experiencing a wide range of complicated medical, psychiatric, emotional, behavioral, and psychosocial problems.
(3) Completes thorough psychosocial assessments to determine the psychosocial functioning and needs of Veterans and/or their families. The incumbent must be able to utilize this assessment in advocating on behalf of the Veteran for attainment of the highest level of independence that is possible and practicable.
(4) Participates as a member of the interdisciplinary treatment team and actively participate through collaboration with Veterans and family as well as interdisciplinary treatment team members in the development and implementation of treatment goals and interventions.
(5) Possess knowledge and experience in use of medical and mental health diagnoses, disabilities, and treatment procedures, including acute, chronic and traumatic illnesses, common medications and their effects/side effects, and medical terminology.
(6) Facilitates discharge planning through collaboration with Veterans and their families as well as interdisciplinary treatment team members to ensure that appropriate discharge plans are executed in a timely manner.
(7) Serves as a liaison between Veterans and/or their families and VA and community resources in order to ensure thorough delivery of services. This involves coordination of Veteran and family involvement in the development of treatment plans, care conferences, discharge plans, as well as facilitating communication as needed and practicable.
(8) Possess knowledge and ability to independently implement treatment modalities, provide counseling and/or supportive groups for Veterans and families.
(9) Provides consultation and education to Veterans and their families regarding community resources, VA benefits and specialty programs, and advance directives. This includes the incumbent's knowledge of the process for accessing and/or coordinating community-based services, including information and referral for additional services from other VA programs, other government programs, and community programs.
(10) Provides consultation to other treatment team and staff members regarding psychosocial needs of Veterans and/or their families and the impact of the identified psychosocial problems on the Veteran's health care planning, compliance with treatment, and discharge planning.
(11) Serves as a liaison between Veterans and/or their families and VA and community resources in order to ensure thorough delivery of services.
