Roles and Responsibilities
* Perform in-home and telehealth care management visits to assess and determine social and behavioral status
* Work closely with Care Team to ensure collaboration and optimal patient outcomes
* Assess social determinants of health needs and develop a plan for addressing them
* Identify, vet, and build relationships with local Community-Based Organizations
* Educate patients on appropriate resources, assist with referral completion, and follow up for closure outcomes
* Serve as subject matter expert on social determinants for other members of the Care Team
* Complete behavioral, environmental, and social support assessments
* Deliver individual, family and group education on living with chronic illness
* Engage family and social support groups in the education and care of patients
* Assess patients and refer to behavioral health specialists if diagnosis and treatment needed
* Help patients to understand, accept and follow medical and lifestyle recommendations
* Review and document patient updates and progress in care management platform
Position Requirements
* This position involves telephonic visits with some car travel to patients' homes
* Basic Life Support (BLS) certification is required in this role. The company will support your certification completion through onboarding.
* Currently licensed as a LCSW or LMSW in the posted state
* Master's degree in social work and passed ASWB masters or clinical exam
* Rare domestic travel may be required to Brentwood, TN
* Self-starter with the ability to work independently with minimal supervision

POSITION SUMMARY:
Holt International finds and supports permanent, loving families for children who are orphaned, abandoned or at serious risk of separation from their family. The master's level socialworker represents Holt in their community and ensures successful adjustment of child and family by conducting an assessment of the family, and providing appropriate services throughout the adoption process. The master's level socialworker works cooperatively with families, the Branch Office and Adoption Teams headquartered in Eugene, Oregon. Note: The Adoption SocialWorker is primarily a home-based position. The Branch SocialWorker position is required to travel (most often by car) to different locations within the assigned area to complete the essential functions of the position.
ESSENTIAL JOB FUNCTIONS (to be completed within timeframes outlined by the Branch Director):
1. Determines a family's ability and capacity to parent an adopted child by conducting pre-home study meetings as requested, reviewing and assessing agency-requested information, evaluating the family's knowledge and perceptions of adoption, teaching adoption education courses as needed, and conducting psycho-social assessments of family members.
2. Contributes to the delivery of high-quality adoption services by producing adoption home studies and post placement reports that comply with provided guidelines and meet current agency, state, country and USCIS requirements.
3. Provides support and case management of all assigned families by maintaining quarterly contacts throughout process, communicating information to the family, referring family to appropriate community support systems when necessary and documenting all correspondence in Holt's database.
4. Prepares family for travel and assists with post placement adjustment by providing training and identifying resources available to help meet the child's needs and aid in the child's adjustment to the family. Conduct home visits as required by state law and country program guidelines, assist in early identification of problematic issues, and make a determination regarding the stability and appropriateness of the placement.
5. Assures child's physical and emotional well-being and eases family's adjustment by providing counseling to the child and family and/or helping arrange for counseling with a clinician outside the agency if needed. In case of an adoption disruption or dissolution, maintain relationship with family to ensure transition plan is developed that meets the child's needs. Assist child and family with transition of child to new placement.
6. Contributes to public knowledge about Holt and supports Holt programs by participating in activities arranged by Holt including picnics or parent groups, representing Holt at adoption meetings, coalitions and organizations, and performing other outreach activities as appropriate.
7. Maintains professional knowledge by attending relevant seminars and course work, keeping informed about State regulations, legislation, adoption issues and child-related issues overseas; maintaining license, if required.
8. Contributes to a positive, productive work environment by meeting attendance and punctuality guidelines and pre-arranging time off with appropriate notice; treating all people with dignity and respect; and exhibiting good listening and comprehension skills including giving and welcoming feedback.

