Responsibilities
Investigate, assess and plan interventions to help patients cope with social, emotional, economic and environmental problems.
Interview patients, assess priorities and document case activity.
Consult with team managers, statutory and voluntary agencies and patient's relatives to ensure compliance with patient's treatment plan.
Assist clients/families with obtaining community assistance by referral to proper resources.
Develop and maintain working relationships with community health, welfare and social agencies and seek creative means to assist the client with his/her needs.
Assure resident safety by identifying safety hazards and emergency situations and initiates corrective action as appropriate.

Summary
The social worker provides a variety of services to pupils, parents, and staff. A particular emphasis is placed on developing social skills, conflict resolution skills, and strategies for dealing with a myriad of challenges and opportunities.
Essential Duties and Responsibilities
Serves on the multidisciplinary team and participates in staffing meetings
Conducts and writes comprehensive social assessments of individual students
Develops and uses a variety of intervention methods in individual, group, family counseling, and crisis management
Works with parents in helping them to understand their child's education, social, and intellectual needs
Helps parents alleviate adverse conditions that lead to impaired functioning in their children
Interprets data, collaborates effectively when writing individual educational plans, and implements intervention strategies
Develops and locates resources and services for students and their parents
Performs casework service with individual students
Consults and collaborates with other school personnel in gathering and giving information on a case and in establishing and planning the respective roles in the modification of the student's behavior

Job Description
I. JOB SUMMARY
The MSW Social Worker provides psychosocial services, including patient/family counseling, crisis intervention during hospitalization, comprehensive discharge planning, coordination of community resources and assisting patients and families in the decision making process.
This is a part time, weekend position. The shifts will be 8am-4:30pm.
II. ESSENTIAL JOB RESPONSIBILITIES
Work in collaboration with Case Management, Social Services and other healthcare providers to evaluate the medical or physical conditions of the patients.
Evaluate needs of the patient, and counsel them on how to overcome their conditions and avoid dependencies as they recuperate from their illness.
Evaluate and suggest the appropriate type of in-home help required by a newly discharged patient as a means to help adjust back to normal life.
Provide Discharge Planning.
Work together with other members of the multidisciplinary treatment team to provide consultations.
Provide staff development and training on assessments, reassessments, care plan development, interventions, prioritizing needs, case conferencing and follow-up.
Conducts Psycho/Social problem assessments.
Performs other duties as needed or assigned.

The Social Worker is responsible for identifying and linking members with social and community resources, ensuring psychosocial assessment and related care coordination while supporting members ability to manage his/her chronic illness.

Assists members to effectively utilize available resources to meet their personal health needs and help them develop their own capabilities.
Provides guidance to members seeking alternative solutions to specific social, cultural or financial problems that impact their ability to manage their healthcare needs.
Facilitates and coordinates behavioral health resources as individual member needs are identified.
Establishes a relationship with member, family, physician(s), and other providers to determine use of benefits and community resources.
Acts as liaison and member advocate between the member/family, physician and facilities/agencies.
Evaluates members' ability to independently manage self and locate alternative resources when limitations are identified via standardized Social Work Psychosocial evaluation methods, processes and tools while maintaining accurate record of activities.
Minimum Requirements:
Requires a BS or MS in Social Work and a minimum of 3 years experience in case management and a minimum of 1 year experience working with aged or disabled populations; or any combination of education and experience which would provide an equivalent background.
Preferred skills, qualifications and experiences:
MS in Social Work (MSW) preferred.

Job Description
I. JOB SUMMARY
The MSW Social Worker provides psychosocial services, including patient/family counseling, crisis intervention during hospitalization, comprehensive discharge planning, coordination of community resources and assisting patients and families in the decision making process.
II. ESSENTIAL JOB RESPONSIBILITIES
a. Work in collaboration with Case Management, Social Services and other healthcare providers to evaluate the medical or physical conditions of the patients.
b. Evaluate needs of the patient, and counsel them on how to overcome their conditions and avoid dependencies as they recuperate from their illness.
c. Evaluate and suggest the appropriate type of in-home help required by a newly discharged patient as a means to help adjust back to normal life. Provide Discharge Planning.
d. Work together with other members of the multidisciplinary treatment team to provide consultations.
e. Provide staff development and training on assessments, reassessments, care plan development, interventions, prioritizing needs, case conferencing and follow-up.
f. Conducts Psycho/Social problem assessments.
g. Performs other duties as needed or assigned.
III. JOB QUALIFICATIONS
Required:
• Master's degree in Social Work
• Current Licensure in the State of Kansas
• Minimum two year's work experience in the mental health field
• Knowledge of third party payers including but not limited to various types of policy coverage
Preferred:
• A practicum in acute mental health
• Current Basic Life Support for Healthcare Providers (BLS)
