Responsibilities
* Assists the patient and family with health related financial, social and emotional concerns and assists with effective coping skills.
Provides social work services in accordance with the plan of care.
Ensures that changes in the patient's clinical status are communicated to other healthcare team members and the care center office staff in a timely manner.
Educates the patient and family members and other team members on medicalsocial work interventions and treatment plan.
Performs care planning, transfer and discharge functions.
Identifies risks or threats to the patient's health in the patient's environment and promptly implements appropriate interventions to reduce risks. Communicates identification of risks/ threats to the patient to the physician and other members of the healthcare team to coordinate care effectively.
Other duties as assigned.
Qualifications
* Holds a masters or doctorate degree from a school of social work, accredited by the Council on Social Work Education
* Has one year of social work experience in a health care setting.
* Meets state and federal requirements for this position.
* Current CPR certification.

Job Description
The MedicalSocialWorker is responsible for the implementation of standards of care for medicalsocial work services provided by the agency.
Identifies and assesses the social, emotional, environmental, financial and support factors underlying patient needs
Assesses the psychosocial status of patient's related to the patient's illness and environment and communicates finding to RN Case Manager
Communicates patient status regarding social and emotional factors to a team of clinicians
Carries out social evaluations and plans intervention based on evaluation findings
Counsels patients on how to overcome their condition and avoid dependencies as they recuperate from their illness
Provides crisis intervention, emotional support and resource information when necessary
Maintains and updates clinical records on all patients referred to social work and prepares clinical and progress notes to share with the clinical team
Educates patients and families on, and assists in, preparation of advanced directives
Serves as the liaison between patients and families/caregivers and community social service agencies
Participates in the development of the individualized plan of care and attends case conferences
Partners with public and private health insurers to determine the patient's benefits and advocate for the patient
Participates discharge planning
Performs other duties as delegated by the Clinical Director
Basic Qualifications
Master's degree insocial work from accredited school of social work
At least two years' experience in a health care setting
Licensed clinical socialworker with the Indiana Professional Licensing Agency preferred
Ability to network with community agencies and utilize community resources
Ability to make evaluations and decisions based on measurable or verifiable criteria
Ability to communicate effectively, both verbally and written
Current holder of Indiana driver's license and automobile liability insurance

Overview

Incorporates patient care goals established in the plan of care, as evidenced by documentation in clinical note.
Performs appropriate skilled services/interventions in accordance with accepted standards of practice and certified by the patient's physician.
Counsels, instructs, and includes the patient and family in following the Plan of Care and meeting social work-related needs.
What is Required?
Master's Degree or Doctoral Degree inSocial Work from a school of Social Work accredited by the Council of Social Work Education
1+ year of social work experience in a healthcare setting.
Current State License as a SocialWorker
Excellent verbal and written communication skills
Social work skills as defined as general social work standards
50% travel required
Sit, stand, bend, lift and move intermittently and be able to lift 50-100 lbs.
Valid driver's license and insurance and reliable transportation to perform job tasks
