MSW Medical Master Social Worker Ocala, Florida The MSW, Medical Social Worker, receives referrals for individuals from at-risk populations from interdisciplinary team members. The Medical Social Worker ensures patient-centered Care Coordination through the Continuum of Care. The Social Worker is assigned to a Community Living Center (CLC) at the Lake City VA Medical Center within the North Florida/South Georgia Veterans Health System (NF/SGVHS) and is aligned under Social Work Service. This position can also float and cover other areas, including but not limited to; medical surgical unit, PACT, and Emergency Department. About the Role IPS is seeking an experienced Licensed Clinical Social Worker (LCSW) or Licensed Professional Counselor (LPC) to join our inpatient team in Ocala, FL. This role offers the opportunity to work directly with patients in acute medical settings, supporting trauma, neurology, general medicine, and rehabilitation units. You will provide timely evaluations, brief therapy, discharge planning support, and coordination with hospital teams to help improve patient outcomes and reduce length of stay. The LCSW will serve as part of the care team, collaborating, consulting, and referring, to coordinate all treatment needs.
