Position Detail:
The Community Liaison serves as a vital connector between our healthcare services and the community. This role is responsible for driving business development and managing both inbound and outbound communications. The Community Liaison acts as the local market leader for B2B (business-to-business), B2C (business-to-consumer), and DTC (direct-to-consumer) engagement, representing our organization to referral partners, patients, and caregivers.
Key Responsibilities:
Business Development & Outreach:
Identify opportunities for in-services in the health care community and coordinate their implementation with appropriate persons
Build and maintain strong relationships with referral sources such as hospitals, physician offices, senior living communities, and other healthcare providers.
Identify and pursue new business opportunities to grow patient census across service lines.
Represent the organization at community events, health fairs, and professional networking functions.
Patient & Family Engagement:
Educate patients, families, caregivers, and legal representatives (POAs, guardians) on the benefits and services offered through Hospice.
Serve as a compassionate and knowledgeable resource to guide families through the enrollment process.
Communication & Coordination:
Develop relationships with medical staff offices, hospitals and nursing homes; act as a consultant to medical staff and hospital committees, as required to provide hospice and end-of-life information
Facilitate seamless communication between referral sources and internal clinical teams.
Ensure timely follow-up on referrals and maintain accurate documentation of outreach activities.
Market Leadership:
Provide formal and informal education opportunities with physicians, physician office staff, hospital social workers/discharge planners/patient unit staff, nursing home staff and staff in community agencies.
Collect and analyze data of referral patterns from all referral sources, to include physicians, social workers, discharge planners, hospital vs community physicians and SNFs.
Maintain a thorough knowledge of the community's health care market, competition, consumer demands and governmental policies which influence the agency's outreach goals and access for patients/families
Act as the face of the organization in the local market, promoting brand awareness and trust.
Monitor market trends and competitor activity to inform strategic planning.
Skills and Experience Required
Proven experience in healthcare marketing, sales, or community outreach-preferably in home health, hospice, or a related field.
Demonstrated ability to meet or exceed performance goals and key performance indicators (KPIs). This includes:
Referral volume targets
Patient admission and conversion rates
Community engagement metrics
Timely follow-up and documentation standards
Relationship development with referral sources
Strong interpersonal and communication skills, with the ability to engage diverse audiences.
Knowledge of Medicare/Medicaid and other payer sources is a plus.
Knowledge of Hospice eligibility and levels of care is preferred
Knowledge of customer referral source [CRM] management systems for activity tracking
Ability to work independently and collaboratively in a fast-paced environment.

WHAT WE'RE LOOKING FOR:
The Assisted Living Coordinator coordinates and addresses the health concerns of each Assisted Living Resident including resident assessments, medication/pharmacy management and resident activities of daily living and personal care needs.
Key Responsibilities
Collaborates with the Director of Nursing (DON) and Assistant Director of Nursing (ADON) relating to day-to-day operation of clinical services for the entire campus.
Ensures that each resident's pre-admission screen is completed prior to move-in.
Directs the Lifestyle and Care Needs Assessments of Residents to determine the initial and ongoing needs of the residents' medical and personal care requirements.
Assists Residents with activities of daily living (ADL), including but not limited to bathing, dressing, grooming, dining, etc. And medication administration.
Leads and assists in the development of the service plan in conjunction with the resident/family and ensures communication of the plan to care givers.
Facilitates the Assisted Living Resident Meeting and follows up with feedback and concerns in a timely manner.
Works as the lead nurse for the assisted living residents and is part of the on-call rotation as designated by the DON.
Qualifications
Active, unencumbered LPN license within the state

Community Outreach Liaison Key Responsibilities:
Generates referrals and admissions using the four-cornerstone approach to business development, within the assigned territory.
Develops and implements territory sales plan which will meet the goal of expanding the customer base in the market areas.
Monitors and recognizes changing referral patterns in assigned area and modifies strategies to meet goals.
Documents all sales related activity within the CRM daily.
Responsible for meeting monthly and annual referral and admission goals in the assigned territory.
Identifies, develops and maintains key relationships in the territory, by participating in various functions sponsored in the medical community.
Promotes Ohio's Hospice through educational opportunities and community events in assigned territory.
Assists in other activities and departments when requested.
Participates in Continuous Quality Improvement activities to support the quality of Ohio's Hospice services.
Precepts new staff to support professional relationships with newly hired team members as required.
Community Outreach Liaison Qualifications:
Must have a bachelor's degree or equivalent experience in healthcare or business development
Preferred two years in a sales/marketing role within the healthcare industry with a proven record of accomplishments in direct sales and marketing.
Computer skills sufficient to properly document services and communicate effectively, CRM experience preferred.
Ability to drive during daytime, nighttime, or inclement weather.
Valid Driver's License with Safe Driving Record

