The Acute Rehabilitation Unit (ARU) at Kindred Hospital Houston Medical Center is a specialized inpatient rehab program located on the hospital's fourth floor. It's designed for patients recovering from serious conditions like stroke, brain or spinal cord injury, orthopedic trauma, neurological disorders, amputation, or other complex illnesses.
Intensive Therapy: Patients receive at least 15 hours of therapy per week, typically spread across five days, under the direction of a physiatrist-a doctor who specializes in rehabilitation medicine.
Newly Renovated Facilities: The ARU features 11 private rooms, a modern rehab gym with advanced equipment, and dedicated spaces for dining and activities. Each room includes amenities like Wi-Fi, cable TV, private bathrooms, and guest recliners.
Seamless Transition: Because it's located within the hospital, patients can move directly from long-term acute care to rehabilitation without transferring to a different facility.
24/7 Nursing Support: Around-the-clock care ensures patients are supported throughout their recovery journey.
Evidence-Based Approach: The ARU follows guidelines from the Centers for Medicare and Medicaid Services and is recommended by the American Heart Association/American Stroke Association for stroke recovery.

How you'll contribute
Evaluates and conducts medically prescribed physical therapy treatment programs. Provides educational information to patients in regard to injury prevention, ergonomics, restorative therapy and rehabilitation therapy.
Supervises physical therapy assistants and aides in performing treatment.
Performs initial and ongoing assessments of patient's condition.
Establishes a plan of care which is appropriate to problems identified and involves the patient according to his or her capabilities and desires.
Plans, prepares and carries out individually designed programs of physical treatment to maintain, improve or restore physical functioning, alleviate pain and prevent physical dysfunction in patients.
Performs therapy interventions utilizing standard physical therapy techniques and skills as appropriate for the age and condition of the patient.
Directs and supervises physical therapy assistants, students, and other support personnel. Assesses competence, delegates specific tasks, and establishes channels of communication.
Administers treatment involving application of physical agents, using equipment, moist packs, ultraviolet and infrared lamps, and ultrasound machines.
Evaluates, fits, and adjusts prosthetic and orthotic devices and recommend modifications.
Identifies and documents goals, anticipated progress and plans for reevaluation.
Qualifications and requirements:
Degree from an accredited Physical Therapy program
Physical Therapist License in State
Basic Life Support (BLS) obtain within 30 days of hire
Requires critical thinking skills, decisive judgment and the ability to work with minimal supervision.
Must be able to work in a stressful environment and take appropriate action.

The Associate Clinical Director will directly supervise and train primary care providers (PCPs) in his/her assigned center. The incumbent in this role is accountable for maximizing overall core model execution, including improving clinical quality, efficiency, outcomes, and clinician/patient satisfaction. In addition to being accountable for the overall clinical outcomes of his/her assigned center, they will have a portion of their time allocated to direct patient care as a PCP and/or other clinical duties (amount dependent on number of direct reports). The remainder of their time is allocated to leadership responsibilities, including PCP performance, engagement, and building a strong clinical-operations synergy and culture. The allocation of time is dependent on several factors, including PCP capacity, market needs, size of centers, patient membership, and Market Clinical Director direction.
ESSENTIAL JOB DUTIES/RESPONSIBILITIES:
Independently provides care for patients with acute and chronic illnesses encountered in older adult patients.
Takes full accountability for patient care and outcomes and appropriately seeks consultation from specialists when needed, though will still stay involved in, and be responsible for, the detailed care of the patient.
Engages with the hospitalist whenever one of their patients is in the hospital (regardless of whether the hospitalist works for ChenMed or not).
Responsible for assessment, diagnosis, treatment, management, education, health promotion and care coordination and documentation for patients with acute and complex chronic health needs.
Leads their care team consisting of care promoter (medical assistant), care facilitator, and care coordinator for patients able to come to the office.
For patients that are unable to come to the office-in hospital, SNF, LTC or homebound, engages with the transitional care team and others including case managers, acute and transitional-care physicians, and other resources that may be available depending on the market.
Leads Super Huddle (SH) and Transforming Care Meeting (TCM) weekly, as well as supports Center Manager/Center General Manager center clinical leader and/or market clinical leader is not available, based on guidance from Market Chief Medical Officer. Fills in as needed for center clinical leadership needs, including monitoring daily center census as part of joint center accountability for outcomes.
Plays an active role in the management of their center and helps cover for other providers who may be out for various reasons. It is also expected that each Clinical Director will take an active role as needed in recruiting patients for the center and recruiting and interviewing additional providers for the company.
Managing, mentoring and coaching PCPs in his/her assigned center to deliver outstanding clinical outcomes, including sampling other PCP's daily huddles within their center
Leadership rounding with the PCPs (reduced involvement of market clinical leader)
Partnering with Center Operations Director/Market General Manager to drive continued improvement of center financial performance, and helping increase center membership
Performs other duties as assigned and modified at manager's discretion.
KNOWLEDGE, SKILLS AND ABILITIES:
Proficient in Microsoft Office Suite products including Word, Excel, PowerPoint and Outlook, plus a variety of other applications used in the company
Ability and willingness to travel locally as needed in their market, if applicable, nationally for initial training (2 weeks) and then occasionally regionally and nationally for recruiting or training purposes
Fluency in English, verbal and written. There may be jobs in some centers that require fluency in other languages, and this will be made known at the time of application.

