Analyzes KPI data to identify trends and operational issues in the OR and Sterile Processing Department (SPD).
Coordinates communication and planning for priority instruments and turnaround times, informing the surgical team of any delays.
Responds promptly to surgical team calls regarding instrumentation problems or urgent needs.
Cleans and processes equipment per manufacturer's instructions and safety protocols.
Ensures instrumentation is processed and available for next-day, on-call, or weekend surgeries.

Job Summary

This position will contribute to these MGH efforts through collaboration with MassGeneral Hospital for Children (MGHfC) and the Department of General Internal Medicine (DGIM) Population Health Management teams.
MGHfC oversees a team of CommunityHealthWorkers. CHWs are trusted members of the community with the skills and experience to understand their patients' circumstances. By building trusting relationships and walking alongside their patients, CHWs help address medical and psychosocial needs in order to promote self-efficacy, help patients meet their goals, and improve health outcomes.
The CommunityHealthWorker (CHW) will be part of the pediatric CHAMPION Program's multi-disciplinary care team, which includes a physician and a dietitian. The evidence-based CHAMPION program and research study seeks to evaluate whether telehealth vs. in-person delivery of CHAMPION is equally effective at reducing child BMI and asthma control in diverse lower-income families. The CHAMPION program consists of individual and group visits guided by a set curriculum with a multi-disciplinary team (medical provider, dietitian, and communityhealthworker), and other tools to support behavioral change and reduce body mass index (BMI) and asthma, such as follow-up phone calls, educational materials, social determinants of health screening and referral and text messaging. The CHAMPION CHW will provide care to children and adolescents with overweight/obesity and asthma, through group and individual visits, both in-person and virtually, and will participate in a Virtual Learning Community and Quality Improvement calls and activities as part of the research project. The CHAMPION CHW will support this program at the MGH Chelsea HealthCare Center and MGH Revere HealthCare Center. Hours are expected to be roughly 10am-6:30pm Mondays, Tuesdays and Thursdays based on clinical schedules, and Wednesdays and Fridays, 8:30am-5pm.
The CHAMPION CHW will engage patients and their families in setting their own short-term goals and will track the benchmarks along the way toward the achievement of these goals. In addition, the Asthma -Healthy Weight Lifestyle CHAMPION CHW will work with patients to help decrease barriers to timely follow-up care and provide coaching to engage patients and families in identification and achievement of care goals.
This is not a clinical position, but the role is part of the clinical team and requires a good knowledge of (and willingness to learn) basic clinical concepts and an understanding of when a referral to a licensed clinician is appropriate.
Qualifications

Patient Engagement and Health Coaching

Systems Navigation and Care Coordination
Help address logistical barriers, scheduling challenges, childcare needs, etc., that would inhibit a patient from showing up at their appointment; help patients to develop plans to get to appointments.
Assist patients in organizing their records, making follow-up appointments, filling their prescriptions, and understanding past medical history.
Work with primary care providers to reinforce provider care plans and health education messages, the importance of follow-up care, medication adherence, routines of self-care, etc.
Provide advocacy, patient education, and support in accessing community-based and hospital-based programs.
Support timely scheduling, communication, and problem-solving across areas to ensure program excellence.
Refer to internal or external case management services within the practice when other issues are identified (i.e., food insecurity, domestic violence issues, etc.)
Collaboration and Documentation
Document each patient encounter in detail. Track benchmarks of progress in care, including short-term goal completion.
Maintain regular communication with the patient's providers (through clinical messages in EPIC, emails, phone calls, case review meetings, etc.)
Support practice staff to develop creative processes to proactively manage patients with asthma and/or obesity in a non-stigmatizing manner; help practice staff to develop patient-centered care goals.
Collaborate and communicate with interdisciplinary primary care team to identify care plan goals.
Additional Ad-hoc Responsibilities (generally on an as-needed basis):
Attend initial and continuing education training programs, including self-directed reading and in-person and online learning.
Complete an initial assessment with the patient and provider to identify the specific areas of focus for the asthma CHAMPION CHW role with particularly high-risk patients.
QUALIFICATIONS:
High school diploma or GED required.
Relevant experience in the community or a bachelor's degree preferred; preference for Psychology/Social Work/Public Health or related field.

Sales & Growth:

Territory Management:
Maintain regular outreach and visibility within the North Shore, Boston North, and Merrimack Valley regions.
Attend networking events, professional conferences, and community gatherings to expand our referral network.

Qualifications
Experience: 3-5 years in outside sales, business development, or community relations (healthcare or senior care experience strongly preferred).
Education: Bachelor's degree in business, marketing, or a related field (or equivalent professional experience).
Communication Skills: Excellent presentation, negotiation, and interpersonal skills, with the ability to instill confidence and build trust.

The Committee for Public Counsel Services (CPCS) is the state agency in Massachusetts responsible for providing an attorney when the state or federal constitution or a state statute requires the appointment of an attorney for a person who cannot afford to retain one. The agency provides representation in criminal, delinquency, youthful offender, child welfare, guardianship, mental health, sexually dangerous person, and sex offender registry cases, as well as in appeals and post-conviction and post-judgment proceedings related to those matters.

