Demonstrates the ability to function within an inter-disciplinary team (nurse care coordinators, social workers, behavioral health clinicians, physicians, resource specialists, clinical support staff, etc.), connecting the patient with resources as needed.
Records and monitors the participants' progress toward goals within specific timeframes. Documents assessments and key patient updates in EMR system; documents relevant day-to-day activities and patient data.
Prepares reports and documents as needed or requested.
Assists patients with organizing their records, making follow-up appointments, attending follow-up appointments, and filling their prescriptions.
Helps patients fill out applications, for example for Medical Assistance, Housing, and SNAP (Supplemental Nutrition Assistance Program).
Provides advocacy, patient education and successful warm hand offs in accessing community-based and hospital-based programs.
Assists patient in addressing and overcoming barriers with a range of concrete supports, including but not limited to: healthcare support services, behavioral health, financial assistance, child-care and caregiver support, housing, support with utility bills, food, financial entitlements, clothing, transportation, food pantries, violence prevention, social isolation and any other appropriate community resources.
Provide intensive home and community-based outreach, motivational interviewing and goal setting, resource connection and accompaniment to medical appointments as needed to help patients appropriately utilize healthcare.
CWAs may visit patients in hospital and ER settings to facilitate with transitions of care.
Establishes culturally appropriate and trusting relationships with patients and their families.
Participates in all training activities as designated by Community Wellness Manager (CWM).
Attends regularly scheduled supervision and other program assigned meetings.
Develops and maintains strong relationships with the community and community resources to ensure patient access.
NOTE: The CWA will not provide hands on care or other services noted as home health services, including but not limited to: performance assessments, provision of care, treatment, or counseling; and/or monitoring of patient's health status.

Engages in a program of field training to provide behavioral health services in a variety of placement settings. Provides behavioral health services including diagnostic evaluations and psychotherapeutic treatment services to children, adolescents, and/or adults in individual, family, couple, and group-clinical interventions under close supervision.
About Internships at Community Healthlink
1. Clinical Interns are in their second field placement and are more often placed in programs which provide individual treatment (in addition to other services).
2. These interns are usually assigned a small caseload of clients in which they are providing treatment with clients under close supervision of their clinical supervisor. This includes assessment, diagnosis, treatment planning, providing therapy, discharge and aftercare planning, and completing clinical documentation.
Hiring Range: $15.00 - $16.00

I. Major Responsibilities:
1. Provides clinical support or treatment as defined by the level of care and service needs of the population served. Specific treatment expectations are defined by licensing and accreditation standards for each level of care and internship expectations as agreed upon between the student, school, and program. This may include diagnostic evaluations and screenings, crisis/emergency assessment and intervention, treatment planning and assessment, and individual/group/couple/and family therapy for adults, children, and/or adolescents.
2. Engages in assessment planning, intervention and aftercare as defined by the level of care and clinical needs of the population served.
3. Engages in clinical supervision and case consultations.
4. Conducts standard assessment and screening tools.
5. Participates in interdisciplinary and integrated diagnostic treatment and planning conferences.
II. Position Qualifications:
License/Certification/Education:
Required:
1. Has completed required practicum hours (or first year of field training) of a Masters or Doctoral level program in Mental Health Counseling, Social Work, Marriage and Family Therapy, Clinical Psychology, or related program.
2. Satisfactory CORI background check.
3. Driving is not a requirement.
Experience/Skills:
Required:
1. Ability to perform diagnostic and risk assessments using current accepted tools and best practices to determine appropriate level of care.
2. Can develop and implement treatment plans which are behaviorally oriented, measurable, and appropriate for the identified level of care.
III. Physical Demands and Environmental Conditions:
1. Work is considered medium. May have to lift up to 10 lbs frequently and up to 50 lbs occasionally.
2. Work occurs in an indoor, patient-focused environment.
ADDENDUM FOR CCBHC
Major Responsibilities:
1. Complete CCBHC Diagnostic Evaluations with individuals and families within the required timeframes: Same Day - 24 hours, Urgent - 48 hours, Routine - 14 days.
2. Complete CCBHC Diagnostic Evaluations within contractual and agency requirements
ADDENDUM FOR Clinical Team
Major Responsibilities:
1. Coordinate and develop treatment plans consistent with Department of Developmental Services ISP or IEP regulations as required
2. Provide therapy and/or crisis assessment and planning in the community as required by the DDS contract.
ADDENDUM FOR Geriatric Team
Major Responsibilities:
1. Provides services to individuals in nursing facilities.
2. Coordinates services with staff in nursing facilities.
ADDENDUM FOR STEP
Major Responsibilities:
1. Participates as a member of the STEP team consistent with the Coordinated Specialty Care model.
2. Provides individual, family, and group therapy consistent with the CSC model and the McFarlane Multifamily Group model.
3. Participates in trainings and collaborative activities as a member of the STEP team.
