Azleway's Substance Abuse Program (ASAP) is seeking qualified individuals who can serve as Direct Care Staff to youth struggling with substance abuse. This position is critical to the delivery of services to a vulnerable population and provides direct supervision of clients at all times. The position provides clients with role modeling and assists them in recognizing behaviors that may be inappropriate and that place them at risk. The Direct Care Staff also provides Life Skills Training, the necessary transportation for clients to and from activities and appointments, and support in the academic and therapeutic environments.
Skills and Abilities:
Must take a positive approach in managing negative behavior through clear communication with with clients in an appropriate, understandable, non-judgmental, and non-threatening manner. Must have good oral and written communication skills. Must be able to maintain all documentation, including, but not limited to, incident reports, daily logs, van logs, communication logs, and medication logs.
Qualifications:
Must be at least 23 years old, have a valid Texas driver's license. Bachelor's degree or high-school diploma preferred.
Responsibilities:
Communicate clearly and efficiently with other staff members in the organization.
Maintain open communication with supervisor and communicate issues as they arise.
Maintain all documentation as instructed by supervisor.
Adhere to all program policies, procedures, rules, and guidelines as established by the agency.

Monitor client behavior and enforce program rules consistently, without judgment, prejudice or malice.
Maintain healthy boundaries with clients, relatives and visitors.
Meets dress and appearance requirements as directed by supervisor.
Direct Care Staff will be punctual and reliable in work habits and will focus on work related issues while on the job site (i.e., No personal telephone conversations, no visits by friends or family to the work station, no personal computer use, etc.).
Advocates for client at all times, avoiding derogatory and/or inappropriate remarks regarding clients.
Acts as a corporate customer service representative in all organization related capacities.
Adheres to all policies and procedures regarding client services (e.g., providing client bill of rights, rules of program, client confidentiality, testing, etc.).

Assists in developing and facilitating wellness activities.
Assists with the planning and hosting of an annual Turkey Trot.
Assists in the day-to-day operations of the Wellness Centers.

GENERAL DESCRIPTION
The DirectSupportProfessional staff provides direct care services and supports to persons with intellectual and/or developmental disabilities. Duties consist of quality interaction and support of individuals in all activities of daily living, personal care, routine housekeeping, cooking, shopping, skills training and community integration. Settings in which services are provided include community (public) settings, individuals home, and center-operated Individual Skills and Socialization/Day Habilitation training sites.
Performs moderately complex (journey-level) habilitative and rehabilitative care work with individuals who have developmental disabilities. Works under general supervision, with limited latitude for the use of initiative and independent judgment.
The assigned shift will generally be M-F, 8am-4pm; however due to client needs, scheduled events, or staff outages there are times in which staff will be asked to provide services outside the typical M-F, 8-4. These events are typically known in advance and supervisor will work with staff to flex their schedule. If flexing is not able to occur, overtime pay will be given for hours exceeding 40 hours in a week (Sunday-Saturday). In the event a client service is scheduled on a center holiday, holiday pay will be given for time worked.
ESSENTIAL DUTIES AND RESPONSIBILITIES
Service Delivery
Provides direct care, meaningful activities and skills training to individuals with intellectual and/or developmental disabilities as outlined in the Person Directed Plan, Individual Plan of Care and Implementation Plan. This may include assisting individuals with hygiene and personal care.
As a member of the service planning team, provides input regarding individuals service participation and progress towards goals.
Provides transportation for individuals based on identified service and support needs.
Health and Safety
Provides direct supervision of self-administration of medications, or administers medication in accordance with Physician's orders and as delegated by R.N.
Observes individual for signs and symptoms of disease, injury, reaction to medications, behavioral changes, or other conditions that warrant medical intervention. Report significant changes in individual's physical conditions or behavior.
Utilizes approved techniques from Prevention and Management of Aggressive Behavior (PMAB) training to manage aggressive behavior among individuals, and provide intervention to ensure that individuals are protected from harm or injury.
Assists in the implementation of behavior support plan, as determined by Licensed Behavior Analyst. This may include completing behavior data cards and interventions specific to targeted behaviors.
Documentation
Required to register as a provider in Texas Medicaid Electronic Visit Verification (EVV). Must complete EVV visit documentation through the EVV portal at the time of service delivery as required by Texas Medicaid.
Completes progress notes and service logs within timelines specified by division policy.
All documentation must meet billing guidelines for the individual's specific program (ex. Home and Community-based Services, General Revenue, Texas Home Living, PASRR).
Completes incident reports within the time frames specified by the center.
Responds to inquiries or directives from division staff tasked with data management and quality assurance. All inquiries should be responded to within 1 business day, unless a shorter timeline is required due to state reporting of encounter data or billing. This includes data in both the center's Electronic Health Records (EHR) and the Electronic Visit Verification (EVV).
Performs other duties as assigned.
Demonstrate high ethical standards, trustworthiness, and personal integrity. Regular attendance, dependability, and promptness are required for the work day hours 100% of the time, to ensure consistency and completeness of program's processes.
Professional Development - Successfully complete training required to maintain skill competency and cross training to ensure consistency across program procedures. Complete continuing education as required by professional standards and the Center.
Frequent daily driving within the catchment area is required.
Staff member is expected to maintain their mental and emotional well-being as part of their role. GENERAL QUALIFICATIONS
Experience/Education/Licensing/Certification
