Housekeeping
Perform general housekeeping duties in resident suites, public areas, and support areas as assigned while meeting or exceeding Sunrise's quality standards.
Perform cleaning duties according to schedule in assigned areas such as resident suites, bathrooms, common areas and offices:
Sweep, dust, dust mop, and wet mop all flooring; spot clean and vacuum floors, rugs, carpets, and runners
Clean, sanitize, and polish lavatory, shower, and sink fixtures
Dust, wash, sponge mop, sanitize and hand shampoo furniture, dust desk and floor lamps, and clean and polish glass surfaces, woodwork, walls, and windowsills
Knock on resident's doors before entering and respect their privacy during the cleaning process.
Knock on public restroom doors before entering and wait for a response before beginning the cleaning process.
Maintain equipment and supplies needed to perform work in a clean and orderly condition.
Transport supplies, to include clean and soiled linen, to the appropriate storage and collection areas.
Collect trash and remove and for maintaining trash receptacles and collection areas in a clean and sanitary manner.
Laundry
Collect, clean, and redistribute the community laundry.
Sort, wash, and dry linens properly following approved laundry procedures.
Fold, count, and stack linens following approved laundry procedures.
Inspect linens routinely for wear and tear, identify worn and torn linens, and follow the community's Standard Operating Procedure for damaged linens.
Maintain the commercial laundry in a clean, orderly, and sanitary condition.
Resident Care
Assist Care Managers and Department Coordinators with resident care when requested.
Practice positive resident relations following our Sunrise Shared Values; respond to resident requests and direct resident feedback and requests to immediate supervisor and or Care Manager.
Review, read, notate, and initial Daily Log to document and learn about pertinent information and any resident's physical and behavioral changes.
Safety and Risk Management
Ensure cleaning chemicals are kept stored and locked when not in use.
Ensure any cords, carts, equipment, and other hazards are always kept out of the way, not blocking exits and in compliance with fire codes.
Partner with community team to ensure community is in compliance with national/provincial regulations pertaining to occupational health and safety requirements, promoting Risk Management programs and policies, and adhering to safety rules and regulations.
Practice safety procedures at all times including Personal Protective Equipment (PPE), fire extinguishers, Safety Data Sheets (SDS), and Lockout Tagout procedures.
Maintain equipment and supplies to avoid waste and damage to areas and equipment and to prevent accidents.
Comply with all infection control techniques, placement of bio-hazard containers and removal techniques, procedures, and policies.
Training and Contributing to Team Success
Participate as a member of a team and commit to working toward team goals.
Demonstrate in daily interactions with others, our Team Member Credo.
Commit to serving our residents and guests through our Principles of Service.
Contribute to the overall engagement programs and processes (customer and team member engagement) including participating in the team member engagement survey and engagement improvement planning workshops.
Attend regular meetings; Town Hall, Department Team, Cross Over, and others as directed by the Supervisor/Department Coordinator.
Maintain compliance in assigned required training and all training required by state/province or other regulating authorities as applicable to this role to ensure that Sunrise standards are always met.

PRIMARY JOB DUTIES AND RESPONSIBILITIES
Responsible for planning and conducting in-person visits, predominantly focused on key stakeholders at proceduralist physician offices to increase selection of centers to perform cases at. This role will receive ongoing guidance from Business Development leadership on providers of focus and productivity expectations.
Identify trends in assigned providers' utilization of the USPI center and facilitate discussions with the provider / practice to understand underlying dynamics, decision making behind facility selection, feedback etc.
Coordinate with Operations team and provider / practice to optimize use of existing block times and increase the provider's access to procedural time at the center
Conduct face-to-face sales meetings with clients ensuring through understanding of the center's attributes, specialty capabilities, processes as well as patient experience & safety outcomes
Complete follow-up meetings with physicians, practice managers etc. to ensure thorough understanding of the physicians' desires, needs and obstacles to growth to increase potential case volumes at USPI centers
Communicate feedback from clients and partner with the appropriate facility resources to facilitate credentialing and onboarding of new physicians as well as resolve issues such that providers practice more at USPI centers
Prepare and present sales reports, measuring case volume growth, identifying trends, lessons learned, opportunities and areas for improvement to achieve facility and / or market goals.
Continuously modify and execute business development tactics to ensure optimal business outcomes, based on feedback from providers and facility leaders.
Maintain latest knowledge of the market hospital, ambulatory surgery and provider landscape, in your defined market service area.
Document all client engagement in a timely manner on a daily basis in the defined CRM tool, including outcomes and required follow-up.
Support the implementation of service line expansion, extended hours, case cancellation recapture and other relevant initiatives, as directed by the Business Development and Operations Leadership.
Perform all duties with consistently high ethical standards and strict adherence to company policies and procedures.
EDUCATION, EXPERIENCE, AND OTHER REQUIREMENTS
Minimum Education
High School diploma/GED required. Associate's degree preferred. Bachelor's degree strongly preferred.

