Job SummaryThe Advanced Practice Provider (APP) cares for our patients within the scope of training and approved the agreement, as outlined by their governing board. The Advanced Practice Provider will be responsible for delivery of personalized compassionate medical care to patients primarily with CKD through ESRD. The Advanced Practice Provider will be responsible for caring for patients, maintaining accurate patient records and scheduling and administering follow-up appointments to patients as required across multiple markets. The successful candidate will work as a team with our field-based nurses, community health workers, and physicians to assist in delivering premium care to every patient. Primary duties include patient health assessment, creating strategies to improve or manage a patient's health, and introducing habits for health promotion. Advanced Practice Providers perform the important task of educating patients about preventative care and prescribed treatments. He/she may also conduct physicals, order tests, prescribe medications, and serve as a coordinator with the patient's primary healthcare provider and their specialists.
Roles and Responsibilities
* Travels throughout assigned territory to conduct home conducts in-home assessments, which include comprehensive annual wellness exams, focused screenings, or other visits both in the patients' home and in the virtual environment
* Ability to travel out of state for up to 2-weeks at a time as necessary
* Counsels and educates patients and families about benefits and programs available to help them live healthier lives
* Documents items such as: appropriate chief complaint, all applicable diagnosis, past medical, family, and social history, review of systems, examinations, medications, allergies, assessment, and plan

JOB SUMMARY
FPDS is seeking dedicated After-School Care Workers (Part-Time) to assist our teaching staff after regular classes end each day. After-School Care Workers, under the direction of the After-School Care Director, are responsible for providing safe, nurturing, and fun care for students in Preschool 3 through Grade 6, while creating a positive, Christ-centered environment. JOB TYPE, SUPERVISION AND REPORTING RELATIONSHIP
This job is for part-time employment during the normal school year, August through May, from 2:15-6:00 pm daily, Monday through Friday. The job reports to the After-School Care Director.
RESPONSIBILITIES
Focus: Reflect the overarching purpose of the school, which is to honor Christ, in every activity. Help enforce the school's behavior and safety policies while maintaining a loving, Christ-centered atmosphere.
Supervision and discipline: Supervise students in Preschool 3 through Grade 6 during after-school activities. Assigned activities and student age group may vary daily. Organize and lead assigned activities such as games and outdoor play that promote social, emotional, and spiritual development. Assist students with homework and provide educational support as needed. Assist with set-up and clean-up after activities, ensuring spaces are left in good order for the next day. Keep proper discipline in the classroom and on all school premises. Follow the Matthew 18 principle in dealing with conflict.
Safety: Ensure students' safety at all times, both indoors and outdoors, by maintaining vigilance and following established protocols. Know the procedures for dealing with emergency situations.
Communication: Foster positive relationships with students, encouraging good behavior, kindness, and respect among them. Maintain open communication with the After-School Care Director regarding student behavior, notable incidents, and general atmosphere.

The Monogram Advanced Practice Provider is a Nurse Practitioner is a critical role that provides access to comprehensive care to underserved patients within their assigned territory. PRIMARY RESPONSIBILITIES:
The Post-Adoption Case Manager/Community Support Specialist (CM/CSS) provides ongoing support to families after an adoption is finalized, helping to strengthen family functioning and resolve issues that may arise. The CM/CSS coordinates services, supports and advocates for the family and connects resources to the family. The CM/CSS focuses on monitoring the progress of a family whose needs require the services of many different professionals, agencies, health care facilities, and human services programs. It typically involves assessing family needs, connecting with community resources, and acting as a service coordinator for the family. Knowledge of best practices, working cooperatively with other agencies within the system of care framework, family friendliness and wraparound philosophy are a necessity.
This position will provide assistance to the Adoption Program Specialist with recruitment and training of respite families and will assist in coordinating support groups and group outings.
ESSENTIAL DUTIES:
The CSS/CM will maintain a caseload of cases which included all identified youth/families who have adopted, are adopting and in need of short-term intensive case management. Increase awareness of post-adoption services and maintain effective working relationships with other child-placing agencies.
The CSS/CM will be responsible for developing and implementing service plans, coordinating services, and providing direct support to the family and child as well as, creating community support plans and/or discharge plans for those families receiving post-adoption services.
The CSS/CM will maintain accurate documentation on all activities in a timely manner, including, but not limited to, case notes, assessments, service plans, case staffing notes, and weekly/monthly reports.
The CSS/CM will maintain confidentiality both in writing through signed release of information forms and in day-to-day interactions with community resources and service families. This will include obtaining information from other service agencies on the consumers served.
When assigned, will work with Adoption Specialist to serve a family's support needs and facilitate service coordination if requested by the family.
The CSS/CM will develop and maintain contacts with other agencies in order to facilitate the creation of service teams and will serve on the child's service team if there is a need identified by the team and/or family.
The CSS/CM will provide transportation as needed for the families served or make arrangements for the families' transportation as needed.
The CSS/CM will provide information and guidance to the family system in order to help meet their own needs in the future by offering guidance, training, and support in areas such as behavior management and coping with past trauma in order to strengthen family relationships.
The CSS/CM will be responsible for gathering data on all assigned cases. This may involve assisting the Program Director or Post Adoption Specialist with enrolling youth from their caseload into the assigned Electronic Health Record.
The CSS/CM will maintain 20 contact hours each week, as well as, other assigned statistical information and assist families during crises, providing immediate support and de-escalation.
Upon employment, the CSS/CM will complete the WRAP 101 training, case management, and Trauma-focused Cognitive Behavior Therapy web based trainings to familiarize him/herself with the type of trauma treatments used.
All other duties as assigned.
KNOWLEDGE/EXPERIENCE:
Bachelor's degree in Social Work or similar field required.
Two years experience in case management/crisis intervention.
SKILLS/ABILITIES:
Ability to work well with seriously emotionally disturbed youth using positive, stengths-based principles and relate warmly to families and children without violating boundaries.
Ability to utilize training to respond appropriately in a crisis situation. Always putting the needs of the youth and family first, and making timely contact with all parties to resolve crisis situations.

