Essential Functions
* Provides basic patient care within scope of practice, including vital sign monitoring, hygiene assistance, and patient comfort measures, ensuring timely and accurate completion of tasks.
* Communicates effectively with the care team, including handoffs, reporting changes in patient conditions, and participating in bedside shift reports.
* Develops clinical skills by asking questions, seeking feedback, and applying evidence-based practices under the guidance of an RN.

* Assists with basic equipment maintenance, such as glucometers, bladder scanners, and scales, and ensures timely removal, cleaning, and proper storage of patient care equipment after discharge.
* Performs hourly rounding and provides purposeful patient interactions to foster therapeutic relationships and enhance the healing process.
* Documents all tasks and observations clearly, accurately, and in a timely manner, adhering to facility policies and standards.
* Upholds excellent customer service relations with patients, families, and internal/external departments.

Qualifications
* Currently enrolled in an accredited Nursing program required and
* Completion of fundamentals of clinicals in an accredited Nursing program required
* 0-1 years of experience in a healthcare setting required

ESSENTIAL DUTIES AND RESPONSIBILITIES

1. Record Review:
* Completes initial medical records reviews of patient records within 24-48 hours of admission for a specified patient population to: (a) evaluate documentation to assign the principal diagnosis, pertinent secondary diagnoses, and procedures for accurate MS-DRG assignment, risk of mortality and severity of illness; and (b) record in business partner designated CDI tool and/or host medical record system.
* Conducts follow-up reviews of patients every 24-48 hours or as needed up through discharge to support assigned working MS-DRG assignment upon patient discharge, as necessary.
* Formulate physician queries regarding missing, unclear, or conflicting health record documentation by requesting and obtaining additional documentation within the health record, as necessary.
* Collaborates with providers, case managers, nursing staff and other ancillary staff regarding documentation and to resolve physician queries prior to discharge.
2.CDI
* Communicates/Completes Clinical Documentation Integrity (CDI) activities and coding issues (lacking documentation, physician queries, etc.) for appropriate follow-up, provider education and DRG Miss-Match reconciliation.
* Assists with Provider education, rounding and communication regarding open queries for resolution.
3. Professional Development: Stays current with AHA Official Coding and Reporting Guidelines, CMS and other agency directives for ICD10-CM and PCS coding. Attends CDI Boot camp, CDI/coding trainings annually and quarterly for inpatient coding. Attends monthly education lecture series (MELS) and all CDI/coding assigned learn share modules as well as any additional required CDI education.
4. Assist in training department staff new to CDI

Position Summary:
Provides direct patient care to patients using the nursing process in accordance with applicable scope and standards of practice.
Performs plan of care intervention, including medication administration, specimen collection, clinical treatments, as well as other medical care treatment.
Documents patient care given.
Monitors, records, and communicate patient condition as appropriate to care team, physician, patient, and family.
Assists patients with performing activities of daily living, including personal hygiene, elimination, nutrition, and ambulation.
Collaborates as needed across disciplines to coordinate patient care, including patient transfer, discharge, referral and spiritual/psychosocial support needs.

Position Purpose: Performs a clinical retrospective review and assessment of complex services previously provided to determine if the level of care and services provided were clinically appropriate. Performs a clinical review and assessment of relevant complex post-care services by reviewing medical records against guidelines and clinical research criteria to determine if the services administered were clinically appropriate and within quality standards at the most efficient and effective level
Reviews medical records for medical necessity of services to identify quality of care issues and, if identified, refer to the Medical Director for review and verification
Applies clinical knowledge to ensure compliance with medical necessity guidelines, medical policy, and accepted standards of care

We are seeking candidates with strong experience in payment integrity and utilization review.
Education/Experience: Requires Graduate from an Accredited School of Nursing or Bachelor's degree in Nursing and 4 - 6 years of related experience.

Strong knowledge of Medicare and Medicaid regulations preferred.

Pay Range: $30.58 - $55.09 per hour