General Role Description: The Home Health Patient Care Manager and RN Hybrid is responsible for the supervision and coordination of clinical services and provides and directs provisions of nursing care to patients in their homes as prescribed by the physician and in compliance with applicable laws, regulations, and agency policies. Coordinates and supervises an interdisciplinary team of staff to assure the continuity of high quality care to home health patients assigned to the team's area in accordance with physician prescribed plan of care, and all applicable state and federal laws and regulations.
Primary Responsibilities:
* Receives referrals and ensures appropriate clinician and/or therapist(s) assignments for timely patient evaluation by signing off after authorization and plotting start of care (SOC) visits.
* Coordinates determination of patient home health benefits, medical necessity, and ongoing insurance approvals.
* Ensures patient needs are continually assessed and care rendered is individualized to patient needs, appropriate and reasonable, meets home health eligibility criteria, and is in accordance to physician orders.
* Manages and documents phone calls and new orders from physicians, clinicians, patients, referral sources, and communicates patient updates/new orders to clinicians.
* Uses coordination notes to document, as needed and appropriate.
* Receives report from weekend and after-hours clinicians admitting new patients.
* Coordinates all aspects of care with all disciplines, physicians, durable medical equipment providers, caregivers/family members, transferring facilities, and any other applicable healthcare providers.
* Follows-up on lab and other clinical diagnostic test, physician contact, and significant changes in the patient condition to ensure adequate physician notification, follow-up, and needed plan of care modifications and communicates such clinicians.
* Schedules, prepares for, facilitates, and documents case conference/SOC reports and facilitates effective exchange of information across disciplines especially with adverse findings, changes in patient condition, daily and urgent updates, as necessary.
* Assists clinicians in coordinating the transfer and discharge of patients from agency services as indicated by the physician.
* Receives report from field clinicians prior to scheduled days off on patient status and ongoing needs.
* Follows-up with On-Call events daily
* Assures payer change documentation is completed properly and timely, and required. Reviews clinician visit notes weekly to ensure timely, complete, appropriate, and accurate submission of all documentation by field staff.
* Takes necessary action to correct adverse findings and communicates trending to clinical director.
* Reviews, evaluates, and supervises service delivery to ensure appropriateness of care and utilization of services, equipment, and supplies through activities such as random patient visits, medical record reviews and case conferences.
* Enters infections and incidents/occurrences into the online Risk Management Incident Reporting System, as specified by policy.
* Assists in the orientation of new agency personnel and provides direction and leadership to clinical team members in collaboration with the clinical director.
* Provides high quality clinical services within the scope of practice and within infection control standards, in accordance with the plan of care, and in coordination with other members of the health care team.
* Consistently meets expected productivity at 50% of full time RN level as defined in the Visit Productivity Point Policy.
* Accurately and timely completes the comprehensive assessments (OASIS) including medication reconciliation. Makes the initial and/or comprehensive nursing evaluation visit, ensures patients meet home health eligibility and medical necessity guidelines as defined by payer source, accurately determines primary focus of care, develops the plan of care within State specific guidelines with the physician, and submits accurate documentation.
* Directly and/or indirectly supervises care provided by the home health aides and licensed practical vocational nurses, provides instruction as appropriate, and assigns tasks according to State and federal regulations.
* Provides required supervisory visits.
* Initiates, develops, implements, and makes necessary revisions to the plan of care in collaboration with the physician and other health care professionals involved in care.
* Communicates relevant information timely and effectively with appropriate agency staff including but not limited to: any patient care issues or needs, visit assignments, dates of scheduled visits, and schedule changes to scheduler, orders and OASIS data sets, coding requests, schedule home visits, to coordinate care with other clinicians.
* Communicates timely and effectively with physicians, patients, and family members to ensure quality care and service excellence.
Required Qualifications:
* Current RN licensure in state of New Mexico
* Current CPR certification required
* Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation

Responsibilities