Key Responsibilities
Deliver safe, confidential, and respectful care at all times.
Provide verbal and/or physical prompts to support individuals in daily activities and promote independent living, such as working toward personal goals, meal preparation, cleaning, transportation, and social engagement.
Accurately document progress toward individual goals and maintain timely and detailed logs during each shift.

Monogram Health is seeking a full-time Palliative Care Advanced Practice Provider (APP) to join the team to support our rapid nationwide growth. This Palliative Care APP will be an in-market provider delivering in-home care for a highly complex patient population in their community, including end-of-life patients with CKD/ESRD, diabetes, COPD, and CHF. The Palliative Care APP will support the Market Physician Executive, the Palliative Care Lead Physician, and an interdisciplinary team to provide palliative care, clinical guidance to patients and their families, including pain and symptom management, develop appropriate care plans and document advance directives to ensure patient-centered care.

Roles & Responsibilitiesâ¯
Collaborate with the Market Physician Executive and clinical leaders including Palliative Care physician to proactively identify patients who qualify for palliative care services and build a patient panel
Develop individualized care plans in collaboration with Market Physician Executive and other clinical leaders including Palliative Care physician to ensure patients are placed on appropriate evidence-based interventions and pathways
Provide compassionate, patient-centric palliative care primarily through in-home visits, with virtual care as needed, to support outcomes and care in alignment with patient's goals
Evaluate and treat symptoms that may interfere with quality of life, like pain, pruritus, depression, anxiety, insomnia, etc.â¯
Collaborate with pharmacists and other clinicians to manage medication panels for complex patientsâ¯â¯
Provide resources and educate patients and their families on the progression of CKD, ESKD, and related conditions, including treatment options and palliative care principles
Help families navigate complex medical decisions and improve understanding of prognosis, including advance care planning to discuss values, goals of care, and document patient/family wishes in state AD/POLST formsâ¯
Deliver evidence-based, timely care in a manner that reduces avoidable hospitalizations, maximizes quality of life, and puts patient health and satisfaction first
Support patients utilizing our interdisciplinary team, including Palliative Care Chaplain
Assist with transition to hospice care as appropriateâ¯
Participate in monthly meetings with clinical leadership to drive Program KPIs and support execution of clinical and financial goals
Educate and support team members on palliative care, hospice care, advance directives, symptom assessment, and communication
As needed for patient care, conducts assessments, which includes comprehensive annual wellness exams on patients both in the patients' home and in the virtual environment
Counsels and educates patients and families about benefits and programs available to help them live healthier lives
Documents items such as: appropriate chief complaint, all applicable diagnosis, past medical, family, and social history, review of systems, examinations, medications, allergies, assessment, and plan. Completes all documentation and paperwork in a timely manner
Responsible for the coordination of care with primary care providers, specialists, and appropriate ancillary services
This position will not be office-based but will be remote in state in which employed and will need to attend periodic training/meetings outside of that state
Prescribe medications, order tests, and collaborate with patient's Monogram physician
Perform effectively, as reflected by improved patient quality outcomes, which will be measured and reported daily
Facilitates closing gaps in care by educating patients about preventive monitoring and working with physician practices to schedule diagnostic testing
Assists patients with enrolling to access educational videos
Participates in the integrated care team meetings
Make assessment of patient's health status and develop treatment plan
Implement a plan consistent with patient' goals of appropriate plan of care
Follow-up and evaluate patient's status
Other duties as assigned
Minimum Qualifications
Active and unrestricted Nurse Practitioner or Physician Assistant licenseâ¯
Board certified for appropriate licensure (NP: ANCC/AANP; PA: NCCPA)
Current and unrestricted DEA certificate
Ability to work without direct supervision and practice autonomously
Must possess reliable transportation and the ability to travel within the designated region to meet patient needs including in-home and clinic visits as needed. Access to transportation, a valid driver's license, and car insurance
Active state-specific licensure and willingness to become licensed in multiple states as company and patient needs require
2+ years working with patients in a palliative care setting; other relevant experience will be considered
Preferred Qualifications
Is certified in ACHPN or CHPN
Demonstrated experience in clinical assessment, care plan design, and evaluating patients using evidence-based clinical criteriaâ¯
Strong communication skills for sensitive conversations about prognosis and end-of life decisions including goals of care discussions and advance care planning
Proficiency with electronic health record (EHR) systems to document complex case details
