Ensure prior diagnostic testing clearance is obtained, including X-rays and/or proper bloodwork/GFR and required documentation including operative notes are scanned into patient's chart. Trained in MRI safety level II, and BLS as required by ACR. Ensure that incoming faxes are delivered and/or acted on immediately. Retrieves and responds to voice mail messages in a timely manner. Other Maintains patient confidentiality; complies with HIPAA and compliance guidelines established by the practice Maintains detailed knowledge of practice management and other computer software as it relates to job functions. This School Based Health Clinician is a full time salaried position that follows a school based calendar, mirroring the school schedule of 195 days per year. **ROLE AND RESPONSIBILITIES** _CLINICAL_ + Completes initial behavioral health evaluations; individual, group and family psychotherapy with children and/or adults in an outpatient clinic. + Will be expected to run at least five (5) groups for each week of direct clinical work. + Practices as part of an interdisciplinary, inter-professional team, accepting "warm hand offs" from primary care providers and other members of the health care team, as well as reaching in to the patient population to identify patients potentially in need of Behavioral Health services as flagged by the BH clinical dashboard. + Consults with providers within and outside the agency regarding the treatment plan and progress of the patients. + Coordinates as needed with psychiatric consultants for diagnostic and consultative needs; refers as necessary for medication management. + Attends clinical Team Meetings and Component Meetings as assigned. + Meets productivity standards as outlined annually during employees performance appraisal review with a focus on quality clinical care. + Encourages clients to express their feelings and discuss what is happening in their lives, helping them to develop insight into themselves or their relationships. + Perform crisis interventions with clients. + Assess for immediate safety concerns, including: high risk patients/clients who may be at risk for homicide, suicide and domestic violence. + Guide clients in the development of skills or strategies for dealing with identified goals. + Develop and implement treatment plans based on clinical experience and knowledge according to state regulatory and joint commission standards. + Evaluate the effectiveness of ongoing treatment on clients' progress in resolving identified problems and moving towards defined objectives. + Modify treatment activities or approaches, as needed, to comply with changes in clients' status. + Discuss with individual patients their plans for life after leaving therapy. + Work with and Support family members to assist them in understanding, dealing with, or supporting clients or patients. + Maintain awareness within discipline about new developments in the field + Gather information about community mental health needs or resources that could be used in conjunction with therapy. + Licensed clinicians may supervise student interns or other staff working toward obtaining licensure. Unlicensed clinicians are actively working towards obtaining licensure. _ADMINISTRATIVE_ + Completes all documentation required by state regulations, Joint Commission standards, and CHC Behavioral Health department by laws in a timely manner, included by not limited to intake, prior authorization request, progress notes, treatment plan documentation, and discharge. + Completes and locks all treatment notes no later than the end of the next business day. + Completes reports as required for outside agencies such as DCF, Family Relations, probation, and others with appropriate releases signed by clients. + Participates as a member of agency wide committees as assigned. **QUALIFICATIONS** + Licensure / Certification / Registration: LCSW, LMFT, LPC, or Licensed as Clinical Psychologist in State of CT. preferred, or may be license eligible with required current Associate license (MLSW, PC-A, MFT-A) with ability to obtain full clinical licensure within 3 years of hire. Job Description Reports To The Med Tech will report to the Director of Health & Wellness. The Med Tech is a non-exempt, hourly position. Job Overview Responsible for providing quality care and services to all resident and their families. Provide assistance of medication administration for resident according to their care plan and physician's orders. Schedule: Full-Time; 7am - 3pm and 3pm-11pm Salary Range: $23.00 hourly Responsibilities and Duties Administer and/or assist with self-administration of prescribed medication to resident and maintain related medical records under the supervision of the RN and/or LPN Complete required annual medication training Provide resident care to ensure care needs are met according to the resident's care plan Maintain medical records and written documentation assuring accuracy, completeness and compliance with federal and state regulations, certification standards, legal and ethical standards. Maintain the comfort, privacy, and dignity of residents in the deliver of services to them Qualifications Successful completion of Medication Technician training and periodic competency testing Ability to perform medication administration in accordance with State Regs Strong Communication Skills Observes & reports changes in residents' physical condition, cognition & emotional status to Supervisor. Knowledge of, and ability to, adhere to HIPAA policies Must have a high school diploma or equivalent Minimum of six (6) months experience working with seniors in Personal Care, Home Health, Hospital, or Long-Term Care environment Able to work weekends and holidays Physical Abilities While performing the duties of this job, the associate is often required to stand, walk, sit, use fine and gross motor skills, reach with hands and arms, balance, stoop, kneel, crouch, talk, hear, and smell. An individual in this position will be required to lift or carry weight in up to 50 lbs. Intermittent physical activity includes lifting and supporting residents. The associate must use proper body mechanics.
