The Foster Care Family Support Worker plays a pivotal role in serving children and families in need through the Foster Care program of One More Child. To ensure that every child placed in an OMC foster home for 30 days or more has a documented exposure to the Gospel.
To provide general support for all OMC Foster Families and to assist with providing 24-hour on-call support/coverage to the families, which includes, but is not limited to, answering/returning calls related to placements and emergency support requests in a timely manner.
To coordinate placements and discharges for foster clients and to ensure at least 70% utilization of available placement beds.
To maintain updated case records, files, and database records in an efficient manner.

Key Responsibilities:
This direct care position supervises a group of 7-10 youth through activities during waking and non-waking hours. While unobtrusively providing external controls necessary to ensure safety and security, Youth Care Specialists guide and teach youth the internal controls needed to live responsibly when and where external controls are relaxed.
Supervise the activities of a group of youths, maintaining knowledge of the whereabouts of all group members. Comply with published procedures on movement, physical security, contraband, head counts, and reporting.
Provide a safe, secure environment using group counseling techniques, individual relationships, “positive discipline” techniques through effective use of the behavior modification system (I-Choose), and the use of crisis intervention and de-escalation techniques as needed.
Represent each youth's problems, needs, and accomplishments to the treatment team, or provides progress reports to the counselor representative on the treatment team.
Call and/or conduct “huddle-ups”, problem solving group meetings, and daily reviews with the assigned group of youths, providing prompts and interventions to ensure that youth are helping each other and are recognized for their efforts.
Assist classroom teachers in the delivery of subject matter and transfer of class content and study practices to the living units.
Assist and promote activity planning and completion by the assigned group of youths. Ensure that planning, doing, and evaluating are part of the daily routing for each group, and that activities or opportunities for individual recognition are created and used to treatment advantage.
Document daily shift occurrences and frequent population counts as required by local and corporate policy. Make referrals for special needs or medical evaluation, and brief oncoming shifts on events of the past shift.
Other Duties Include:
Participate in training and development activities as required.
Report any acts, incidents or conditions that reflect the possibility of inappropriate youth-to-youth or staff-to-youth relationships.
Perform other duties as assigned.
Requirements:
Education - Two years of college coursework in social or human service field preferred. High school diploma or equivalent required.
Experience - For incumbents with a high school diploma or equivalent, two years' work experience working with youth preferred.
Skills - Verbal skills to relate effectively with youth on an adult level, without patronizing, are required. Group counseling and leadership skills are required. Good analytical skills and the ability to distinguish between cause and effect and opinion and fact are also required, as is the capacity to think and act independently. Certification in First Aid, CPR, and appropriate crisis intervention methods must be obtained during the probationary period. Proficiency in group work must be demonstrated during probationary period and remaining employment with YOI. Must be able to provide accurate, objective written documentation of shift events and unusual incidents.
Physical Demands - Must be able to lift 30 pounds from floor level to shoulder height. Extensive walking and standing required. Must be capable of detecting and rapidly responding to client altercations, attempts at self-harm, or escape attempts, and must be able to apply approved physical restraint techniques. Must be capable of participating with clients in active and passive team or individual activities in outdoor or indoor environments, occasionally during inclement weather. Must be capable of transporting a client. Must be free of communicable diseases as defined by the state and to the extent that such knowledge is attainable under the law.
Other - Must maintain an appropriate and valid state driver's license. Must meet state and criminal background check requirements.

Position Summary
The Case Manager is responsible for delivering exceptional customer service and ensuring the accurate, timely, and thorough coordination of patient referrals. This role supports continuity of care by managing referral workflows, securing authorizations, and serving as a liaison between patients, providers, and external specialists. The Case Manager also identifies community resources and supports patients in overcoming barriers to care.
Description of Primary Responsibilities
1) Customer Service
a) Provide professional and courteous support to patients in accordance with Palms Medical Group within his/her field of training in accordance with stated policies and procedures of PMG.
c) Answer, screen and route telephone calls promptly and professionally (within 3 rings)
e) Operate a multi-line phone system efficiently, including transferring calls, managing voicemail and navigating queues).
f) Document phone interactions in the EHR.
g) Maintain a working knowledge of all PMG departments and services.
2) Referral Coordination
a) Follow standardized referral policies and workflows
b) Prioritizes referrals based on urgency and organizational goals.
c) Ensures complete and accurate demographic, insurance information and clinical information is submitted to specialists
d) Communicate referral details and expectations to both providers and patients. including next steps and contact
e) Ensure all necessary consents are obtained or coordinated
f) Process incoming and outgoing patient data to support continuity of patient care
3) Community Resource Utilization
a) Identify and maintain relationships with local service providers and community resources.
b) Assist patients in navigating to language or social barriers that may impact referral completion.
4) Referral Tracking
a) Follow EHR protocols for referral tracking.
b) Retrieve and document from the inbox.
c) Record all reports received and close open orders per PMG policy and procedure
d) Ensure timely processing of all referrals.
5) Insurance Authorizations
a) Contact insurance companies to verify and obtain prior authorizations.
b) Present necessary medical information to prove medical necessity of services
c) Maintain knowledge of insurance-specific authorization requirements and document accordingly.
6) Administrative Duties
a) Adhere to the Referrals Policy Manual, Medical Records Policy Manual and HIPAA Policy Manual
b) Check and respond to emails at least twice daily.
c) Maintain patient confidentiality in compliance with HIPAA regulations.
d) Assist with audits and surveys as directed by the Case Manager Coordinator
e) Perform any other duty assigned by the Case Manager Coordinator, Executive VP of Patient Services or CEO to improve the efficiency of PMG
