What tax form is received when you start a job?

• State withholding forms. Depending on the state you live in, you may need to fill out a state withholding form in addition to the federal W-4 form. This form will be used to determine how much state income tax should be withheld from your paycheck.

• I-9 form. This form is used to verify your identity and eligibility to work in the United States. Your employer is required by law to verify your identity and work eligibility within the first three days of employment.

• Direct deposit form. This form is used to set up direct deposit of your paycheck into your bank account. It will ask for your bank account and routing numbers.

• Employee information form. This form is used to collect basic information about you such as your name, address, phone number, emergency contact information, and employment history.

• Benefits enrollment forms. If your employer offers benefits such as health insurance, retirement plans, or other benefits, you may be asked to fill out enrollment forms to sign up for these benefits.

If you are asked to complete an I-9 form when starting a job, then you will most likely have to bring two forms of government ID such as your passport, driver's license, voter registration card, or social security card. By requiring two forms of ID, the employer can be sure that you are who you say you are and that you are legally authorized to work in the United States.
