Job Duties for Caregivers/Home Health Aides:

The Wound Care LPN plans, coordinates, and provides total nursing care for residents and provides supervision and guidance to clinical staff members in relation to wound care.
Qualifications:
Education:
Graduate of accredited LPN School of Nursing.
Licenses/Certification:
Valid LPN license in the state employed.

Experience:

Essential Functions:
Assesses, plans, implements, and evaluates the ostomy care for patients on units.
Provides advanced clinical guidance to physicians and nursing staff for complex wound drainage, drainage confinement, skin protection, and the prevention of pressure necrosis (skin deterioration and skin loss)
Develops and conducts educational programs for staff.
Determines appropriate nursing interventions for identified patient problems, symptoms, and teaching needs.

Selects the stoma (opening) site on the patient prior to surgery, based on the patient's anatomy, activity level, and lifestyle.