Position Detail:
The Community Liaison serves as a vital connector between our healthcare services and the community. This role is responsible for driving business development and managing both inbound and outbound communications. The Community Liaison acts as the local market leader for B2B (business-to-business), B2C (business-to-consumer), and DTC (direct-to-consumer) engagement, representing our organization to referral partners, patients, and caregivers.
Key Responsibilities:
Business Development & Outreach:
Identify opportunities for in-services in the health care community and coordinate their implementation with appropriate persons
Build and maintain strong relationships with referral sources such as hospitals, physician offices, senior living communities, and other healthcare providers.
Identify and pursue new business opportunities to grow patient census across service lines.
Represent the organization at community events, health fairs, and professional networking functions.
Patient & Family Engagement:
Educate patients, families, caregivers, and legal representatives (POAs, guardians) on the benefits and services offered through Hospice.
Serve as a compassionate and knowledgeable resource to guide families through the enrollment process.
Communication & Coordination:
Develop relationships with medical staff offices, hospitals and nursing homes; act as a consultant to medical staff and hospital committees, as required to provide hospice and end-of-life information
Facilitate seamless communication between referral sources and internal clinical teams.
Ensure timely follow-up on referrals and maintain accurate documentation of outreach activities.
Market Leadership:
Provide formal and informal education opportunities with physicians, physician office staff, hospital social workers/discharge planners/patient unit staff, nursing home staff and staff in community agencies.
Collect and analyze data of referral patterns from all referral sources, to include physicians, social workers, discharge planners, hospital vs community physicians and SNFs.
Maintain a thorough knowledge of the community's health care market, competition, consumer demands and governmental policies which influence the agency's outreach goals and access for patients/families
Act as the face of the organization in the local market, promoting brand awareness and trust.
Monitor market trends and competitor activity to inform strategic planning.
Skills and Experience Required
Proven experience in healthcare marketing, sales, or community outreach-preferably in home health, hospice, or a related field.
Demonstrated ability to meet or exceed performance goals and key performance indicators (KPIs). This includes:
Referral volume targets
Patient admission and conversion rates
Community engagement metrics
Timely follow-up and documentation standards
Relationship development with referral sources
Strong interpersonal and communication skills, with the ability to engage diverse audiences.
Knowledge of Medicare/Medicaid and other payer sources is a plus.
Knowledge of Hospice eligibility and levels of care is preferred
Knowledge of customer referral source [CRM] management systems for activity tracking
Ability to work independently and collaboratively in a fast-paced environment.

The position of Community Business Development Liaison (CBDL) aspires to be the leader in the market's external, community-based business development, while being an advocate and example of superior customer service. They work to continuously build census through community-based relationships, events, and public relations. The CBDL will develop external relationships with community accounts including, but not limited to, home health agencies, hospice, physicians, senior service organizations, senior centers, non-medical home health, and disease specific advocacy organizations. This dynamic leader will work in tandem with the center's Admissions Directors to ensure census development efforts are aligned and yield positive outcomes. The CBDL drives the community marketing and lead base management programs to be a resource to our communities. They help to support and elevate the community relationships of the Admissions Directors by providing market intelligence, strategic planning, and direction of relationship development.

