    **Become a part of our caring community and help us put health first** Care Manager Support Assistants assist in ensuring TRICARE beneficiaries receive the highest quality health care for Autism Spectrum Disorder (ASD) related services in accordance with the Autism Care Demonstration (ACD). This role supports the Autism Service Navigator (ASN) by verifying eligibility for the ACD and monitoring and supporting the family for 180 to assist with any barriers to accessing services; education and initial registration into ECHO registration portal for active duty family members; assistance with regional transfers and ensuring the receiving Managed Care Support Contractor receives the appropriate paperwork; processes faxes for ECHO eligibility or other paperwork pertaining to ACD requirements such as eligibility, referrals, etc.; and identification of ABA providers and researching necessary resources to support the beneficiary. **KEY ACCOUNTABILITIES** + Assist beneficiaries with eligibility process and support the family to ensure enrollment into the ACD and overcome barriers to obtaining required paperwork. Assist with regional transfers in and out of the region and ensure required paperwork is indexed into the electronic records system. Ensure all paperwork and documentation is notated within the clinical management system. + ASN with care coordination efforts of assigned beneficiary caseloads to include but not limited to managing incoming & out-going communications to manage provide placement; researching time availability and beneficiary preferences that will impact access to care; identifying available local resources based on beneficiary needs; ECHO enrollment issues, identification of ABA providers and other non-clinical beneficiary concerns. Includes education and referrals for initial outcome measure completion for seamless start to ABA services. Interface with providers as necessary and appropriate for issue follow-up and resolution. + Families who are dependents of active-duty service members must be enrolled with EFMP and associate will educate and register them in the ECHO registration portal and begin the provisional period. Assist in monitoring ECHO faxes and provisional paperwork to facilitate transition to permanent status. Ensure documentation is accurate and documented in all applicable systems. Be able to communicate ECHO requirements to beneficiaries to ensure they provide necessary paperwork so there are no gaps in care. + Communicates effectively with other departments to coordinate needs of beneficiaries, support the goals of the ACD and exhibits Humana's values at all times. Oversees the Residency Program. Assists with and provides clinical supervision and evaluation of patient care related to echocardiography. Available to perform inpatient and/or outpatient cardiac ultrasound testing to include 2D echo / Doppler, stress exams, and transesophageal echocardiograms as well as other duties assigned by manager or director. Provides complex technical care with the use of ultrasound technology for adolescent, adult, and geriatric patients and provides all necessary documentation and preliminary exam findings. Must have excellent understanding of structural heart procedures as they relate to echocardiography. Must also have full understanding of the use of 3D cardiac ultrasound. KEY RESPONSIBILITIES: 1. Oversee Residency Program a. Assists with activities related to echocardiography laboratory accreditation for Piedmont Healthcare hospitals to ensure compliance to IAC standards. b. Assists with monitoring and reporting on quality metrics identified by Piedmont Healthcare and associated with IAC standards. c. Compile physician and sonographer competency / registry and CME attendance records. d. Routinely review cardiac sonographer exams as part of a formal quality assurance program. 2. Training and Education a. Assess the educational needs and competencies of cardiac sonographer staff and interns and develop, schedule, coordinate, and conduct educational programming for new and existing staff to improve quality of care and reduce variation in workflow. b. Maintain an up to date knowledge of echocardiography trends and best practice and conduct, participate in, facilitate, and utilize research to foster evidence-based practice. c. Provide clinical supervision and evaluation of patient care related to echocardiography. d. Attend and participate in echocardiography leadership activities. e. Quality/Innovation on echo protocols, policies and procedures f. Maintain relationships with technical colleges and oversee student interns. 3. Clinical Support a. Perform patient evaluations including verifying patient identity, reviewing the medical record for orders and prior studies, and addressing patient questions and concerns. b. Perform echocardiograms including 2D / Doppler with and without contrast, dobutamine stress and exercise stress echo exams, and transesophogeal echocardiograms according to protocol. c. Complete appropriate documentation, post processing, and preliminary findings in the medical record and cardiovascular PACS system. d. Acquire and maintain training and competency in electronic health record system as required to perform duties. e. Maintain personal competency file if applicable. KNOWLEDGE, SKILLS, ABILITIES Skill and competency in performing detailed cardiac ultrasound studies. Skill and ability to communicate effectively both verbally and in writing. Proficient in Microsoft windows-based computer software. Experience with electronic health records and cardiovascular PACS systems. Ability to work as a member of a team.
