The Program Manager, Pharmacy Operations is a Clinical Pharmacist who provides comprehensive medication management of patients with Non-Tuberculous Mycobacteria (NTM) infection, Bronchiectasis, and Cystic Fibrosis (CF) in the Division of Pulmonary, Critical Care and Sleep Medicine outpatient practice. This part-time role is essential in optimizing therapeutic outcomes, minimizing drug-related problems, and ensuring safe and effective use of complex medication regimens for this unique patient population.
The Program Manager develops and implements policies and procedures to optimize workflow and ensure patient satisfaction; partners with the Directors of the programs to lead program initiatives and quality assessment and performance improvement activities; coordinates with multidisciplinary ancillary staff; maintains familiarity with local and federal regulations for advanced therapies for diseases, advances the research productivity of the team, and assists with all of the programs marketing and outreach pursuits.

Direct Patient Care Responsibilities
* In collaboration with physicians, clinical team members and program directors, coordinates all pharmacotherapy needs patients in the outpatient practice.
* Maintains contact with assigned caseload between visits, based on their particular needs. Initiates appropriate responses to issues and problems during these contacts.
* Communicates with the multidisciplinary team to optimize patient care. Interfaces with physicians, social workers, nurses, nurse practitioners, dietitians, and others to plan and execute specialized protocols.
* Informs physicians of patient status and needs and monitors clinical patient indicators to coordinate care with the patients physician.
* Provides patient and family education and excellent patient services to ensure satisfaction.
* Develop and maintain a high-level clinical pharmacy practice. Participate in multidisciplinary, bedside rounds. Pharmacotherapeutic, pharmacokinetic and pharmaco-economics consultation
* Patient and Caregiver medication counseling. Participate in educational initiatives
* Develops and implements clinical practice guidelines that will enhance pharmaceutical care and medication safety
* Clinical pharmacy intervention, adverse drug reaction, and medication error documentation and reporting
Education, Development and Research
* Promotes own professional growth and development in clinical and managerial role;
* Serves as a clinical expert and resource for the education of peers, staff members and other health professionals;

* Assists in the preparation, implementation, and evaluation of research protocols when applicable.

Position Summary:
We have an exciting opportunity to join our team as a Part-Time Main Site Dental Assistant - Brooklyn.
In this role, the successful candidate The Dental Assistant assists the dentist on all modalities of dentistry (operatory, oral surgeon, endodontics, periodontics, orthodontics, pediatric dentistry). Practice four-handed dentistry. The assistant also assumes the standards of care related to infection control; all OSHA guidelines & safety are upheld at the station they are assigned. The assistant exposes, develops and mounts radiographs properly, sets trays & prepares room before accepting or discharging a patient. They are expected to assure a patients comfort while waiting for the dentist. The assistant is responsible to sterilize hand pieces, transport instruments for sterilization, I.D. any inoperable equipment, light lab work, document patient records as dentist dictates information, & comply with all standards & policies & procedures of the department.
Job Responsibilities:
* Provide chairside assistance to the dentist, utilizing proper four handed dentistry procedures and observing universal precautions/protocols and sterile techniques
* Compliance with hospital, health center and departmental policies and procedures
* Assumes the standards of care related to infection control; all OSHA guidelines & safety are upheld at the station they are assigned.
* Takes, exposes, develops and mounts radiographs properly (when applicable).
* Maintain dental operatory in an organized and clean fashion. Assures that appropriate instruments and supplies are available
* Assures patients comfort while waiting for the dentist.
* Cleans instruments and prepares them for sterilization
* Sterilizes and/or disinfects instruments and hand pieces following guidelines of the Dental Department Infection Control Manual
* Documents patient records as dictated by dentist. Assists the dentist in filling out charts accurately and processing the patients forms and records
* Pours and trims patient study models in the dental laboratory. Complete laboratory forms accurately
* Demonstrates knowledge and skills necessary to provide care based on physical, psychosocial, educational, safety and age related criteria pertinent to the patients served in his/her assigned area
Minimum Qualifications:
To qualify you must have a High School Diploma
1 year experience of chairside dental assisting
Current shift: M&F 9am-5pm, which will change once the site becomes fully operational. The schedule will then include: Tuesday, Wednesday, and Thursday, 9am-7pm and Saturdays: 8:30am-4pm.