MENTAL HEALTH LITIGATION DIVISION OVERVIEW
The Mental Health Litigation Division provides individuals with attorneys for Civil Commitment proceedings in the District and Superior Courts and provides attorneys for appointment by the Probate and Family Courts to represent persons in Guardianships and Substituted Judgment proceedings, including cases involving Conservatorship, Elder Abuse, the Validation of Health Care Proxies, Nursing Home Admission and Do Not Resuscitate orders. Civil Commitments are handled by both staff and private attorneys, while most cases in the Probate Court are handled by private panel attorneys.
In addition to our staff offices in Brockton, Roxbury, Northampton, and Worcester, the MHLD has attorneys in our trial support and oversight office and our training department. All our attorneys receive training in issues relating to mental health and the statutory and constitutional rights of our clients. The Mental Health Litigation Division offers interns a variety of experiences and assignments under the supervision of highly experienced attorneys.
Mental Health Commitment Defense Units
The MHLD offices located in Brockton, Northampton, Roxbury, and Worcester seeks interns and law coop students who have completed their 2L year and are or can be 3:03 certified to assist attorneys with representation of citizens facing involuntary civil commitment to mental health facilities, and involuntary medical treatment for mental illness, including the administration of anti-psychotic medications. 1Ls are welcome to intern with the commitment defense offices to assist with client calls, reviewing medical records and legal research. Under the supervision of the Program Manager, the Health Navigator will provide counseling and supported referrals for people who use drugs (PWUD) who are at high-risk of infection or are HIV/HCV/STI positive, while delivering harm reduction services in a drop-in setting. This includes: providing prevention and screening services to clients who use drugs, syringe exchange, safer use supply distribution, overdose prevention training, emergency response and overdose response, conflict de-escalation, motivational interviewing, crisis counseling, data collection and entry, and individual and group-level interventions aimed to reduce infectious disease transmission and increase access to care and treatment. The Health Navigator will seek to engage and empower clients with a focus on their strengths, dignity, and lived wisdom.
Representative Duties:
Provide accessible and low threshold services to PWUD that operate within the harm reduction framework.
* Assist PWUDs with warm connections to support services such as primary care, MAT and behavioral health, homeless shelters, housing, and legal advocacy.
* Connect PWUDs to HCV testing and treatment through relationship building, viral hepatitis education, and motivational interviewing.
* Conduct risk assessments and risk reduction plans. Provide harm reduction counseling on safer injection and vein care strategies, wound care, overdose prevention, sexual health and HIV/HCV/STI transmission.
* Train PWUDs in overdose prevention and how to use Naloxone. Distribute Naloxone and other safer use supplies and complete all associated MDPH data collection and internal data collection.
* Collect and distribute syringes from PWIDs. This includes safely handling, storing and disposing of syringes, and general biohazard and infection control protocols
* Administer Naloxone during overdose events and support revival efforts by way of calling 911 and using a bag valve mask/ambu bag until help arrives on scene.
* Escort clients to the building's bathroom, ensuring that all precautions, protocols and safety checks are performed.
* Conduct periodic sweeps of the outside perimeter to ensure that there are no discarded materials and ensure community safety.
* Work with team to organize daily operations and programs in the drop-in center, including opening and closing procedures to help ensure facilities are clean, safe and welcoming; creating and distributing harm reduction kits; and restocking and organizing supplies in the drop-in center.
* Assist in maintaining inventory levels of harm reduction supplies and work with leadership to adjust par levels as needed throughout the year.
* Coordinate with leadership to maintain organization of back supply closet and needle room.
* Participate in restorative justice protocols and support the community advisory board.

Pay Range: The expected base pay for the position is $19-$26 per hour and may be increased based on other factors, such as language, certifications, etc. Under the supervision of the Director of Nursing, the CommunityHealth Advocate provides support and linkage to services, advocacy, education, and information in order to ensure optimal health outcomes, increase access of services, and support the patient in self-management of health goals. The Advocate also serves as a cultural bridge to the health care system aiding in identifying and supporting the patient's cultural identity.
DUTIES:
* Promotes and educate patient community around community resources, and assist in accessing programs.
* Provides and links patients with community resources and referrals.
* Completes needs assessment including Social Determinants of Health.
* Provides Case Management services including Patient Centered Treatment Plan.
* Helps patient obtain transportation to necessary medical appointments via PT-1 forms, bus vouchers, cab vouchers and other resources.
* Provides a cultural bridge between patients and the health care system.
* Works within the Patient Centered Medical Home Model with primary care providers, Behavioral Health, home care providers, and other care team members on complex client situations.
* Assists patients/family with identifying barriers and problem solve solutions and self-management skills.
* Provides direct support on immigration issues including applying for citizenship (N648) and completing the Adjustment of Status.
* Participates in various UCHC committees to address the complex needs of patients.
* Participates in work groups to pilot strategies to increase arrival rates.
* Completes outreach to patients who are newly and chronically lost to follow up.
* Works with Care Teams to assist people with new and chronic health illnesses such as Diabetes Mellitus, cardiac conditions, HIV, and others.
* Participates with planned outreach/education activities in the community.
* Identifies and help resolve potential/actual crises in collaboration with multidisciplinary team.
* Provides cross-cultural interpreter services and education for patients throughout the care continuum.
* Conducts home visits after consultation with care teams and management.
* Documents patient/ collateral contacts in the EHR and maintain statistics.
* Participates in quality improvement projects within department and interdepartmentally as needed.
* Attends in-service trainings and outside seminars, as indicated.
* Performs other related duties as required.
