Responsibilities
Our PRN Registered Nurse (RN) administers skilled care visits to clients requiring intermittent professional services and teaches the client, family, and other members of the health care team. These services are performed in accordance with the physician's orders and the established plan of care, under the direction and supervision of the Branch Director.
Qualifications 1. Must hold a current and unencumbered Registered Nursing License in the state which you intend to work.2. Must be organized, detail-oriented and possess effective communication skills both orally and in writing. 3. RN must understand the issues related to the delivery of home health care and be able to problem-solve effectively. 4. Ability to assume a flexible work schedule.

Our Transition Team is a supplemental care unit of skilled nurses whose primary function is to quickly and efficiently get patients home while primary nursing staff are actively recruited. This team is deployed after we receive a patient referral for home care services and the patient is admitted to Maxim.
Transition Team nurses provide care for all authorized hours in a home during a provisional period between 1-90 days. During this time, our office staff will be recruiting to identify full-time, replacement home healthcare nurses. The new caregivers will have an opportunity to train and shadow in the home with the Transition Team caregivers prior to working independently with the patient. Once the replacement nurses are trained and have a schedule, the Transition Team are then phased out of the home.