Luther Park Community is looking for a part-time socialworker to assist the Director of Social Services at the Trinity Center, a 120-bed skilled nursing facility which specializes in Alzheimer's care.Job Summary:
The part-time socialworker is responsible for assisting in meeting and assisting residents in attaining or maintaining their highest practicable well-being. This includes identifying the need for medically-related social services and ensuring that these services are provided in accordance with State and Federal regulations.
Essential Functions:
Complete or delegate the completion of the social services component of the comprehensive assessment and contribute to and/or direct/delegate contribution of social services goals and approaches to the comprehensive care plan. These goals and interventions will be individualized to match the skills, abilities, and interests/preferences of each resident in compliance with Federal and State regulations, to include identifying and promoting individualized, non-pharmacological approaches to care that meet the mental and psychosocial needs of each resident.
Facilitate residents' safe transition back into the community through interdisciplinary discharge planning and arrangement of community based services and follow-up care. Assist residents and their representatives in locating and accessing financial, legal, and other community resources.
Coordinate implementation and oversight of procedures to ensure social services actions and interactions are adequately documented in each resident's medical record, and that legal, ethical, and professional standards of social work practice are being upheld in written recordings.
Assist the Social Services Director in overseeing the establishment of departmental QA procedures and modification of those procedures where appropriate.
Assist in the process of Advance Care Planning for each resident upon admission and make sure any Advance Directives are reviewed with the resident/resident representative on a regular basis. Ensure staff members are made aware of the resident's code status and end-of-life wishes and assist with informing and educating residents and their representatives about health care options and ramifications.
Participate in facility policy development in order to positively impact the quality of care delivered to residents.
Ensures that staff members are knowledgeable about Resident's Rights and encourages staff to maintain and enhance each resident's dignity in recognition of each resident's individuality. Advocate for residents and assist them in assertion of their rights within the facility. Report any allegations or suspected abuse, neglect or exploitation to the Director/Administrator and appropriate State agency and assist in a thorough investigation into the allegation.
Ensure residents who display mental illness or psychosocial difficulties such as coping with grief and loss, will have access to appropriate treatment and resources.
Work alongside other departments to ensure the facility is in compliance with Trauma Informed Care. Ensure all residents are assessed for trauma upon admission and on a regular basis and that trauma and corresponding interventions are reflected on the care plan.
Assist with the grievance process and educate staff, as well as residents and families of the grievance process.
Additional Assigned Tasks
· Make arrangements for residents to obtain items such as clothing and personal care supplies.
· Act as a resource for staff participating in behavioral interventions with residents.
· Participate in Resident and/or Family Council as needed or requested.
· Assist with or arrange for, a resident's communication of needs through the resident's primary method of communication or in a language that the resident understands.
· Treat all residents with dignity and respect. Promote and protect all residents' rights.
· Establish a culture of compliance by adhering to all facility policies and procedures.

The Holden Comprehensive Cancer Center at the University of Iowa is seeking a SocialWorker III who will be an integral member of the multidisciplinary team, the SocialWorker III will provide direct social work services including assessment, treatment, interventions, and recommendations to facilitate movement through the continuum of care to Adolescent and Young Adult (AYA) oncology patients as well as their families on both inpatient and outpatient settings. The AYA program services cancer patients age 13-39 old and their families.
The Holden Comprehensive Cancer Center AYA SocialWorker will plan, develop, implement, and evaluate comprehensive social work services for adolescent and young adult patients with complex and/or specialized needs. This role will also participate in and provide the social work perspective to interdisciplinary activities, as well as provide social work treatment, individually or in groups, to patients and clients and their families with complex emotional or social problems.
This position will report to AYA Program Manager within the Holden Comprehensive Cancer Center, with functional oversight by the AYA Medical Director.
This is a two-year specified term position and may be eligible for extension based on funding.
Duties to include:
Provide high quality patient care for Adolescent and Young Adult cancer patients ages 13-39 with complex or specialized needs including evaluation, assessment, and case work appropriate for patient's condition and age.
Perform transition planning activities and psychosocial assessment at onset and throughout the patient's hospitalization utilizing clinical skills to create and update personalized transitional care plans.
Prepare psycho-social histories, participate in clinical staff diagnostic and treatment staffing, interpret patients' and clients' and their families' psycho-social functioning and needs, and recommend and plan social treatment methods to meet those needs.
Plan for and provide consultation to medical or clinical service staff and community social agencies regarding the handling of medical-social or other social functioning problems of individual of groups of patients and clients and their families.
Participates in the development of goals for the AYA Program.
May represent AYA in monthly Teen Cancer America webinars and other national committees.
Attend meetings and participate on committees and support groups as appropriate; serves as a member of the AYA Core team and AYA Steering Committee.
Assists patients in completing AYA Enrollment form and documents relevant information.
Maintains EPIC documentation and metrics of all AYA enrolled patients.
Shares pertinent information with AYA Nurse Coordinator, AYA Patient and Family Life Specialists and other team members on a continual basis regarding AYA enrolled patients.
Provide comprehensive survivorship support for AYA patients, including education, care planning, and resources to help navigate life after treatment and promote long-term well-being.
Participate in Cancer Center initiatives or other projects according to departmental and organizational monitors.
Function as a liaison between the clinic and community-based resources on an as needed basis.
Participate in the development, strengthening and delivery of needed health and social services.
Advocate for the unique needs of AYA patients, families, and staff.
Establish new patient programming and support that specifically targets the adolescent and young adult oncology population and is based on evidence-based standards.
Write case reports covering on-going social services for inclusion in the patients' or clients' clinical records, maintain statistical records of social service activity.
Attend and participate in courses, meetings, conferences and workshops in attainment of new or advanced social work practice skills.

Required Qualifications
