Location: Based in Melbourne, FL Your Role as a Home Health Aide (HHA/CNA): Provide personal care, bathing, and hands-on care as outlined in the patient's plan of care. Complete visit records promptly and accurately according to Agency policy. Report any changes in the patient's condition to the supervising RN. Perform essential household tasks to support health care in the home as assigned. Assist patients with ambulation and exercises as directed by the care plan. Qualifications: Successfully completed a Home Health Aide training program and competency evaluation as required by Medicare Conditions of Participation. Demonstrates a compassionate and caring attitude toward the sick and elderly. Ability to follow directions, read, and write effectively to carry out assigned tasks.