In this role, the successful candidate Responsible for the independent operation of sonographic equipment, and for performing diagnostic examinations using sonography. The Ultrasound Technologist may be responsible depending on work location for daily operations of the sonographic laboratory, patient schedule, equipment maintenance, reports of equipment failures, and quality assessment (QA). The sonographer maintains a High standard of medical ethics at all times and is self-motivated to increase their level of understanding & knowledge of the field, disease(s), and new procedures as they evolve.
Job Responsibilities:
* Performs other duties as assigned
* Operates Ultrasound equipment to take diagnostic images of specific body areas/parts as ordered. Produces diagnostic images for interpretation by physician. Demonstrates a thorough knowledge of Ultrasound equipment.
* Prepares patient using proper positioning andor immobilizing patients and giving them proper instructions regarding procedure. Quality review of the technologist s images fall within guidelines of departmental SOP. Technologist is observed to give clear and concise instruction to the patient before beginning an exam.
* Accurately utilizes the PACS system to make images available to the physician for interpretation. Technologist creates a minimum number of exceptions.
* Accurately utilizes systems. Enters proper exam coding. Arrives and completes exams in a timely fashion.
* Assists in the care and maintenance of facilities, equipment, and supplies. Reports any equipment problems or malfunctions to the supervisor in a timely fashion.
* Instructs and supervises ultrasound students in a clinical practicum. Works with the ultrasound student to perform exams as well as performing individual image evaluation.
Minimum Qualifications:
To qualify you must have a Technical Diploma Graduate of an accredited two-year Ultrasound Program required. Registered Diagnostic Sonographer OR Registered Vascular Technologist required. Ability to effectively operate sonographic equipment required. Ability to evaluate sonograms in order to acquire appropriate diagnostic information required. Ability to follow established departmental procedures required. Ability to work efficiently and cope with emergency situations required. Emotional and physical health sufficient to meet the demands of the position required. Strength sufficient to: lift some patients, move heavy equipment on wheels (up to approx. 500 lbs), and to move patients in wheelchairs and stretchers required. Ability to maintain prolonged arm positions necessary for scanning required.
Preferred Qualifications:
Associate's degree In a related field. 3 - 5 Years Minimum 3 years of Ultrasound experience. 3 - 5 Years OB/GYN, small parts and/or vascular experience.

Ability to perform basic arithmetic calculations for collating and duplicating of radiology records.
Ability to operate computer system and use numerous functions for a variety of tasks.
Good interpersonal skills, including poise, tact and diplomacy, required to handle sensitive situations with internal and external customers.
Analytical skills necessary to maintain filing system, collate radiology records in preparation for reading and so forth.
Must adhere to all HIPPA requirements.

* Organizes daily schedule to insure that patient meals are assembled and distributed appropriately.
* Accept daily call outs and determine need to replace staff or reassign existing staff to fill needed positions
* Plan employee schedule when assigned
Recommends hiring, orients, trains and evaluates new Nutritional Aides and Nutritional Clerks:
* Participates in all aspects of training Nutritional Aides and continually documents observations of staff achieving milestones in reaching performance goals.
* Ensures Nutritional Aides' job completion and productivity as per the collective bargaining agreement.
* Participates in writing performance appraisals for Nutritional Aides, which are objective and meaningful; using appraisal system as an opportunity to review and teach; provides on-the-spot counseling and corrections to employees as appropriate
* Writes performance appraisals for the aides and uses the process as an opportunity to review, evaluate and teach.
* Communicates and Documents all counseling with employees to ensure all staff members are informed of their work performance.
* Follows hospital policy of progress discipline for counseling staff when necessary.
Responsible for day to day supervision of Nutritional Service Department:
* Documents temperatures on quality control logs and takes corrective action when needed
* Assures all catering orders are completed and delivered
* Reports problems to appropriate managers
* Involved in Performance Improvement studies as needed.
* Reports needed repairs to Engineering and follows up as needed with appropriate individual or department.
* Able to assist with menu system in cases where it becomes necessary due to staffing or computer issues.
Minimum Qualifications:
To qualify you must have a High school diploma or equivalency.
Food Service Manager Certificate (120 hours) or equivalent
Five years experience in patient food service preferred
Experience in diet office systems and methods preferred
Able to stand for 60-70% of workday

* Performs specific procedures, including but not necessarily limited to: drawing blood specimens for testing; blood counts; urinalysis; pap smears; bacteriologic smears; gram stain of sputum and other culturable material(s); vital capacity; administering oxygen; incisions and drainage; wound care and debridement; nastogastric and nasotracheal intubation; gastric analysis; lumbar punctures; routine suturing of lacerations; skin biopsies; placing indwelling arterial catheters and making blood gas determinations; applying and removing casts and traction apparatus; starting I.V. solutions, administering I.V. medications and other therapeutic procedures for which he or she has received specific supervised training and evaluation.
* Orders and interprets appropriate laboratory tests, x-ray, EKGs diagnostic procedures and writes prescriptions according to protocols established by the service and in compliance with the New York State rules and Regulations governing the performance of Physician Assistants.