EDUCATION AND EXPERIENCE CRITERIA:
MD or DO in Internal Medicine, Family Medicine, Geriatrics, or similar specialty required
Current, active MD licensure in State of employment is required
A minimum of 1-year clinical experience in geriatric, adult or family practice setting preferred, with Lead PCP ideally being a ChenMed PCP Partner
Completion of Chen Medical training, including Masterful Conversations and meeting facilitation, as part of the individual development plan
Board certification in Internal Medicine, Family Medicine, Geriatrics or similar specialty is preferred, Board Eligibility is required
Once Board certified, PCP will maintain board certification in their terminal specialty by doing necessary MOC, CME and/or retaking board exams as required
Must have a current DEA number for schedule II-V controlled substances
Basic Life Support (BLS) certification from the American Heart Association (AMA) or American Red Cross required w/in first 90 days of employment

Duties and Responsibilities:
As a BPA, you will be part of our 60,000+ workforce that strives to protect the American people by safeguarding our borders, deterring illicit activity, and enhancing the nation's economic prosperity. Being a BPA makes you a valuable member of the Federal Law Enforcement Officer (LEO) profession.
Typical assignments include:
· Detecting and questioning people suspected of violating immigration and customs laws and inspecting documents and possessions to determine citizenship or violations.
· Preventing and apprehending aliens and smugglers of aliens at or near the borders by maintaining surveillance from covert positions to include using infrared scopes during night operations.
· Interpreting and following tracks, marks, and other physical evidence of illegal entry of persons or contraband.

Qualifications:
GL-9: You qualify for the GL-9 grade level if you possess one (1) year of specialized experience, equivalent to at least the next lower grade level, performing duties such as:
· Performing physical searches of detained individuals, their vehicles, and their immediate surroundings for weapons, contraband, currency, and other evidence.
· Apprehending, physically restraining, or working with law enforcement officials to identify or apprehend violators of state, federal, or immigration laws.
· Proficient in the use of firearms, preparing investigative reports, serving court orders (e.g., warrants, subpoenas, etc.), and gathering evidence for criminal cases prosecuted through the court system.
If you have previous or current law enforcement or military law enforcement experience, you may qualify at the GS-11 grade level.
GS-11: You qualify for the GS-11 grade level if you possess one year of specialized experience equivalent to at least the next lower grade level, with the authority granted by the state or government to enforce laws, make arrests, and investigate crimes, performing primary duties in an official law enforcement capacity such as:
· Utilizing intelligence information to track illegal operations, criminal activity, threats to our nation, and/or contraband while serving as a state, federal, or military law enforcement official.
· Leading investigations of fraud, contraband, criminal activity, threats to our nation, and/or illegal operations to determine sources and patterns while serving as a state, federal, or military law enforcement official.
· Apprehending violators of state, federal, or immigration laws by utilizing various forms of complex technology that include surveillance, detection, situation awareness systems, and/or communications equipment.
The above experience will be applied in connection with the following: Make arrests and exercise sound judgment in the use of firearms; deal effectively with people in a courteous manner in connection with law enforcement matters; analyze information rapidly and make prompt decisions; or develop and maintain contact with a network of informants.
There are no education substitutions for the GL-9 or GS-11 grade-level Border Patrol Agent opportunities.

Residency: You must have had primary U.S. residency (including protectorates as declared under international law) for at least three of the last five years.

Key Responsibilities:
Technical Support & Troubleshooting
Resolve complex configuration and infrastructure issues through simulation and analysis.
Perform deep technical problem isolation and root cause analysis.
Escalate to vendor (L4) support when necessary to meet SLAs.
Correlate incident trends into problem management and implement permanent fixes.
Infrastructure Management
Administer and optimize Windows Server environments (AD, DNS, GPO).
Manage VMware vSphere environments including ESXi, vCenter, SRM, and vMotion.
Execute monthly patch management and infrastructure updates.
Conduct lab simulations and compatibility testing for new builds.
Client Engagement & Reporting
Generate customer-facing incident reports and operational summaries.
Conduct regular client support visits and technical review meetings.
Present root cause corrective actions and trend analysis to stakeholders.
Project & Process Leadership
Architect and enhance managed service offerings and infrastructure.
Participate in RFP responses and pre-sales technical discussions.

Required Skills & Experience:
10+ years in enterprise Windows environments with multi-site domains.
5+ years in VMware vSphere administration with clustered storage and HA.
Strong knowledge of Active Directory, Group Policy, DNS, DHCP.
Experience with NetApp SAN, Cisco UCS, HP DL servers, and CommVault.
Demonstrated experience in service level management and RCA reporting.
Certifications (Preferred):
Microsoft: MCSA / MCSE / MCITP (Windows Server 2008 or higher)
VMware: VCP-DCV (vSphere 5.x or higher)
ITIL Foundation Certified
Soft Skills:
Excellent communication and client-facing skills.
Strong documentation, reporting, and presentation abilities.

How you'll contribute

Performs initial and ongoing assessments of patient's condition.

Directs and supervises physical therapy assistants, students, and other support personnel. Assesses competence, delegates specific tasks, and establishes channels of communication.
Administers treatment involving application of physical agents, using equipment, moist packs, ultraviolet and infrared lamps, and ultrasound machines.
Evaluates, fits, and adjusts prosthetic and orthotic devices and recommend modifications.
Identifies and documents goals, anticipated progress and plans for reevaluation.
Qualifications and requirements:
Degree from an accredited Physical Therapy program
Physical Therapist License in State
Basic Life Support (BLS) obtain within 30 days of hire
Requires critical thinking skills, decisive judgment and the ability to work with minimal supervision.
Must be able to work in a stressful environment and take appropriate action.
