1978

Ashland Community Hospice, Inc. was established in the spring of 1978 by a group of concerned citizens who recognized the need for a program that would provide physical and emotional care to terminally ill patients and their families.

In 1978, our founders — Carolyn Jaffe, RN and Peter Van Arsdale, PhD — recognized the need for comprehensive, expert and collaborative care for those facing aging, serious illness and grief.

1979

Founded in 1979, Addus HomeCare is one of the nation’s largest home care providers.

1982

In 1982, five women with a vision and $20 each banded together to form Hospice for McHenry County.

1986

In 1986 as the organization began serving additional counties in Eastern Kentucky, its name was changed to Community Hospice, Inc.

In 1986, Hospice of Lawrence County, Ohio merged with Ashland Community Hospice.

1991

In 1991, a satellite office was opened in Louisa, Kentucky to improve access and quality of hospice services to residents of Lawrence County, Kentucky.

1992

In 1992, Horizon became the first Chicago hospice to care for patients with AIDS at end-of-life by partnering with The Chicago House Program.

1994

In 1994 the organization's main office and administrative headquarters were moved to their present location at 1538 Central Avenue, Ashland, Kentucky.

Since occupying our previous facility in 1994, Community Hospice has experienced steady, sustained growth with triple the number of employees and patients served during that time.

2004

On January 26, 2004, Community Hospice, Inc. opened the first freestanding hospice inpatient facility in Kentucky.

2012

In 2012, Horizon opened the first inpatient hospice unit at a major medical center in Chicago at Rush University Medical Center

2015

In 2015, three nonprofit, like-minded and mission-driven organizations, each with deep community roots, exceptional patient-centered care, expert staffs and a history of innovation came together to form one organization, JourneyCare.

2022

JourneyCare was acquired by Addus HomeCare on February 1, 2022.
