Completes and utilizes the Level of Care Utilization System (LOCUS) criteria assessment to aid in the identification of the appropriate level of care for the clients. 4. Encourages clients to express their feelings and discuss what is happening in their lives and help them to develop insight into themselves and their relationships. Guides clients in the development of skills and strategies for dealing with their problems. 5. Completes an individualized treatment plan review within 30 calendar days of the initial treatment plan in partnership with client. 6. The Activities Coordinator is responsible for coordinating the creation and implementation of individual and group activities to stimulate the mind, body, and spirit of our assisted living and memory care residents. Consistently maintain a joyful and energetic attitude/demeanor along with being organized and highly motivated to assist residents with life enrichment activities. Ensure all activities are conducted in a manner and in an environment that ensures residents safety and security at all times. Coordinate with outside individuals/groups/organizations when they visit Fellowship Home to ensure the needs of Fellowship Home and our residents are being met, including performance groups, church groups, devotions, gifts from individuals, etc. Assist in training volunteers as to resident care standards as needed with such events/visitations.

Required to work 20 shifts within a 6-week schedule cycle. Weekends and holidays are required. Weekend Schedule Minimum of 2 weekends within 6-week schedule cycle.
