Here are a few of the daily responsibilities of our Health Center C.N.A.:
* Participate in report and make rounds throughout shift.
* Provide assistance with activities of daily living, including toileting, bathing, shaving, and dressing, brushing hair, brushing teeth and dentures, placing hearing aids, placing dentures, and placing glasses.
* Respond promptly to all call lights and requests for assistance from residents.
* Assist with meal services, including distribution of trays, set up of tray, assistance with eating, responding to special requests or requests for additional items, and distribution of snack cart items.
* Properly document all ADL flow records, bowel, and bladder function, repositioning sheets, sign-out sheets, meal intake, intake, and output sheets.
* Obtain resident vitals and weight upon admission as well as provide a general introduction and orientation to new members.
* Maintain resident rooms in a manner that is clean and orderly, including making the bed, marking and keeping personal toiletries in the medicine cabinet on the appropriate shelf, maintaining a clear floor, placing clothing in the appropriate wardrobe, maintaining a clean overbed table and removing all soiled linens from the room.
* Provide range of motion for all assigned residents per guidelines and maintain documentation.
* Assist with activities as it relates to the cognitive ability and enjoyment of a dementia resident.
* Engage the resident in reminiscence activities.
Here are a few of the qualifications we need you to have:
* Valid NC CNA license and completion of training course.
* First Aid/CPR certified.
* Minimum two years of experience.
* Experience in geriatrics, and with Dementia and Alzheimer's preferred.
* Ability to observe changes in a resident's condition and to act as required for the resident's total care.
* Demonstrate understanding, patience, tact, respect, and courtesy in dealing with residents, family, and visitors.
* Proficient skills and knowledge of CNA duties and responsibilities.
* Ability to read, write, speak, and comprehend written and oral instructions in English.
Pay Range $17.50 - $18.30 per hour depending on experience, plus potential shift differentials depending on shift.

EDUCATION AND EXPERIENCE
Requires a high school diploma or equivalency, successful completion of a Board of Nursing approved training and competency evaluation program, current listing with the Division of Facility Services Registry, and one year of patient care and record keeping experience in an acute care or custodial setting. REQUIRED KNOWLEDGE, SKILLS, AND ABILITIES
Must be able to independently organize work and provide effective health care services to hospice patients through appropriate patient care, and document said care thoroughly, accurately, and completely on a timely basis. Minimum of twelve (12) hours continuing education required annually. CPR certification required. Must have a valid drivers' license. Accept travel based on agency need as well as weekend/evening and holiday call assignments.

PHYSICAL REQUIREMENTS
Must be able to stoop or kneel, bend, lift, push up to 50 pounds, reach with hands and or arms, and smell frequently when providing patient care and performing office related duties. DETAILED RESPONSIBILITIES
Provide direct patient care and coordination of hospice/homecare services.
Provide/assist patients with personal care needs, to include bath, transfer and ambulation, and care of the mouth, skin, and hair as assigned by the hospice/home health RN. Reinforce principles of care to patient/family.
Provide patient care treatments as assigned by the hospice/home health - RN, as appropriate under the Nurse Practice act and as ordered by the physician. Tasks may include: changing non-sterile dressings, monitoring fluid intake and output, oral suctioning, and basic catheter care (refer to Home Care Assistant Skills Inventory).
Perform incidental household services within the patient's immediate environment that are necessary to prevent or postpone institutionalization, including laundering necessary to the comfort and cleanliness of the patient; ensuring nutritional needs of the patient are met; washing utensils used in the course of the visit; and light housekeeping. (May include changing bed linens, light cleaning and rearrangement of equipment and supplies to accommodate patient needs.)
Responsible for making visits on assigned patients on weekdays/ weekends/ holidays.
Follow the aide assignment sheet appropriate to the patient's needs and condition as developed and assigned by the hospice/home health nurse-RN.
Coordinate services with the Clinical Manager/Registered Nurse and other agency personnel to optimize patient care, as well as, with the patient, family, and community agencies.
Update the hospice/home health nurse on the patient's condition and/or patient/family situations affecting care and report changes in the patient's condition immediately.
Plan the weekly patient schedule based on the visit frequency ordered and in the best interest of the patient. Notify patients of any schedule changes.
Attend and participate in IDT meetings, and quality review as scheduled by the agency.
Provide appropriate documentation as required.
Complete documentation of patient care data and company required records on a timely basis.
Document coordination of team services as necessary.

Document any instance where prescribed medication is not administered, including reason(s) for refusal. Promptly notify the Charge Nurse, Nurse Supervisor, or Director of Nursing Services of any errors or reactions to medications by residents. Verify the identity of the resident before administering the medication treatment.
Accurately measure, record, and report the vital signs of residents. Follow the facility's procedures allowing residents to self administer his or her medications. Be familiar with the meaning and usage of common medical abbreviations, symbols, and terms relative to the administration of medications as used by the facility, pharmacists, and physicians.
Attend and participate in continuing education programs designed to keep you abreast of changes in your profession, as well as to maintain your certification on a current status Agree not to disclose resident's protected health information and promptly report suspected or known violations of such disclosure to the Administrator. The Care Giver/Med Tech role includes providing hands-on care, physical and emotional support to each resident while maintaining a safe and comfortable home-like environment.
Responsibilities:
Care Giver:
Responsible for a designated group of residents during the shift; knows where their residents are and physically checks on them throughout the shift.
Observes, reports and documents symptoms and conditions of residents for changes in condition such as skin, behavior, alertness, weight, dietary and participation in activities.
Responds to security system and resident call bells promptly.
Notifies supervisor and/or Health Care Coordinator if a resident has increased care needs.
Assists with continence management and disposes of all continence products properly to ensure sanitation of resident suite and community restrooms.
Serves and collects food trays and assists as needed; reports refusal of meals or loss of appetite.
Maintains cleanliness of resident's room and work areas.
Practices good standard care precautions of cleanliness, hygiene and health.
Helps residents maintain independence, promotes dignity and physical safety of each resident.
Actively participates/leads and assists residents with activities of daily living (i.e. bathing, dressing, toileting, grooming, ambulation, transferring, eating) as instructed.
Engages residents in life skills and other life enrichment activities.
Greets and assists all internal and external customers, guests, family members, residents, vendors and team members.
Does resident laundry as assigned and needed.
Medication Tech duties:
Reviews service plan to learn pertinent information about residents.
Assists/observes medications and treatments for each resident using the medication observation record and the Six Rights of Medication Pass.
Documents and initials form as medications are given including appropriate documentation for refusal or missed doses.
Maintains confidentiality of all resident information including resident medication.
Reports all resident concerns made while assisting resident with the medication to the Nurse and/or Health Care Coordinator (HCC).
Restocks medication cart after all medication passes.
Assists in checking medication regardless of packaging system.
Counts all narcotics with another Medication Care Manager or Nurse each shift.
Maintains and cleans the Medication Room, med charts, treatment carts for neatness cleanliness, availability of medications and expired medications.
Follows re-fill process for medications.
Participates in the development of the Service Plan and monthly updates.
Takes and records temperature, blood pressure, weight, pulse and respiration rates.
Reports occupational exposures to blood, body fluids, infectious materials, sharp sticks and hazardous chemicals immediately.
Other duties as assigned by the Supervisor.
Qualifications:
CertifiedNurse'sAidcertification preferred.
High School diploma/GED
Must be 18 years of age.
CPR Certification preferred
First AidCertification preferred
Previous experience working with seniors preferred.
