Become the Case Manager you always wanted to be
Work with interdisciplinary team, guiding treatment plans based on patient needs and preferences.
Coordinate with interdisciplinary team to establish tentative discharge plan and contingency plans.
Participate in planning for and the execution of patient discharge experience.
Monitor patient experience: quality/timeliness/service appropriateness/payors/expectations.
Facilitate team conferences weekly and coordinate all treatment plan modifications.
Complete case management addendums and all required documentation.
Maintain knowledge of regulations/standards, company policies/procedures, and department operations.
Review/analyze case management reports, including Key Care Indicators, and plan appropriate actions.

Make appropriate/timely referrals, including documentation to post discharge providers/physician
Ensure accuracy of discharge and payor-related information in the patient record
Participate in utilization review process: data collection, trend review, and resolution actions.
Participate in case management on-call schedule as needed.
Qualifications
License or Certification:
Must be qualified to independently complete an assessment within the scope of practice of his/her discipline (for example, RN, SW, OT, PT, ST, and Rehabilitation Counseling).
If licensure is required for one's discipline within the state, individual must hold an active license.
Must meet eligibility requirements for CCM or ACM™ certification upon entry into this position OR within two years of entry into the position.
CCM or ACM™ certification required OR must be obtained within two years of being placed in the Case Manager II position.
· Minimum Qualifications:
For Nursing, must possess minimum of an Associate Degree in Nursing, RN licensure with BSN preferred. A diploma is acceptable only in those states whose minimum requirement for licensure or certification is a diploma rather than an associate degree.
For all other eligible licensed or certified health care professionals, must possess a minimum of a bachelor's degree and graduate degree is preferred.
2 years of rehabilitation experience preferred.

Responsibilities
Assists patients in the process of navigating post-acute care with an overall goal of creating a positive impact on patient outcomes and referral source satisfaction. Integrates evidence-based clinical guidelines, preventative guidelines, protocols, and other metrics in the development of transition plans that are patient-centered, promoting quality and efficiency in the delivery of post-acute care. Represents the area branches in strategic relationships with health systems, hospitals, inpatient facilities, physicians and physician groups, and executive level opportunities.
Qualifications
Education and experience, essential
Must be a graduate of an approved school of nursing or therapy. RN Registered Nurse, LN Licensed Vocational Nurse or PT Physical Therapist.
Must be licensed in the state of Texas
Must have two years' demonstrated field experience.
Must have demonstrated experience and understand federal, state, and local laws and regulatory guidelines governing the operations of Medicare certified home health and hospice.
Must have basic demonstrated technology skills, including operation of a mobile device.
Education and experience, preferred
A registered nurse or licensed vocational nurse is preferred.
Three years of field experience is preferred.
Previous experience in home health or healthcare sales is preferred.
Requirements
Must possess a valid state driver license
Must maintain automobile liability insurance as required by law
Must maintain dependable transportation in good working condition

Job Summary and Responsibilities As a Rehab Tech, you will assist licensed therapists in providing therapeutic care and support to patients undergoing rehabilitation.
Every day you will prepare treatment areas, help patients with exercises and mobility, manage equipment, and maintain a safe and organized clinical environment as delegated.
To be successful, you will demonstrate strong interpersonal skills, attention to detail, and a proactive approach to supporting patient progress and therapist needs within the rehabilitation setting.
Patient Assistance: assists the therapist in the treatment of patients; transfer, transport and assist patients to therapies according to patient schedules; implement the plan of care in a thorough and complete manner under the direction of the therapist including patient preparation, gait training, functional training, exercise therapy, and ADL functions.
Cleaning: keep all treatment areas and equipment clean and orderly; perform routine housekeeping as required; maintain all cleaning and temperature logs.
Clerical: record clearly and completely all therapy treatments and time with patients on the appropriate forms; manage transmission and receipt of pertinent medical records; perform light clerical duties as required.
Communication: encourage, educate and assist patients to reach their maximum functional level; ensure confidentiality of all patient information; communicate in a respectful and courteous manner.
Safety: Demonstrate proper body mechanics when performing tasks and seek assistance when needed; utilize universal precautions when appropriate.
Work Behavior: be responsible, reliable and honest in all patient care and work ethics; demonstrate flexibility in job tasks; adhere to all facility and department policies and procedures.
