The role of the DirectSupportProfessional is to help individuals live in their own homes, identify and achieve goals, and improve the quality of their lives. Your role is responsible for providing personal care and helping these individuals to live independently and achieve their goals.
Job Roles/Responsibilities:
Provide directsupport assistance to the person served.
Assist person served with daily living activities (including, but not limited to bathing, dressing, and grooming)
Complete mandatory online training modules
Help individuals develop and maintain independent living skills
Provide transportation to appointments and community activities
Administer medication and monitor health conditions
Maintain accurate records and documentation
Communicate effectively with individuals, families, and other team members
Schedule: Friday 3p-11pm
Pay Rate: $16.00 an hour
Requirements

No more than two minor moving violations in the past 3 years

7. Preparation of shopping list appropriate to a individual's dietary needs and financial circumstances, performance of grocery shopping activities as necessary and preparation of meals.
8. Personal laundry.
9. Incidental neighborhood errands as necessary, including driving and accompanying the individuals to medical or other appropriate appointments and accompanying individuals for short walks outside the home.
Physical: Be aware of exposure to occupational diseases. Able to be physically capable of safely intervening
Requires frequent sitting, standing, walking, driving, and occasional bending, lifting, pulling, squatting, and stooping. Good hearing and ability to tolerate loud noise levels and vision with or without correction are essential.
Verbalization, reading, and writing skills are essential. Must be able to lift and move up to 25 lbs. without assistance. In some cases, lifting, pushing or pulling requirement will be higher. Possible exposure to chemicals, i.e., insect extermination, cleaning chemicals, etc.
Must have a valid Ohio Driver's License with no more than 2 points if a driver.

Attends meetings and training sessions as required by DODD and Sangomas Residential;
Completes annual trainings related to consumers' IP, BP and related plans;
Daily Programing
Provides transportation to and from activities;
Assists in the implementation of individual plans for individuals and behavioral plan goals;
Assists individuals in the pursuit of recreational, social and community activities based on their preferences and individual plans;
Assists and encourages individuals in the establishment of social networks; Promotes participation and involvement in community outings;
Works all assigned schedules, including but not limited to arriving and departing on time, requesting time off in advance, and notifying your supervisor of changes to your assigned work schedule;
Ensures each individuals' rights are protected.
