Key Responsibilities Conducts hospice referrals and accurately completes admission paperwork/consents. Conducts hospice comprehensive assessments at the time of admission. Assess the patients and family's social emotions and financial needs. Assesses the patient's/family's psychosocial needs on an ongoing basis while hospice care is being provided through comprehensive assessment updates. Maintains an individualized, up to date care plan that reflects the care being provided to patients/families. Provides social work visits/services to patient/family as determined in the plan of care and through the plan in documentation. Provides PCG and family with periodic updates as indicated in the plan of care Refers patient/family to appropriate community resources. Coordinate patient care with contracted nursing homes and hospitals. Provides accurate, efficient patient status reports to team members in appropriate meetings, and in periodic updates.