JOB DUTIES/RESPONSIBILITIES
As the Community Business Development Liaison, you will take charge of:
Census Development
Responsible to exceed budgeted census goals and grow market share within assigned market by working in collaboration with Admissions Directors, Executive Directors, and Liaisons.
Develop, maintain, and grow community-based relationships with diverse referral sources that include but are not limited to home health, service organizations, senior service programs, senior living communities, non-medical home services, and durable medical companies.
Participate in Business Development meeting and planning of facilities in the market territory.
Cultivate a lead database through prospecting, person-centered sales process, and creative follow ups
Responsible for planning, with business development team support, and hosting two community and two professional events per month.
Responsible for onsite assessment and completion of onboarding form of community referrals.
Responsible to maintain community sales and referral log and document sales efforts, referrals, and admissions.
In rare exception where CBDL is assigned a hospital: Onboarding Forms and bedside assessment are required on all readmit and new referrals and good working relationships with DC planners must be maintained.
Assess and recommend additional marketing opportunities such as direct mailings, media campaigns, etc.
Assess and recommend program development or specialty service opportunities based upon market needs.
Attend All mandatory regional and CHS sales and marketing trainings and meetings.
All other assigned duties by Regional Director of Sales and Marketing
Admissions Process
Train on the Admissions Process. Possess a comprehensive understanding of admission criteria, PASRR and LOC criteria, and contract compliance.
Support the Central Intake Admissions Process, ensuring that referrals are processed through the Central Intake Department.

Activate Care is hiring a team of hybrid, Community Health Workers-(CHW) located in Nevada, who will play a key role in supporting the screening, assessment, and care navigation for local Nevada community members enrolled in the Path Assist program. This role will be both work from home, and require commuting in the field or local designated area. This is an exciting role that will help accelerate local change happening in your state to drive toward better and more equitable community health.

Responsibilities:
Provide care coordination and resource navigation to an assigned caseload of community member clients with unmet social needs.
Conduct consistent telephonic outreach, follow-up, and coaching to clients to assist with enrollment in services/benefits/programs for which they are eligible.
Administer social determinants of health (SDOH) screening, intake forms, and any needed assessments in the Activate Care platform.
Assist clients with prioritizing goals and creating client-centered care plans.
Coordinate with community nonprofits and resources to help clients meet their needs.
Provide resources to clients to improve their health literacy and self-sufficiency.
Take a proactive approach to assist with assigned cases (eg. help schedule appointments, complete applications, make reminder calls, etc.)
Maintain client privacy and uphold confidentiality at all times.
Participate in weekly team meetings, workshops, and trainings to expand knowledge of department priorities, while remaining current on new developments, as required.
Ability to commute to and from client's homes
Other duties as assigned.
Requirements
Qualifications & Skills:
Degree requirements: Candidates should possess a minimum of a high school diploma or equivalent.
Must have a valid driver's license in the state of Nevada
Must be able to use personal vehicle to commute to and from client's homes
2-3 years of relevant work experience providing direct care coordination services to individuals and families (
preferred
)
Experience working directly with nonprofits, social service providers, faith-based groups, or government agencies that address social determinants of health.
Exceptionally strong independent working skills with strong communication.
A collaborative team player who is committed to supporting, encouraging, and helping their team of colleagues.

Summary of Position: The Community Outreach Specialist is responsible for building relationships with key contacts at the assisted living facilities, nursing home facilities, hospitals, and physician offices for adding clients to EverHeart Hospice programs. The Community Outreach Specialist is responsible for educating the community and referral sources about EverHeart Hospice services.
Education and Experience:
Bachelor's degree in marketing, business or health related science preferred
Minimum of two years of Hospice or community health experience.
Healthcare sales and/or marketing experience preferred.
Special Skills and Requirements:

Responsibilities of Position:
Develop profiles for referral sources to include current key personnel, current occupancy, needs assessment, and track referral statistics.
Calls on physicians, facilities (hospitals, skilled nursing, rehabilitation, assisted living, etc.) within EverHeart's service area to open new accounts and promote Hospice and Palliative Care referrals.
Coordinate training needs for referral sources with appropriate presenters.
Participate in community programs, events, and health fairs.
Act as a liaison between referral sources and our staff to provide information on EverHeart's Services, resolve problems and maintain positive customer relations.
Make sales calls, arrange appointments and deliver educational programs to keep referral sources current in our offered services.
Establish and maintain effective working relationships with appropriate stakeholders to keep service offerings abreast.
In conjunction with the Chief Growth Officer, prepare a goal-directed development plan and quarterly marketing plan.
Meets with appropriate EverHeart Hospice staff to discuss sales activities, new opportunities, potential problems, etc. on a regular basis. Gathers and organizes account related information and provides input on key customer opportunities, services and other offerings.
Work collaboratively with the Business Development Team and the Admissions Specialist to identify and address issues with referral sources.
Utilize and analyze the referral source database.
Maintains market awareness and prepares competitive updates.
Other duties as assigned.
