In this RN Field Clinical Care Manager role, you will be an essential element of an Integrated Care Model by relaying the pertinent information about the member needs and advocating for the best possible care available, and ensuring they have the right services to meet their needs.
There will be travel expectations throughout central upstate NY region.
If you reside in NY, you will have the flexibility to work remotely and in the office in this hybrid role* as you take on some tough challenges.
Primary Responsibilities:
Assess, plan and implement care strategies that are individualized by patient and directed toward the most appropriate, lease restrictive level of care
Perform the NYS UAS Assessment in the member's home at least twice per year and as needed
Identify and initiate referrals for social service programs, including financial, psychosocial, community and state supportive services
Manage the care plan throughout the continuum of care as a single point of contact
Communicate with all stakeholders the required health-related information to ensure quality coordinated care and services are provided expeditiously to all members
Advocate for patients and families as needed to ensure the patient's needs and choices are fully represented and supported by the health care team

Required Qualifications:
Current, unrestricted RN license for the state of New York
2+ years of relevant clinical work experience
1+ years of experience of community case management experience coordinating care for individuals with complex needs
Experience in long-term care, home health, hospice, public health or assisted living
Proficiency with MS Word, Excel and Outlook
New York state issued ID or ability to obtain one prior to hire
Reside in New York state
Ability to travel in assigned region to visit Medicaid members in their homes and/or other settings, including community centers, hospitals or providers' offices
Preferred Qualifications:
Behavioral health or clinical degree
Experience with electronic charting
Experience with arranging community resources
Field based work experience
Proficient in use of UASNY
Proven background in managing populations with complex medical or behavioral needs

Provide administrative and customer service support to internal colleagues, consumers, training centers, and instructors.
Maintain current knowledge of regulatory requirements; assure excellence and compliance at all levels of program service, both verbally and in writing.
Support the Training Center Coordinator with instructor renewal processes, new training center and instructor acquisition, state licensing processes, etc.
Assist existing training centers with orders of training materials and class forms; resolve problems through research and communication.
Provide general feedback from customers/instructors/training centers about program administration, service, and NSC guidelines to colleagues.
Work cooperatively with program staff to develop and maintain the current manual of rules and procedures for training centers and instructors.
Provide project support to management as regards marketing, development, and revision of forms/procedures, correspondence, report writing, data analytics, trends, etc.

B.S. Civil/Environmental Engineering with 12+ years of experience desired; demonstrated expertise accepted. * Technical expertise in water and wastewater linear infrastructure design with specific experience in designing large diameter water transmission mains, sewage force mains and gravity systems, CSO, and/or stormwater conveyance systems. * Technical expertise in pumping stations and water storage design experience is a plus. * Strong personnel development skills with experience mentoring people in engineering, and design positions. * Strong communication skills with ability to convey complex technical knowledge to clients, public and technical societies. * Trenchless pipe installation and rehabilitation technologies experience * Professional certification or licensure in the Northeast, MidAtlantic and/or MidWest Regions and ability to obtain multiple licenses through reciprocity * Client facing with strong existing client relationships within the water/wastewater infrastructure and/or resiliency field a plus. This role will require a founder's mindset as we grow Toast's presence in this new vertical with a new offering: the Toast Retail platform.
Toast offers an all-in-one platform that helps restaurateurs and retailers operate their business, increase sales, engage guests, and keep employees happy. As a District Manager, you are pivotal to the growth of the Toast brand in your district. You will report into a Director of Retail Sales and will hire, build, and coach a team of Retail Territory Account Executives who are working everyday to transform the way grocery stores, bottle shops, and convenience stores operate. You'll work closely with your sales team to understand the Retail operators' unique needs, and you'll work closely with the product team to build solutions that help Retailers thrive.

A day in the life (Responsibilities)
Hire, develop, motivate, and manage a quota-achieving sales team
Coach your team on a full cycle sales process. This includes coaching on how to conduct efficient discovery meetings, live demos, and craft a solution proposal that best meets the prospective customer's needs
Spend time coaching and empowering your team through field visits that include walk-ins, prospecting, trade shows, and local business partnerships to help exceed quotas
