Center for Human Development (CHD), is seeking Senior Direct Care professionals for a new Intensive Treatment Residential (ITR) program that will be located in West Springfield, MA. The program is slated to open in November of 2025. This program provides long term support for male adolescents (5-13 years of age). In this program, the population served will receive housing, therapeutic, emotional, and educational support with a goal to develop healthy and fulfilling relationships that will solidify skills and apply them to real world settings. Your role as Senior Direct Care: The Senior Direct Care assists program leadership with the oversight of their assigned shifts to meet the needs of the youth and their families. The behavioral health profiles/histories of the youth being referred to the congregate care settings are complex and require 24/7 supervision. The Family Support Provider (FSP) for Stronger Families, Stronger Futures (SF2) provides support and education in the home environment to parents and their children (pre-natal to age 5) who are at risk for child abuse and neglect. The FSP works in collaboration with service providers to reduce abuse or neglect. SF2 also focuses on the child development of the child(ren) through various assessments and the Parents as Teachers curriculum. We focus on the three areas of emphasis; Family Well-being, Parent-Child Interaction, Development-Centered Parenting. The Primary Care Behavioral Health Provider - Independently Licensed operates as a behavioral health consultant on the medical team utilizing the Primary Care Behavioral Health (PCBH) model of care. Acting as a generalist, the PCBH Provider sees patients of all ages with wide-ranging medical, behavioral health and behaviorally influenced conditions. As a fully integrated member of the primary care team, the PCBH provider provides episodic behavioral health services and interventions to a high volume of patients.
