Resident Aides will be responsible for ensuring the safety and security of all clients, staff, visitors, and property around the facility. This position reports to the Shift Supervisor. Barrier Free Living Apartments (BFLA) is a supportive housing program. POSITION OVERVIEW
Under the supervision of the Promesa Community Residence Assistant Director and Shift Supervisor, the Community Residence Aide (CRA) has the responsibility of providing residents with the highest quality care within a therapeutic environment. The CRA is responsible for a wide range of daily living and supportive tasks. This individual must demonstrate the knowledge and skills needed to provide appropriate care based on the assigned unit. This CRA will adhere to all policies, procedures and regulations set forth by New York State Office of Mental Health and Acacia Network to ensure resident safety and program compliance. The requirements listed below represent the knowledge, skill, and/or ability required. KEY ESSENTIAL FUNCTONS
Conduct intake assessment planning, service coordination, service provision, and discharge planning.
Provide assertiveness and self-advocacy training designed to promote residents' ability to make life changes and increase self-awareness.
Assist residents with developing daily living skills.
Ensure seamless coordination between the program and primary health and clinical care teams.
Assist in record keeping, monitoring, storage and supervision associated with the self-administration of medication.
Manage incidents as needed while remaining calm and in compliance with the organization's Incident Reporting Policy.
Assist with all mental health projects to meet the goals and objectives established by, and in accordance with, company policies, contracts and applicable laws and regulations.
Assist with skill development activities to help clients gain and utilize the skills necessary to undertake employment or pursue educational opportunity.
Assist in the development of social skills intended to diminish tendencies towards isolation and withdrawal.
Maintain and develop work-related knowledge and skills through continuous learning activities, attendance at in-services and workshops, industry seminars and conferences, speaker series, etc.…
Provide services to address the following areas: dressing, personal hygiene, grooming, selection and/or preparation of food, cleaning and washing clothes, maintenance of environment, budgeting and money management.
Ensure restorative services occur in a timely manner.
Ensure compliant documentation in charts and all other programmatic areas.
Other duties as assigned.
REQUIREMENTS
Associate Degree or higher required.
Significant desire to work with people struggling with a mental illness and to help them improve their lives.
Three (3) years of experience in Human Services or Behavioral Health preferred.
Positive attitude with excellent customer service skills.
Ability to work well within a team and to contribute positively.
Excellent documentation skills, ability to meet deadlines, and follow protocol.
Comfortable working in a culturally diverse environment.

Follow through with all assignments on a timely basis in accordance with chain of command when Health and Wellness Director is not on duty.
Report any unusual behavior of a resident to Wellness Leadership.
Learns, understands, and maintains residents' rights.
Follows established infection control and standard precaution policies, fire, and safety policies, and practices good body mechanics.
Attends training courses at community and maintains up to date monthly, quarterly, annual training through designated Learning Management System (LMS).
Attend all required all staff meetings.
Other duties as assigned, and which relate to the success of the community and the care, comfort, and happiness of our residents.
Requirements
EXPERIENCE, QUALIFICATIONS & SKILLS:
High school or equivalent.
STNA/CNA (Preferred).

