Responsibilities
The Case Worker is responsible for conducting interviews with clients to assess their needs and challenges, such as personal and family adjustments, financial difficulties, employment issues, housing concerns, and physical or mental impairments. They gather information on medical, psychological, and social factors affecting the client's situation and evaluate their capabilities. The Case Worker provides counseling and support to help clients develop plans to meet their needs, mobilize internal and external resources, and improve their social functioning. They monitor client progress, refer them to community resources, maintain accurate documentation, and ensure effective communication with supervisors and funding sources. Additionally, the Case Worker may assist with housing individuals and providing necessary supplies and support to help them maintain their housing stability.
This Position is Full time at 40 Hours a Week, Monday to Friday 8:30 pm to 5:30 pm. Pay For This Position is $23.50 Per Hour.
Qualifications
Education and/or Experience Required at Entry:
Associate Degree in Human Services or a related field
Three years of experience working with vulnerable individuals
Equivalent combination of education and experience will be considered
Candidates with lived experience are preferred and encouraged to apply
Licenses/Certificate/Clearances:
Valid N.Y.S. Drivers License
Registered and insured vehicle
N.Y.S. Child Abuse Registry clearance
Working Conditions and Environment/Physical Demands:
Mobility sufficient to drive to and participate in meetings at client's homes and/or community facilities

Rome Health is seeking a Licensed SocialWorker/Licensed Mental Health Counselor to work within our Community Recovery Center. The job duties for the SOCIALWORKER - LMSW, LCSW, or LMHC will include:
Providing individual, group and/or family sessions,
Providing psychotherapy, intake screenings and clinical assessments
Safety planning and providing treatment recommendations
Assisting patients with treatment planning
Utilizing evidenced based assessments and treatment approaches
Providing case management, linkage and referrals as appropriate
Experience/Education:
Knowledge of Mental Health disorders, ability to create effective safety plans, complete suicidal/homicidal assessments and the ability to utilize various therapeutic interventions.
The ability to identify individual strengths and weaknesses and provide patient centered care.
Knowledge of the DSM - 5 and the ability to substantiate criteria and provide basis for SUD diagnosis.
Knowledge of Professional Standards/ Canon of Ethics and OASAS 822 regulations
Knowledge of case management including availability of resources and referral processes.
Ability to know the family dynamic of the cases as needed.
LMSW, LCSW, OR LMHC required.
Your typical week would be Monday through Friday 8-4 with one of the shifts being 12-8, you will report to the Clinical Supervisor.