Position Summary:
We have an exciting opportunity to join our team as a ED Phlebotomist - Part Time, Days - Clinical Labs-ED Medical Asst - Part-Time - Monday - Friday with.
In this role, the successful candidate Under supervision performs routine phlebotomy and laboratory central receiving duties. Prepares specimens for send-outs to Reference Laboratories and results reports into the laboratory computer system.
Job Responsibilities:
* Phlebotomy/Capillary Collection
* Properly greets patient.
* Properly identifies patient.
* Prepares patient in an appropriate position.
* Assembles appropriate material. (Tubes, gloves, etc.)
* Selects appropriate tubes for tests ordered
* Follows IC procedures (Gloves, Hand washing)
* Follows order of draw in multiple specimen collection.
* Performs proper site sterilization techniques (preparation)
* Properly labels and documents tubes and label sets.
* Knows appropriate site selection and exclusions (i.e. scarring, mastectomy)
* Follows proper procedure for Blood Culture Collection.
* Follows policies and procedures for sharp's disposals.
* Normal Draw technique
* Finger stick technique
* Heel stick technique
* Butterfly technique
* Keeps a clean, neat and professional working appearance
* Keeps a clean, neat and professional working phlebotomy tray.
* Patient left in a safe manner, bed rails up.
* Performs EKGs correctly.
* Customer Service and Patient Care
* Explains procedure and reassures patient.
* Protects privacy of patient. (Closing curtains etc.)
* Protects confidentiality of patient.
* Refers question of patient to appropriate person. (i.e. Nursing, Supervisors)
* Identifies any additional need of patient and acts accordingly.
* Reports to supervisor or nursing any unusual condition of patient and document on label set and in computer
* Prioritizing draws
* Identifies his/herself, wears hospital ID badge and clarifies his/her role to patient in an understandable and respectful fashion.
* May be required to draw outpatients at off-site locations.
3. Other Functions and Performance Expectations
* Maintains a consistent pattern of attendance
* Reports to assigned area on time for scheduled shift
* Adheres to hospital/department dress code. Arrives to work appropriately dressed, well groomed and neat.
* Completes all hospital requirements at designated times (i.e. annual assessment, educational, MedNetTra)
* Conforms to policies regarding confidentiality of information.
* Follows call-in procedure when sick, late or absent.
4. Teamwork/Collaboration
* Communicates in a fashion that respects the individuality and dignity of each person. Promotes dialogue that is hones and non-confrontational.
* When there are disagreements regarding issues he/she will make a personal attempt to resolve the problem in a collaborative manner that promotes a successful encounter.
* Demonstrates through his/her actions that the patient and/or departmental goals are a priority.

In the absense/or to support the Data Entry-Accessioning staff (as needed):
1. Central Receiving (Specimen Processing)
* Understands general computer operation for this area.
* Prepares document in computer or deletion, addition or changes in order.
* Properly documents all pertinent patient information.
* Verifies and processes samples in an organized and timely fashion.
* Delivers Separately all STAT requests to sections.
* Notes in computer any specimen integrity issues.
* Relay to supervisor any conflict in specimen identification or labeling.
* Handles equipment in accordance to laboratory policy.
* Completes work or explains to coverage appropriate stage of processing and outstanding issues.
* Identifies need for written (faxed) requests for patients results from non-staff physicians
* Proper procedure for picking up samples from Ambulatory Surgery area
* Knowledge of computer downtime procedure.
* Ability to handle light stock.
* Responsible to make sure adequate supplies are in the area.
* Works collaboratively with other staff to maintain cleanliness of work environment and equipment. (ie. Carts, EKG machine).
* Assists with prevention of transmission of infection by adhering to Standard Precautions as evidenced by effective handwashing techniques.
* Recognizes safety hazards and acts promptly and appropriately.
2. Reference Laboratory Sendouts
* Organize work to complete in a timely fashion.
* Understands process of lab necessary and specimen integrity.
* Investigates unusual tests for proper collection and transport.
* Identifies appropriate Reference Lab for analysis.
* Notifies supervisor of unusual issues or specimens.
* Directs to Pathologist requests requiring approval.
* Informs coverage of appropriate stage of processing and any outstanding issues.
* Able to transmit electronic orders
* Knowledge of proper specimen submission to reference lab.
* Knowledge of manual billing process for "Miscellaneous" reference tests.
3. Resulting and Reporting
* Properly faxes reports to Physician's offices
* Knows complete process for manual and electronic reporting.
* Handles phone conversation in polite and complete manner
* Notifies Supervisor of any unusual report, or phone request.
* Distributes manual reports to appropriate location.
* Assists in processing or outpatient area when necessary.
Minimum Qualifications:
To qualify you must have a High School diploma
Phlebotomy Certificate Preferred or other Equivalent documentation and/or 5 years experience
General Office skills (Typing, Filing, Phone)
General Computer skills
General Organizational Skills
Knowledge of OSHA Regulations
Knowledge of Standard precautions
Must possess a valid drivers license