About the Role:
We are seeking Advanced Care Providers (Nurse Practitioners and Physician Assistants) to join our GI team and collaborate closely with our GI Chief Medical Officer. Ideal candidates will demonstrate a commitment to providing high-quality patient care and bring an innovative, forward-thinking approach to their work. Candidates should possess strong GI experience as well as exposure with internal medicine knowledge. Experience in urgent care, primary care, or family medicine, with a comfort level in treating the adult population is highly preferred.
Essential Job Duties - GI Focus:
Patient Assessment and Diagnosis: Nurse Practitioners and Physician Assistants (NPs/PAs) will partner with CMO to perform comprehensive GI-focused health assessments, including detailed medical histories, physical examinations, and diagnostic evaluations relevant to gastrointestinal disorders. NPs/PAs will formulate differential diagnoses and develop appropriate treatment plans in collaboration with supervising physicians.
Treatment and Management: NPs/PAs will collaborate with GI CMO to provide specialty care services, including prescribing GI-specific medications, ordering and interpreting GI-related diagnostic tests (e.g., colonoscopies, endoscopies, liver function tests), and implementing therapeutic interventions. They will manage chronic GI conditions, monitor patients' progress, and adjust treatment plans to optimize digestive health outcomes and patient quality of life.
Health Promotion and Disease Prevention: NPs/PAs will educate patients and families about GI health promotion strategies, disease prevention, dietary and lifestyle modifications, and long-term management of chronic GI conditions. They will emphasize preventive care, including colorectal cancer screenings, hepatitis vaccinations, and liver health monitoring.
Collaborative Care: NPs/PAs will work closely with interdisciplinary healthcare teams, including gastroenterologists, hepatologists, primary care providers, dietitians, and nursing staff, to coordinate comprehensive patient care. They will consult with specialists, refer patients appropriately within the GI specialty or to external providers, and ensure continuity of care across inpatient and outpatient settings.
Clinical Care: NPs/PAs will be required to perform clinical skills relevant to gastroenterology, including but not limited to phlebotomy, IV insertion, EKGs, and the management of patients undergoing GI procedures. They will independently manage patient encounters for both acute and follow-up GI care as needed.
Documentation: NPs/PAs will maintain accurate and up-to-date electronic medical records, including detailed documentation of GI-related patient histories, physical findings, assessments, diagnoses, treatment plans, procedure reports, and follow-up notes. They will ensure compliance with legal, regulatory, and organizational documentation standards.
Ethical and Legal Compliance: NPs/PAs will adhere to ethical principles and professional standards of GI practice, maintaining confidentiality, integrity, and respect for patients' rights and dignity. They will comply with all legal and regulatory requirements governing advanced practice in gastroenterology, including licensure and scope of practice.
Compensation:
$170,000 - 190,000, based on years of experience

Responsibilities:
Ensures all residents are evaluated prior to residency as required by individual state regulations or with change of condition to ensure services are delivered. Presents evaluation to Director, Health and Wellness, Nurse, or Executive Director for approval.
Communicates with residents, family, and staff on an as needed basis to meet resident needs. Participates in care conferences with residents and family members to discuss resident needs and appropriate service delivery in association with service plan updates.
Responsible for screening, hiring, training, and supervising all Care Partner staff and activity staff in the Specified Care Area. This includes managing poor performance, coaching to improve skills, directing staff to provide activities that correspond to care plans and appropriate documentation and compliance with all human resource guidelines. Assures that all staff maintain appropriate annual training to keep up with state requirements.
Verifies all associate hours on time cards and weekly hours report. Completes care staff schedules to assure proper coverage to meet resident needs and according to budget.
Assigns weekly blood pressures and weights in rotating shifts.
Monitors staff in administration of medication, to include frequents audits of medication sheets.
Assists the Director, Health and Wellness in medication management, including administration of medications to residents, ordering medication from pharmacy and maintaining records as dictated in company policies and procedures manual. Assures that medications are properly stored, medication carts/cupboards are cleaned and medication room is organized according to standards.
Observes residents' health statuses, takes actions to address concerns within scope of practice, and reports all significant changes, reactions to medications and treatment, or significant incidences to Director, Health and Wellness in a timely manner. Reviews capabilities of resident who self-administer medications on a regular basis (refer to state regulations regarding medication assistance and documentation).
Conducts pre-move-in screening of prospective residents as needed and according to state requirements. Assures that all required documentation including service plans and nursing assessments is completed.
Performs ongoing assessments of resident physical and psycho-social needs and coordinates with nurse to assure quality proactive care as allowed by state regulation.
Communicates with families regarding residents' personal needs and achievements. Reminds families of need for scheduled physician, dental, or eye appointments.
Assists in organizing and developing service plan reviews as required, with appropriate resident care team members and resident families.
Observes residents, records significant reactions, and notifies Executive Director, Director, Health and Wellness, physician, and pharmacist (as applicable) of resident's condition and reaction to drugs, treatments, and significant incidents.

Responsible for oversight and compliance with the communities CQI (Continuous Quality Improvement) Program and takes steps to assure that services delivered are completed as designed. Audits medication administration by staff, documentation, resident weights, and reassessments to assure compliance.
Responsible for all local, state, and federal rules and regulations as they apply to services delivered in the Specified Care area. This involves staff selections, training and oversight, resident care and documentation, and compliance with HIPAA, OSHA, Blood Borne Pathogen Guidelines, and any other rule that applies to the work we do.
Ensures they and any employees reporting to them work in a safe manner and that unsafe actions are corrected. Compliance with Safety Committee Standards is required. Should workplace injuries occur, when required, the Department Heads and Directors assist the Executive Director with the investigation, return to work, and management of the injured worker communication with care provider and the management company.
